Gerpevir – capsules, lyophilisate, suspension, tablets

Pharmacodynamics and pharmacokinetics

This remedy is especially effective against herpes and shingles . In addition, it acts against cytomegalovirus and Epstein-Barr virus . The medicine interacts with enzymes and prevents their reproduction.

After penetration into infected cells, the active substance is converted into acyclovir monophosphate , and then successively into acyclovir diphosphate and acyclovir triphosphate . The latter prevents the synthesis of viral DNA. It has almost no effect on healthy cells.

In the case of herpes, the drug prevents the spread of the rash, accelerates the formation of a crust, reduces the risk of visceral complications and skin dissemination , and reduces the severity of pain during exacerbation of herpes zoster . Gerpevir also has immunostimulating properties.

The tablets are partially absorbed after oral administration (approximately 20%). Their active substance penetrates well into tissues and liquids, as well as into breast milk.

The maximum content of the drug depends on the dosage. On average, it occurs 90-120 minutes after administration. The half-life is approximately 3 hours, and in case of renal failure - 19.5 hours.

The drug is excreted mainly through the kidneys unchanged. With age, elimination time increases.

When applied topically, Gerpevir ointment is almost not absorbed into the systemic circulation. The systemic effect is not significant. When using the drug, a therapeutic concentration is formed on the affected area of the skin, which remains constant for at least 5 hours.

Interaction with other drugs

No clinically important interactions of acyclovir with other drugs have been identified.

Acyclovir is excreted predominantly unchanged by the kidneys by tubular secretion, therefore any drugs that have a similar mechanism of excretion may increase the concentration of acyclovir in the blood plasma.

Probenecid and cimetidine increase the half-life of acyclovir and the area under the concentration/time curve. When used simultaneously with immunosuppressants in patients after organ transplantation, the plasma level of acyclovir and the inactive metabolite of the immunosuppressive drug also increases, but given the wide therapeutic index

acyclovir, no dose adjustment is necessary.
