Introduction to the drug, dosage form and composition

Elenium is sold in coated tablets. One cardboard package of the drug consists of 2 blisters of 25 tablets.

The active component of Elenium is the anxiolytic chlordiazepoxide.

Each tablet contains 10 mg of the main component, the core also consists of:

  • sodium carboxymethyl starch;
  • Talc;
  • Gelatin;
  • Lactose monohydrate;
  • Potato starch;
  • Magnesium stearate;
  • Polysorbate 80.

The protective film of the capsule is made of:

  • Maltodextrin;
  • Green varnish based on dyes;
  • Polyvinyl alcohol;
  • Sahara;
  • Talc;
  • Titanium dioxide.

The drug is sold only by prescription, as it can cause addiction and a negative reaction from the body when you stop taking it. For these reasons, Elenium should only be taken under the strict supervision of a healthcare professional.

The drug is produced in the form of small coated tablets. Elenium is a round tablet, convex on both sides. The medicine is covered with a green film. If the capsule is broken, inside we will see a compressed powder with a white or creamy tint.

When choosing, pay attention that the tablets do not have microdamages in the form of cracks and chips of the enamel, as well as stains on the shell.

Pharmacology and mechanism of action of the drug

The tablets are psycholeptic anxiolytics, benzodiazepine derivatives, which in a more understandable language means tranquilizers. The drug has a local effect on certain structures of the central nervous system, suppressing the reactions of specific brain neurons.

How the tablets work:

  1. Relaxes muscles, reducing their tone;
  2. Helps you fall asleep, ensuring a normal duration of rest;
  3. Eliminate feelings of fear and anxiety;
  4. Calm the central nervous system, reducing irritability;
  5. Stops tremors and convulsions.

The drug acts quite quickly and has a cumulative effect. The medicine affects cellular GABA receptors, as a result, the spread of involuntary automatic reactions of the body is blocked.

The tablets dissolve in the gastrointestinal tract within a few minutes, but the peak of their effect comes an hour after taking the capsule. The drug works for quite a long time, as it is eliminated from the body in about 30 hours.

Elenium has a certain number of indications for which this drug is prescribed for use only by a doctor.

Who should take it?

Doctors prescribe sedative tablets for the treatment of mental disorders or to relieve neurotic one-time reactions. Judging by the reviews, this medication helps to cope with feelings of anxiety before surgery and is often used as part of anesthetics.

The doctor prescribes this drug:

  • People with sleep disorders;
  • For certain types of depression;
  • For hyperkinesis (rheumatoid twitching based on neurosis);
  • Women during PMS;
  • For the treatment of convulsive syndromes and tremor;
  • During preparation for surgery;
  • Patients suffering from neurocirculatory dystonia;
  • For neuroses and hysteria;
  • As an aid to general therapy aimed at treating radiculitis;
  • For nervous tension, anxiety and irritability;
  • Women during menopause with complex disruption of the body systems;
  • Children with behavior disorder;
  • To relieve withdrawal symptoms (in simple terms - withdrawal symptoms).

Similar means

The medicine does not have generics, but on the pharmaceutical market there are its analogues for therapeutic and pharmacodynamic effects. These include:

  1. Diazepam is a Ukrainian medicine produced by TDV Interchem. It is also a benzodiazepine derivative and an anxiolytic. The active ingredient is diazepam. Available in solid and liquid forms. The mechanism of action is similar to Elenium. It is used to treat the adult population in the presence of anxiety, critical insomnia, muscle spasms, and for the purpose of premedication during surgical interventions.
  2. Gidazepam is an anxiolytic, also produced by. The active ingredient is gidazepam. It combines a combination of psycholeptic and antidepressant effects. Used as a daytime tranquilizer for asthenic conditions accompanied by fear and anxiety.
  3. Zolomax is an alprazolam drug. It has a wider selection of dosages: from 0.25 mg to 1 mg. Has anticonvulsant, hypnotic, muscle relaxant effects. Has powerful sedative properties. The mechanism of these processes is similar to that discussed above. Zolomax is indicated for panic disorders, insomnia, and anxiety states of any etiology.

When is the drug contraindicated?

The medication may be prohibited if you are hypersensitive to the composition of Elenium or if an allergic reaction to the drug occurs.

Tablets are also contraindicated:

  1. In case of acute disturbance of gas exchange in the lungs (respiratory failure) and respiratory dysfunction;
  2. People with sleep apnea syndrome;
  3. Patients suffering from all kinds of phobias or obsessive states;
  4. For chronic mental disorders;
  5. For muscle fatigue (myasthenic syndrome).

Indications and contraindications

Elenium is used in psychiatric practice for depressive and anxious changes of any origin. The current indications for use are as follows:

  • psychoorganic syndrome with concomitant anxiety disorder;
  • psychotic symptoms of any disease;
  • anxiety disorders;
  • acute phase of alcohol withdrawal syndrome;
  • hypertonicity of muscle tissue;
  • sleep disturbances accompanied by feelings of fear and anxiety.

Anxiety associated with problems in everyday life is not an indication for the use of the drug. It is also prohibited to prescribe Elenium if you have:

  • hypersensitivity to the components of the drug;
  • acute respiratory disorders associated with respiratory failure and depression of the respiratory center;
  • chronic psychosis-like seizures.

Elenium in obstetrics can be used exclusively for strict indications, in cases where the appointment of an alternative and safer remedy is not possible. Taking medication during the first and second trimesters of pregnancy can lead to congenital pathology in the fetus, and also threatens the development of drug dependence in the newborn child. The use of a tranquilizer in the last trimester of pregnancy, regardless of the dose of the drug, causes a decrease in the muscle activity of the newborn and the disappearance of the sucking reflex. Women planning pregnancy should also avoid prescribing Elenium.

Since the active component tends to pass into breast milk, women on anxiolytic therapy should stop breastfeeding. In pediatric practice, Elenium is used from the age of 4 years.

Instructions for use and dosage

The minimum dosage of Elenium for adults is 5–10 mg of the substance at a time, with a daily component requirement of 20 g (2–4 doses). If necessary, the dosage of the drug can be increased to 50 mg per day. This should be done gradually, strictly according to the instructions, 5 mg per day.

Instructions for using these tablets and basic rules for taking them:

  1. For sleep disturbances and anxiety, use the minimum daily dose (10–20 mg) of the drug an hour before bedtime.
  2. For people with mental disorders or disorders, 50 to 100 mg per day is initially indicated. Gradually increase the dosage to 300 mg, distributing the amount by 3 to 4 times.
  3. In case of withdrawal syndrome, a dosage greater than the above is allowed.
  4. The daily intake of the substance for preschool children is from 5 to 10 mg, and for school-age children – up to 20 mg. Adolescents under 18 years of age, if indicated, should receive from 20 to 30 mg of the drug, the same as in other cases, in 3 to 4 doses.
  5. For parenteral use, the dosage for adults is 50–100 mg of the drug at a time.

For people weakened after illness or unhealthy, the dosage of the drug is controlled by a doctor. Do not stop taking the medication abruptly, but gradually reduce the dose.

Important: the higher the daily dosage of the drug, the greater the number of doses per day should be.

Instructions for medical use of the drug

Description of pharmacological action

A tranquilizer from the group of benzodiazepines. It has an anxiolytic, sedative, hypnotic, anticonvulsant, and central muscle relaxant effect. The mechanism of action is associated with an increase in the inhibitory effect of GABA in the central nervous system due to increased sensitivity of GABA receptors to the mediator as a result of stimulation of benzodiazepine receptors.

Indications for use

Neuroses, sleep disorders, nervous tension, anxiety, irritability, reactive depression, withdrawal syndrome, convulsive state or its threat, NCD, menopausal syndrome, premenstrual tension syndrome, for preparation for operations.

Release form

film-coated tablets 10 mg; blister 25, box (box) 2; Composition: 1 tablet contains chlordiazepoxide 10 mg; 25 pcs in a blister, 2 blisters in a box.

Pharmacodynamics

A tranquilizer from the group of benzodiazepines. It has an anxiolytic, sedative, hypnotic, anticonvulsant, and central muscle relaxant effect. The mechanism of action is associated with an increase in the inhibitory effect of GABA in the central nervous system due to increased sensitivity of GABA receptors to the mediator as a result of stimulation of benzodiazepine receptors.

Pharmacokinetics

After oral administration, chlordiazepoxide is almost completely absorbed from the gastrointestinal tract. After intramuscular administration, absorption is slow. Plasma protein binding is high - about 96%. Chlordiazepoxide penetrates the BBB, is excreted in breast milk, and penetrates the placental barrier. Metabolized to form pharmacologically active metabolites desmethyldiazepam, desmethylchlordiazepoxide, demoxepam and oxazepam. T1/2 of chlordiazepoxide is variable and ranges from 5-30 hours, T1/2 of the main active metabolite (desmethyldiazepam) is several days. It is excreted in the urine unchanged and in the form of conjugated metabolites.

Use during pregnancy

Chlordiazepoxide should not be used during the first trimester of pregnancy unless absolutely necessary. It should be borne in mind that when using chlordiazepoxide during pregnancy, a significant change in fetal heart rate is possible. When used in obstetrics in doses recommended to facilitate childbirth, temporary muscle hypotension, hypothermia, and respiratory failure are possible in newborns, more often in premature infants. If regular use is necessary during lactation, breastfeeding should be discontinued. The use of chlordiazepoxide in neonates should be avoided.

Contraindications for use

Myasthenia gravis, acute respiratory failure, severe chronic hypercapnia, phobias, obsessive states, chronic psychoses, a history of alcohol or drug dependence (except acute withdrawal), hypersensitivity to chlordiazepoxide and other benzodiazepines.

Side effects

Drowsiness, fatigue, fatigue, memory loss, impaired concentration, dizziness, incoordination, decreased libido; rarely - nausea, diarrhea, agranulocytosis, jaundice, paradoxical reactions (psychomotor agitation, insomnia, muscle tremors), allergic skin manifestations.

Directions for use and doses

Orally, for adults, usually - in cases of anxiety - 5-10 mg 2-4 times a day, in severe cases (especially with alcoholic psychosis) - up to 40-100 mg / day in divided doses; as a sleeping pill - 10–20 mg 1 hour before bedtime; children: 4–7 years old — 5–10 mg/day, 8–14 years old — 10–20 mg/day, 15–18 years old — 20–30 mg/day in 3–4 doses.

Interactions with other drugs

Antiepileptic and antihistamine drugs, neuroleptics, narcotic analgesics and hypnotics enhance the effect; prolong - cimetidine, disulfiram, oral contraceptives.

Precautions for use

Prescribe with caution in case of insufficiency of liver and kidney function (dose reduction required); patients over 65 years of age (half the usual dose is recommended). Should not be used during work by vehicle drivers and people whose profession involves increased concentration of attention. It must be borne in mind that long-term use (for several months) can lead to addiction and drug dependence. To avoid the development of withdrawal syndrome (excitement, insomnia, loss of appetite), treatment is stopped gradually. During the course and for 3 days after its completion, you should not drink alcohol.

Special instructions for use

Use with extreme caution in patients with cardiac and chronic respiratory failure, organic changes in the brain (in such cases it is recommended to avoid parenteral administration of chlordiazepoxide), with angle-closure glaucoma and predisposition to it, with myasthenia gravis, with chronic kidney and/or liver diseases. Particular caution is required when using chlordiazepoxide, especially at the beginning of treatment, in patients who have been receiving centrally acting antihypertensive drugs, beta-blockers, anticoagulants, and cardiac glycosides for a long time. When discontinuing therapy, the dose should be reduced gradually. If chlordiazepoxide is suddenly discontinued after prolonged use, anxiety, agitation, tremor, and convulsions may occur. With prolonged use in high doses, addiction and drug dependence may develop. Chlordiazepoxide should be discontinued if paradoxical reactions develop (acute agitation, anxiety, sleep disturbances and hallucinations). Avoid intravenous administration. During the treatment period, avoid drinking alcohol. Effect on the ability to drive vehicles and operate machinery Patients taking chlordiazepoxide should refrain from engaging in potentially hazardous activities that require increased attention and rapid psychomotor reactions.

Specifics of use and special recommendations

If characteristic signs and typical indications are present, Elenium is taken in the standard way.

For patients with liver and kidney diseases and people over the age of 65, this medicine is prescribed, but with caution. The dosage under these circumstances is usually reduced by half.

The medicine also has its limitations:

  • Doctors do not recommend using Elenium for people whose work requires brain activity, attention and concentration.
  • When driving, it is better to avoid these tablets.
  • The drug causes dependence and addiction during long-term treatment.

Advice: in order to avoid a negative reaction of the body when discontinuing the drug, expressed in nervous excitability, sleep disturbance and loss of appetite, you should stop taking it, gradually reducing the dosage.

Shelf life and storage conditions

The maximum shelf life of the product in its original packaging, subject to storage conditions, is 4 years.

The drug should be stored in a dark, dry place at room temperature (just select a shelf in the closet).

Keep medications in a secluded place, out of reach of small children and pets, and do not use tablets after the expiration date. Expired tranquilizers are not only ineffective, but also dangerous.

Pregnancy and lactation

Pregnant women, especially those with minimal pregnancy, should avoid taking chlordiazepoxide. A doctor can prescribe this drug only if absolutely necessary.

Taking the drug by the expectant mother may cause changes in the fetal heart rate.

Sometimes Elenium is prescribed in obstetrics to facilitate labor. In such cases, infants may be at risk of temporary hypothermia, respiratory problems, and decreased muscle tone. This is especially true for premature babies.

If a nursing mother needs long-term treatment with the drug, breastfeeding should be stopped.

Attention: the drug is strictly prohibited for use by newborn children.

Side effects

Associated reactions and the frequency of their occurrence directly depend on the dosage of the drug and the individual susceptibility of the body to its individual components. Similar phenomena occur mainly after incorrect cessation of medication and pass quite easily.

Major additional unexpected drug reactions:

  1. In the cardiovascular system, a decrease in blood pressure, pain in the heart area and an abnormal pulse rate may be observed.
  2. In relation to the central nervous system, drowsiness and slow reaction, disorientation and confusion, dizziness and headaches, and loss of coordination of movements sometimes appear. These changes in the body can be observed at the beginning of the course of treatment in elderly patients, but there is no need to worry too much, since the phenomena stop while taking the drug. If such reactions intensify, it is enough to reduce the dosage of the drug, and the symptoms will not be so pronounced. In some cases (when taking the drug in large doses), patients may experience inexpressiveness of speech, incorrect pronunciation, memory impairment, and impaired sexual desire. These phenomena occur when taking other benzodiazepines.
  3. As for lymph, when taking Elenium, hemolysis (destruction of red blood cells) of the blood may occur.
  4. In the organs of vision, disorders and disturbances in visual function may occur in the form of blurred or double images.
  5. In the area of the kidneys and urinary system, a reaction often occurs in the form of urine retention or incontinence.
  6. Constipation, dry mouth, nausea and indigestion may occur in the digestive system. Loss of appetite or its complete absence is also possible.
  7. Tremors and weakness of muscle tissue may appear in the musculoskeletal system.
  8. In terms of immunity, a sharp outbreak of an allergic reaction (anaphylactic shock) may develop, but this phenomenon is quite rare.
  9. Among the general disorders, weakness of the body is observed, fainting is possible.
  10. A rash like hives may appear on the skin.
  11. Jaundice sometimes occurs in the biliary tract and liver (due to impaired liver function), as well as a violation of metabolic processes within organs.
  12. Women often experience irregularities in their menstrual cycle.
  13. Mental abnormalities include insomnia, akinetic phenomena, aggression and excitability, partial memory loss and depressive syndrome. Nightmares, inadequacy, convulsions, tremors and even hallucinations also often occur.

The most strange reactions often occur in older people, patients who drink alcohol, and those with mental pathologies.

Dependence on chlordiazepoxide can develop even from a therapeutic dosage of the drug. In most cases, addiction occurs in people who combine medication with large amounts of alcohol. Abruptly stopping the use of the medicine is fraught with withdrawal syndrome.

Consequences of use: addiction, impact on the psyche

Damage caused by abuse of Elenium to vital organs and systems:

  1. Nervous system: muscle weakness, depression, visual impairment, diplopia. Fear, anxiety, confusion, tremors, hallucinations.
  2. From the gastrointestinal tract: nausea, dry mouth, constipation, jaundice.
  3. Genitourinary system: urinary incontinence.
  4. Cardiovascular: changes in blood pressure.
  5. Respiratory: respiratory rhythm disturbances when administered parenterally.
  6. Allergy – a rash on the skin rarely appears.

What are the consequences of dependence on Elenium?

  • severe diseases of the cardiovascular system develop;
  • possible suicide attempts;
  • complete loss of sense of reality;
  • uncontrolled, inappropriate behavior, fraught with accidents;
  • rapid wear and tear of orgasm, which leads to a reduction in life expectancy.

What are the dangers of overdose?

The main signs of an overdose are impaired consciousness, pronounced drowsiness, and loss of muscle tone. If the dosage is severely abused, a hypotonic coma with collapse and reflex complications is possible.

Other symptoms:

  • A sharp and severe drop in blood pressure;
  • Fainting;
  • Bradycardia.

If the daily norm is exceeded and symptoms indicate this, it is necessary to urgently empty the stomach before the entire portion of the drug has yet been absorbed. You can use artificial methods or activated carbon.

Important: when eliminating the symptoms of an overdose, it is important to monitor the patient’s pulse, breathing and blood pressure.

As an antidote, you can use Flumazenil, a detoxifying drug used for intoxication of the body with toxic substances. The medication is created only for use in emergency cases and is administered intravenously.

However, this drug does not last as long as Elenium, so it is important to monitor blood pressure, respiratory function and pulse for 30 hours after taking the pill, so that if overdose symptoms recur, an antidote can be administered in a timely manner.

The patient should not be given sedatives even if he is excitable during an overdose of chlordiazepoxide. The feasibility of the method in this case has not been established.

Interaction with other drugs

Drug interactions can benefit or harm the body. It all depends on the type of drugs themselves.

Namely:

  1. In combination with analgesics, antidepressants, neuroleptics and ethanol, chlordiazepoxide has a depressant effect on the central nervous system. The same effect will occur when Elenium is combined with sleeping pills and sedative tablets, as well as anesthesia.
  2. Antacids in combination with the substance slow down the absorption of the active ingredient, but the absorption of the drug is not inhibited.
  3. With hormonal contraceptives, the effect of chlordiazepoxide is enhanced. In this case, frequent bleeding is possible.
  4. The effectiveness of muscle relaxants is enhanced when Elenium is used with them. Levodopa in combination with a tranquilizer may cause symptoms of parkinsonism.

Important: when Elenium is combined with cyclophosphamide, the toxicity of the latter increases.

Drug interactions

When choosing a treatment regimen, it is necessary to take into account the compatibility of the medication with other substances:

  1. The combined use of Elenium with medications from the groups of neuroleptics, sleeping pills, antihistamines, and antidepressants helps to potentiate the mechanism of action of the tranquilizer.
  2. When combined with antiepileptic drugs, a large number of side effects occur and the toxicity of the active substance increases.
  3. Hormonal contraceptives increase the pharmacodynamic effect of chlordiazepoxide. This increases the risk of breakthrough bleeding.
  4. Antacids slow down the absorption of the active component of Elenium.
  5. Phentoin and Phenobarbital help accelerate the metabolism of chlordiazepoxide.
  6. Combined use with Levodopa minimizes the antiparkinsonian effect of the latter.
  7. Elenium increases the effectiveness of muscle relaxants.
  8. It is prohibited to combine Elenium with alcoholic beverages, smoking, or narcotic substances, since the inhibitory effect on the nervous system is enhanced.

Prices and analogues

Elenium has different prices in pharmacies. It depends on the commercial focus of the institution. On average, the cost of the drug varies from 400 to 550 rubles per package.

There are drugs that have the composition of Elenium, but are produced under other trade names, such as:

  • Napotona;
  • Chlozepida;
  • Radepura.

There are also plenty of drugs in the pharmacy chain with similar effects on the body to Elenium. They are called drug analogues. This:

  • Sibazon;
  • Seduxen;
  • Tazepam;
  • Relanium;
  • Nozepam.

Important: if for any reason (for example, intolerance to a substance) it is necessary to replace Elenium with any drug similar in action, you should look for tablets among benzodiazepines.

Do not forget that analogues have a completely different dosage, regimen of use, as well as side effects and contraindications, so replacing the drug is the responsibility of the doctor.

Reviews about the use of Elenium

The drug is widely used in medicine, so there are various reviews about its effectiveness. In most cases they are positive. Although sometimes people indicate the presence of side effects.

Doctors

Vitaly Kovalev, psychiatrist: “I discovered Elenium 5 years ago at a scientific and practical conference. Then for the first time I listened to the assistant professor’s report on the effectiveness of this remedy. After my first practical experience of using it, I was surprised by the rapid positive dynamics. Most of my patients do not experience remission of the disease after treatment with Elenium.”

Evgeniy Ashman, psychiatrist: “An excellent drug for both monotherapy of reactive depression and complex treatment. I have never observed any side effects.”

Patients

Irina Bezverkhnyaya, 28 years old: “I suffered from a severe form of insomnia, I didn’t sleep for days. I went to doctors for a year, tried dozens of different remedies - some helped, some didn’t, but there was no lasting effect. While taking Elenium, I felt for the first time in a long time what proper sleep was, and no new disturbances had arisen for a month.”

Natalya, 39 years old: “I was treated for alcohol addiction. Treatment was voluntary. I liked the fact that after a course of Elenium, the withdrawal syndrome went away without a trace and I don’t feel like drinking alcohol.”

Stanislav, 64 years old: “Elenium helped normalize the condition after a mini-stroke. The drug relaxes muscles and eliminates tremors of the limbs. True, at the beginning of treatment I felt a little nauseous.”

The significant share of the drug Elenium in the total sales volume of the group of psycholeptics, which has persisted over the past several years, confirms the high degree of confidence of consumers and the medical community in this drug. Numerous studies have confirmed the effectiveness and safety of the drug in the treatment of anxiety disorders of various etiologies.

Opinions about the drug

The drug Elenium is considered a highly effective remedy. Numerous reviews convince of its high effectiveness.

Ivan, 34 years old, Orenburg. I have been suffering from insomnia for many years. Over the course of my life, I tried many different ways to get rid of the problem, but I didn’t feel much effect. I decided to seek help from doctors. After the examination, I was prescribed Elenium. I just started taking the drug, but the changes are obvious: just one tablet two hours before bed is enough to sleep peacefully all night and not wake up. I hope that this drug will also help me well in the future.

Alina, 40 years old, Omsk. Lately I have noticed increased nervousness and irritability. At first I attributed the condition to PMS, but when the syndrome dragged on, I realized that something was wrong with me and went to the outpatient clinic. I was diagnosed with mild depression and prescribed Elenium. Just a few days after starting the course, I felt much better, but to consolidate the results, I need to complete the full course of therapy, so I will continue to take these “miracle pills.”

Alina, 39 years old, St. Petersburg. After a serious illness, I began to have nightmares that ended in insomnia. I consulted a neurologist and psychotherapist and underwent a complete examination. To improve sleep and eliminate mental problems, I was prescribed Elenium. I took the pills for about a week, in minimal doses. During this time, I began to sleep normally, but I began to have headaches, and sometimes even double vision. The drug was immediately stopped for me and an analogue was prescribed. I don't know if this is due to the change in medication, but the symptoms have disappeared and I feel great now.
