Spironolactone

Spironolactone belongs to the group of potassium-sparing diuretics. Produced by many pharmaceutical companies in the form of tablets, the type of packaging may vary. The main active ingredient is also called spironolactone.

The product is widely used:

  • to eliminate swelling in patients suffering from chronic heart failure, cirrhosis of the liver, during pregnancy (in the 2nd and 3rd trimester);
  • with increased secretion of aldosterone by the adrenal glands;
  • hypertension;
  • in the treatment of polycystic ovary syndrome and premenstrual syndrome.

A positive property of the medication is that when excess fluid is removed from the body, there is no disturbance in the electrolyte balance of the blood plasma. Spironolactone is a mild diuretic, the effect after starting treatment appears after 2-5 days.

It has a number of undesirable side effects: gynecomastia, impotence, hirsutism, hyperkalemia, menstrual irregularities.

Pharmacodynamics and pharmacokinetics

According to its pharmacological properties, the drug is close to thiazide diuretics . Increases the concentration of sodium, chlorine, potassium and magnesium ions in the urine. Increases the elasticity of arterial walls, gently reduces the resistance of peripheral vessels. Does not affect carbohydrate metabolism and lipid in the blood, helps reduce hypertrophy of the left ventricle of the heart .

Indapamide is a stimulator of prostaglandin E2 and has a significant effect on the production of free oxygen radicals.

The medicine begins to act 30 minutes after administration (bioavailability is about 93%), the therapeutic effect lasts for 24 hours. The maximum concentration in the blood is 12 hours after the tablet is dissolved in the gastrointestinal tract. The half-life is 18 hours. Eating may slightly prolong the absorption time, but the drug is nevertheless completely absorbed. The kidneys excrete up to 80% of the substance in the form of metabolites , the intestines – up to 20%.

When is the best time to take it?

Inspra

Inspra is also a potassium-sparing diuretic. The drug effectively removes excess fluid, removes swelling, while preventing the loss of a microelement, potassium, necessary for the full and proper functioning of the heart muscle.

Inspra is produced in tablet form by PFIZER, Inc. (Pfizer), USA. The active ingredient is eplerenone, containing 25 mg in pills. Created later than spironolactone and has fewer side effects. No dose adjustment is required, which is important for therapy for people with diabetes, the elderly and those with reduced renal function.

Side effects

The drug, as a diuretic, can lead to a decrease in potassium content in the blood serum, a decrease in sodium concentration, which leads to dehydration of the body . In this regard, symptoms such as dry mouth , constipation , nausea , headache , allergic reactions .

Rare side effects are cardiac arrhythmias , hemolytic anemia .

special instructions

Often, after stabilizing the pressure and feeling well, patients stop using medications.

This is a wrong decision; doctors advise taking Indapamide constantly.

Reducing the dosage or completely stopping the medication should only be done under the supervision of a physician.

An important step is regular blood tests for creatinine, potassium, and other indicators that will interest the doctor.

In the first week of taking Indapamide, you should avoid working with dangerous machinery or driving a car. When you are sure that the medicine does not have negative effects, return to your previous lifestyle. The drug can be used regularly for six to nine months, after which you can consult your doctor for recommendations.

Indapamide tablets, instructions for use (Method and dosage)

The tablets are taken strictly in accordance with the instructions - once a day, preferably in the morning, one tablet or capsule.

The medicine can be combined with other antihypertensive drugs, but only the attending physician can determine how to take the drugs in various combinations.

Instructions for use of Indapamide Retard and instructions for use of Indapamide MV Stada (made in Germany) do not contain differences regarding the conditions of administration and dosage. However, the drug Retard has a longer and, at the same time, milder effect of the reagent due to the slow release of the active substance.

The doctor decides how long Indapamide can be taken, taking into account the stage of hypertension , but in medical practice this drug refers to medications that are prescribed for a long period of time (including for life).

How long does the treatment last?

When patients with hypertension are prescribed antihypertensive drugs, the course of treatment lasts several weeks. After your blood pressure normalizes, you can stop taking it.

The attending physician can resolve the issue. To prevent a rebound increase in blood pressure, the patient must follow proper nutrition and all the instructions of the attending physician.

The duration of the course is determined by the doctor. Treatment may last differently for each patient. All this depends on the individual characteristics of the body and the degree of hypertension.

Interaction with other drugs

  • Antidepressants and antipsychotics enhance the hypotensive effect and increase the likelihood of developing orthostatic hypotension .
  • Aluretics, cardiac glycosides, and laxatives increase the risk of developing potassium deficiency .
  • Erythromycin can lead to the development of tachycardia with ventricular fibrillation.
  • Nonsteroidal anti-inflammatory drugs and glucocorticosteroids reduce the hypotensive effect.
  • Preparations containing iodine can lead to fluid deficiency in the body.
  • Cyclosporine contributes to the development of hypercreatininemia .

Hypothiazide

Hypothiazide is a first-generation thiazide diuretic drug with moderate diuretic and antihypertensive effects. The active substance is hydrochlorothiazide. They are used both in monotherapy and in combination with drugs from other pharmacological groups. Enhances the excretion of potassium, magnesium, bicarbonate and phosphate ions from the body in the urine. Retains calcium and urate ions.

After oral administration, the effect of the drug occurs very quickly, within half an hour, and lasts for 6-14 hours. The mild effect of Hypothiazide is used to treat the following ailments:

  • hypertension;
  • chronic edema;
  • diabetes insipidus;
  • for the prevention of stone formation in the urinary organs in patients with hypercalciuria.

Hypothiazide can be taken for a long time, but electrolyte levels should be periodically monitored by laboratory blood tests.

It also causes significantly the following negative metabolic side effects:

  • increases the level of cholesterol, low and very low density lipoproteins, triglycerides;
  • reduces glucose tolerance;
  • increases hyperuricemia.

Therefore, some doctors prefer Indapamide as a more advanced drug.

The drug is often prescribed to diabetics when their blood pressure rises. Take the medicine in combination with other tablets.

Many diuretics increase blood sugar levels, which is not the case with Indapamide.

Such cases while taking this medicine are rare. But the patient is still recommended to use the glucometer more often to measure glucose levels. Indapamide is used in combination with other drugs.

Indapamide is an effective and harmless drug for lowering blood pressure in patients with diabetes.

ACE inhibitors and angiotensin II receptor blockers lower blood pressure and protect the kidneys from complications. Patients with diabetes are prescribed Indapamide and Perindopril, which are classified as ACE inhibitors. This combination lowers blood pressure and reduces the risk of cardiovascular complications.

As a result of the action of the drugs, the amount of protein in the urine remains stable and the kidneys do not suffer from complications of diabetes.

Among patients, Noliprel, containing indapamide with perindopril, is in particular demand.

Their goal is to lower blood pressure and maintain it at 135/90 mmHg. Art. When Noliprel does not allow it to be achieved, Amlodipine is added to the medication regimen.

Veroshpiron

The drug Veroshpiron is actually an analogue of Spironolactone, since it contains the same active ingredient in 25, 50 and 100 mg doses, in tablets and gelatin capsules.

The only difference is that Veroshpiron is always produced by one company, GEDEON RICHTER, Plc. (Gedeon Richter) at factories in Hungary or Russia. All of the listed effects of Spironolactone also apply to the drug Veroshpiron.

Overdose

Indapamide, even in very high concentrations (up to 40 mg, i.e. 27 times the therapeutic dose), does not have a toxic effect.

Symptoms of acute drug poisoning are primarily associated with water and electrolyte imbalance (hyponatremia, hypokalemia). Nausea, vomiting, decreased blood pressure, convulsions, dizziness, drowsiness, confusion, polyuria or oliguria leading to anuria (due to hypovolemia) may also occur.

Treatment: urgent measures aimed at removing the drug from the body: gastric lavage and/or administration of activated charcoal, followed by restoration of normal water and electrolyte balance.

Pharmacokinetic characteristics

The blood pressure medication is taken orally. After entering the stomach, the drug is quickly absorbed. Bioavailability is 93%. Eating food simultaneously with the medication reduces the rate of absorption of the components. The maximum concentration is reached 1-2 hours after taking the tablet. The results from taking the drug are consolidated after a week of regular therapy.

The half-life of the components is 18 hours. Metabolites bind to blood proteins by 70-80%. The components of the drug penetrate into breast milk and through the placental barrier.

Metabolites are formed in the liver and excreted by the kidneys.

Mechanism of action of diuretics in hypertension

Doctors' opinion

Practitioners believe that all analogues of Indapamide are quite effective and act approximately the same. The main difference is only in price, manufacturer, composition of auxiliary components and amount of active substance.

Indapamide

The doctor prescribes medication, focusing on the individual characteristics of each person. If there is no therapeutic effect or an adverse reaction occurs to one of the drugs, another one with a similar effect is prescribed.

From the beginning of therapy, the patient should carefully monitor his well-being and, if he notices a deterioration, immediately inform his doctor. Thus, it is not possible to say which of the analogues is more effective, because the active substance is the same in each of them - indapamide.

Torasemide

The drug Torasemide belongs to the group of loop diuretics, like furosemide, but is a medicine of a new generation. It has a number of fundamental differences from furosemide:

  • has a more pronounced diuretic and natriuretic effect;
  • removes potassium to a lesser extent;
  • the antihypertensive effect is not based on a decrease in circulating blood volume, but is caused by vasodilation and the elimination of vasospasm.

These properties allow the use of Torasemide in patients with arterial hypertension. The prolonged form can be combined in therapy with anticoagulants, cardiac glycosides, nitroglycerin and calcium channel blockers. Simultaneous use with potassium-sparing diuretics prevents electrolyte disturbances in the body.

The effect of the medicine begins an hour after taking it. The maximum effect of Torasemide occurs after 2 hours and lasts from 12 to 18 hours.

, and if used incorrectly can cause the development of an overdose and

Indapamide and Furosemide

Acripamide

The product is produced in Russia. Acripamide is a diuretic prescribed for hypertension to lower blood pressure. The main substance that has a therapeutic effect is indapamide (as in the drug Indapamide itself).

Indapamide

Main characteristics:

  • Available in tablet form;
  • dosage: 2.5 mg of active ingredient;
  • group: the medicine is intended to reduce blood pressure in hypertension;
  • subgroup: diuretic;
  • composition: active ingredient, additional ingredients – glycerin, lactose, talc, potato starch, povidone;
  • reception: once a day;
  • price: can be purchased in the price range from 35 to 50 rubles.

The effect of the medication is to increase the excretion of chlorine, magnesium and potassium ions (to the least extent) from the liquid. Selectively blocks calcium channels. Increases the elasticity of arterial tissues, reduces vascular resistance. Reduces left ventricular hypotrophy.

Indapamide

Contraindications include liver disease, kidney disease (in the acute phase), allergies to components, age (less than 18 years). The tablets are not recommended for pregnant women. The lactation period is also a contraindication to the use of Acripamide.

Special recommendations: those who take herbal substances or laxatives, as well as elderly patients, are advised to regularly measure the amount of creatinine and potassium ions. If you are diagnosed with diabetes, you should measure your sugar levels more frequently.

When taking the medication, people suffering from ischemia, heart failure or cirrhosis of the liver are advised to undergo regular examination by their doctor in order to reduce the risk of worsening the condition.

Does the composition affect potency?

The fact that hypertension is a disease that negatively affects potency does not require proof. But hypertensive drugs can also have a temporary negative effect on erectile function. If you take antihypertensive medications for a long time, this option is quite possible. There are no drugs today that both reduce blood pressure and increase potency. And Indapamide in this sense is also not the most beneficial remedy: it has a qualitative effect on blood pressure, but among its side effects it has a decrease in libido and potency. And the duration of the course is not important.

Negative consequences, what to prepare for

Tests have shown that the use of Indapamide for systemic lupus erythematosus can provoke an exacerbation. Also, when using the drug there is a risk of leukemia, thrombocytopenia, anemia, and bone marrow aplasia. Some patients develop agranulocytosis. The use of the drug is associated with the likelihood of an allergic reaction. As a rule, it manifests itself on the skin as itchy areas and rashes. Vasculitis and urticaria are possible.

Patients taking Indapamide have a higher risk of infection of the urinary system. There is a risk of polyuria. There is a greater likelihood of inflammation of the ENT organs. Sometimes the pills cause a cough.

Indapamide can cause nausea and vomiting, and weight loss. Some patients note that while taking the drug they feel headaches and dizziness, dry mouth, stomach pain, and stool problems. There is a risk of liver encephalopathy and pancreatitis, asthenia and increased fatigue. "Indapamide" can provoke a nervous and restless state, a craving for sleep, and an impairment of the ability to maintain balance. In some patients, sleep is disturbed, their condition worsens, and their psycho-emotional status is depressed. There is a risk of muscle spasms, tension, and anxiety. Patients may be irritable while using Indapamide tablets.
