Contraindications

According to the annotation for the drug, the use of Antaxon is contraindicated:

The first stage of Antaxon treatment for heroin addiction is the administration phase.

Therapy begins at least 7 days after the last use of the drug (as determined by urine analysis), provided that all symptoms of withdrawal syndrome are absent.

Next, a so-called provocative test is necessarily carried out with intravenous administration of 500 mcg of naloxone (if there is a need to repeat the test, a 24-hour interval is observed).

As soon as the provocative test shows a negative result, the patient is given the first dose of Antaxon in the amount of 20 mg. After one hour, provided that the patient does not show signs of withdrawal syndrome, another 30 mg is given - the remainder of the daily dose.

The second stage of treatment with Antaxon is maintenance therapy. It is carried out according to one of the following schemes:

  • 50 mg of naltrexone (1 capsule) once a day;
  • 50 mg during the first five days, 100 mg on day 6, then repeat;
  • 100 mg every two days or 150 mg every three days and repeat again.

Patients with a long-term deprivation of narcotic substances are recommended to adhere to the following treatment regimen: on the first and second days of maintenance therapy - 100 mg, then on the fifth day - 150 mg, etc.

Note: one capsule of Antaxon (50 mg of naltrexone) is enough to block the effect of 25 mg of heroin administered intravenously.

The minimum recommended course of maintenance therapy is 6 months, the minimum acceptable is 3 months.

For chronic alcoholism, Antaxon, according to the instructions, is prescribed after a naloxone test, in the absence of signs of withdrawal syndrome: 1 capsule once a day for at least 3 months.

Release form and composition

Antaxone is produced in the form of hard gelatin capsules with a blue cap and an opaque body, which are filled with white powder inside, with the inscription “Antaxone® 50” on the cap and the company’s trademark on the body, 10 pieces each. in blisters.

One capsule contains 50 mg of naltrexone hydrochloride and excipients - magnesium stearate, indigo carmine, lactose monohydrate, gelatin and titanium dioxide.

Antaxon is also produced in the form of a transparent yellowish solution with a characteristic odor, 10 and 20 ml in bottles.

10 ml of the product contains 50 mg of naltrexone hydrochloride and such excipients as:

  • Aromatic bitterness No. 1631;
  • Sorbitol 70%;
  • Methyl parahydroxybenzoate;
  • Propyl parahydroxybenzoate;
  • Saccharin;
  • Purified water.

Side effects

According to numerous reviews, when the doctor-recommended dosages of Antaxon are followed, serious adverse reactions do not occur in patients who do not have opioids in their bodies.

However, in some cases the drug has side effects:

  • In more than 10% of cases, the following are observed: nausea, weakness, pain in the abdomen, muscles and joints, increased irritability, vomiting, anxiety, sleep disturbances;
  • Less than 10% of patients experience: dizziness, diarrhea, loss of appetite, constipation, skin rashes, psychomotor agitation, delayed ejaculation and/or decreased sexual potency;
  • In individual cases, photophobia, hallucinations, increased fatigue, cough, depression, confusion, and nightmares are possible.

When conducting laboratory tests, lymphocytosis and increased transaminase activity are sometimes observed.

It is important to note that all the described negative reactions of the body were observed in patients both during the use of Antaxon and before the start of therapy, and therefore these symptoms cannot be unambiguously considered as side effects of the drug.

Description of pharmacological action

Competitively binds to opioid receptors of all types and prevents or eliminates the effect of both endogenous opioids and exogenous opioid drugs - narcotic analgesics and their surrogates.

Indications for use

Opioid addiction (dependence) - with the aim of maintaining a patient’s condition in which opioids cannot have a characteristic effect (as part of complex therapy). The drug is prescribed only after the withdrawal syndrome has stopped. The use of Antaxon begins in specialized drug addiction treatment units 7–10 days after the last use of an opioid drug. In the future, the patient should be under strict medical supervision. The patient must have a positive attitude toward drug addiction treatment. Alcoholism (as part of complex therapy), including with maintenance therapy in the same dosages as for heroin addiction during psychotherapy.

Pharmacodynamics

It does not have significant pharmacological properties of its own, with the exception of some constriction of the pupil. At a dose of 50 mg, Antaxon blocks the pharmacological effects caused by intravenous administration of 25 mg of heroin for 24 hours. When doubling the dose of the drug, the effect is extended to 48 hours, when tripling it - up to 72 hours. By administering opioids in increased doses, the effect of Antaxon can be weakened or eliminated. Antaxon does not cause addiction or drug dependence.

Pharmacokinetics

It is well absorbed after oral administration and is 95% metabolized in the liver to form pharmacologically active metabolites. The main metabolite, 6-beta-naltrexone, also has the properties of a competitive opioid antagonist. In addition, 2-hydroxy-3-methoxy-6-beta-naltrexone is formed. Cmax of naltrexone and 6-beta-naltrexone in blood plasma is achieved after 1 hour. It penetrates well through histohematic barriers, the volume of distribution is 1350 l. T1/2 for naltrexone is 4 hours, for its metabolite 6-beta-naltrexone is 13 hours, which explains its ability to cumulate. Antaxon and its metabolites undergo enterohepatic recirculation. It is excreted through the kidneys unchanged (about 1%) and metabolized (38% as free and bound 6-beta-naltrexone).

Use during pregnancy

Contraindicated during pregnancy. Breastfeeding should be stopped during treatment.

Contraindications for use

History of hypersensitivity to naltrexone drugs; use of narcotic analgesics or a positive urine test for opioids; lack of data on conducting a provocative test with naloxone (see “Method of administration and dosage”); acute hepatitis or liver failure; age under 18 years (safety of use has not been established); pregnancy, breastfeeding.

Side effects

In therapeutic doses in patients whose bodies do not contain opioids, Antaxon usually does not cause serious side effects. It should be borne in mind that with a frequency of more than 10%, sleep disturbance, anxiety, nervousness, abdominal pain, nausea, vomiting, weakness, headache, pain in muscles and joints are observed; less than 10% - loss of appetite, constipation or diarrhea, psychomotor agitation, dizziness, skin rashes, chills, decreased potency, delayed ejaculation. The noted symptoms occurred with equal frequency both before the start of treatment and during the use of Antaxon. In isolated cases - increased fatigue, confusion, depression, hallucinations and nightmares, photophobia, cough. During laboratory diagnostics, it is possible to increase the level of transaminases in the blood and lymphocytosis. One case of idiopathic thrombocytopenic purpura was described in a patient who may have been sensitized to Antaxon during previous treatment (recovery occurred after discontinuation of the drug and a course of corticosteroid therapy).

Directions for use and doses

The doctor should monitor the act of swallowing the drug solution. Maintenance therapy with Antaxon Antaxon is prescribed 50 mg every 24 hours (this dose is sufficient to prevent the effects of IV heroin administered at a dose of 25 mg). Other treatment regimens can be used: 1. 50 mg daily for the first 5 days of the week and 100 mg on Saturday. 2. 100 mg every 2 days or 150 mg every 3 days. 3. 100 mg on Monday, 100 mg on Tuesday and 150 mg on Friday. This regimen is convenient for patients with long-term opioid deprivation. The minimum course is 3 months, the recommended course is 6 months. Treatment of alcoholism Daily - 50 mg, minimum course - 3 months. Treatment is not started until a naloxone test has been performed to rule out the presence of opioids in the body.

Overdose

If poisoning is suspected, symptomatic therapy should be prescribed and contact a specialized medical institution.

Interactions with other drugs

Some drugs containing opioids (antitussives, antidiarrheals, narcotic analgesics) may reduce the effect of Antaxon. In these cases, alternative medications that do not contain opioids should be used. Hepatotoxic drugs increase the risk of liver damage. Cases of incompatibility with other drugs have not been described.

Precautions for use

The drug should be discontinued at least 48 hours before surgery that requires the use of opioid analgesics. The patient must have an Antaxon prescription card with him for the information of other health care workers in case of emergency care. If it is necessary to overcome the blockade of opioid receptors (anesthesia, pain relief in emergency cases), increased doses of short-acting opioid analgesics should be used to reduce the risk of respiratory and circulatory depression.
