Release form and composition

Dalacin is available in the following dosage forms:

  • Gel for external use 1%: transparent, colorless, viscous (30 g in tubes, 1 tube in a cardboard box);
  • Vaginal cream 2%: white (20 or 40 g in tubes made of polyethylene or aluminum foil, complete with 3 or 7 applicators (respectively), 1 set in a cardboard box);
  • Capsules: gelatin, hard, with a white body and cap, labeled “P&U 225” (150 mg each) or “P&U 395” (300 mg each); contents of capsules – white powder (8 or 10 pieces in blisters, 2 or 10 blisters in a cardboard box);
  • Vaginal suppositories: torpedo-shaped, hard, with a smooth surface, almost white or white (3 pieces in strips of laminated foil with or without an applicator, 1 strip in a cardboard box).

Composition of 1000 mg gel for external use:

  • Active substance: clindamycin (in phosphate form) – 10 mg;
  • Auxiliary components: methylparaben – 3 mg; allantoin – 2 mg; polyethylene glycol – 100 mg; propylene glycol – 50 mg; carbomer 934P – 7.5 mg; 40% sodium hydroxide solution - in sufficient quantity; purified water - in sufficient quantities up to 1000 mg.

Composition of 1000 mg vaginal cream:

  • Active substance: clindamycin (in phosphate form) – 20 mg;
  • Auxiliary components: cetyl palmitate – 32.1 mg; stearic acid – 21.4 mg; polysorbate 60 – 50 mg; sorbitan monostearate – 20 mg; propylene glycol – 50 mg; benzyl alcohol – 10 mg; cetostearyl alcohol – 32.1 mg; mineral oil – 64.2 mg; purified water - in sufficient quantity.

Composition of 1 capsule:

  • Active substance: clindamycin (in the form of hydrochloride) – 150 or 300 mg;
  • Auxiliary components: talc, lactose, corn starch, magnesium stearate, gelatin, titanium dioxide.

Composition of 1 vaginal suppository:

  • Active substance: clindamycin (in phosphate form) – 100 mg;
  • Auxiliary components: solid fat (Witepsol H-32, a mixture of monoglycerides, diglycerides and triglycerides) - about 2400 mg.

Indications for use

Dalatsin in the form of capsules is prescribed for the treatment of the following infectious and inflammatory diseases caused by anaerobic bacteria sensitive to the action of the active substance or strains of gram-positive aerobic bacteria (Streptococcus spp., Pneumococcus spp. and Staphylococcus spp.), as well as Chlamydia trachomatis serovars that are sensitive to clindamycin:

  • Scarlet fever, inflammation of the middle ear;
  • Upper respiratory tract infections, including pharyngitis, tonsillitis, sinusitis;
  • Infectious diseases of soft tissues and skin, including furunculosis, impetigo, acne, infections of subcutaneous fat, infected wounds, abscesses, specific infectious processes in soft tissues and skin caused by pathogens sensitive to the action of Dalatsin (erysipelas and felon (paronychia));
  • Lower respiratory tract infections, including pneumonia, bronchitis, lung abscess and pleural empyema;
  • Gynecological infectious diseases, including endometritis, infections of the tissues surrounding the vagina (abscesses of the fallopian ovaries and tubes, subcutaneous fat), inflammatory diseases of the pelvic organs and salpingitis (simultaneously with antibacterial drugs active against gram-negative aerobic pathogens, for example, gentamicin);
  • Infectious diseases of the joints and bones, including septic arthritis and osteomyelitis;
  • Infectious diseases of the abdominal cavity, including abdominal abscesses and peritonitis (simultaneously with other antibacterial drugs affecting gram-negative aerobic bacteria);
  • Infectious diseases of the cervix caused by Chlamydia trachomatis (as monotherapy);
  • Oral infections (periodontitis and periodontal abscess);
  • Endocarditis and septicemia;
  • Malaria, incl. caused by multidrug-resistant Plasmodium falciparum (as monotherapy or simultaneously with chloroquine or quinine);
  • Toxoplasmosis encephalitis in patients with AIDS (simultaneously with pyrimethamine if standard therapy is intolerant);
  • Pneumocystis pneumonia in patients with AIDS (simultaneously with primaquine in case of intolerance to standard therapy).

The drug is also used to prevent endocarditis in patients with hypersensitivity to penicillins.

Dalacin in the form of vaginal cream and suppositories is prescribed for the treatment of bacterial vaginosis.

Dalacin in the form of a gel for external use is prescribed for the treatment of acne vulgaris.

Contraindications

Dalacin is contraindicated for use in the presence of hypersensitivity to clindamycin, lincomycin or auxiliary components of the drug.

The drug in capsule form should be taken with caution by patients with gastrointestinal diseases (history), especially ulcerative colitis, myasthenia gravis (possibly impaired neuromuscular transmission), severe liver failure.

Additionally, Dalacin in other dosage forms is contraindicated for use in the following diseases/conditions:

  • history of antibiotic-associated colitis;
  • Age: up to 18 years – for vaginal cream and suppositories; up to 12 years - for gel for external use (there are no data on the effectiveness and safety of using Dalatsin in this group of patients);
  • History of hypersensitivity to lincomycin or clindamycin (for gel for external use).

Directions for use and dosage

Dalacin in capsule form is taken orally. To avoid complications, it is recommended to use it strictly as prescribed by your doctor.

To avoid irritation of the esophageal mucosa, capsules should be taken with a full glass of water.

The drug is usually taken:

  • Adults: 600-1800 mg per day, frequency of administration – 2-4 times (in equal doses);
  • Children: 8-25 mg/kg body weight per day, frequency of administration – 3-4 times (in equal doses).

In the treatment of infections caused by beta-hemolytic streptococcus, Dalacin is used in the doses described above. The duration of the therapeutic course is at least 10 days.

Treatment of infectious and inflammatory diseases of the pelvic organs begins with intravenous administration of clindamycin, at a dose of 900 mg every 8 hours, simultaneously with a suitable antibacterial agent for at least 4 days. After the patient’s condition improves, therapy is continued for at least 2 more days, after which Dalatsin is prescribed in the form of capsules in a single dose of 450-600 mg. The drug is taken daily every 6 hours. The full course of treatment is 10-14 days.

Depending on the indications, Dalacin is prescribed:

  • Toxoplasmosis encephalitis in patients with AIDS: in case of intolerance to standard therapy, the drug is taken simultaneously with pyrimethamine according to the following regimen: Dalacin - 600-1200 mg every 6 hours for 14 days, then 300-600 mg with the same frequency of application; pyrimethamine – 25-75 mg daily. As a rule, the treatment course is 8-10 weeks. When using pyrimethamine in higher doses, 10-20 mg of folic acid per day should be prescribed;
  • Cervical infections caused by Chlamydia trachomatis: 450-600 mg daily, dosage frequency – 4 times a day, duration of therapy – 10-14 days;
  • Acute streptococcal pharyngitis/tonsillitis: for 10 days, 300 mg 2 times a day;
  • Pneumocystis pneumonia in patients with AIDS: for 21 days, 300-450 mg every 6 hours simultaneously with primaquine 15-30 mg once a day;
  • Malaria: adults – 10-20 mg/kg per day; children – 10 mg/kg per day. The drug is taken in equal doses every 12 hours, the course duration is 7 days. It is also possible to use Dalatsin simultaneously with quinine (every 12 hours, 12 mg/kg) or chloroquine (every 24 hours, 15-25 mg), the course duration is 3-5 days.

To prevent endocarditis in patients with hypersensitivity to penicillins, Dalatsin is prescribed: adults - 600 mg; children – 20 mg/kg. The drug should be taken 1 hour before minor dental or surgical intervention, or any other procedure in which there is a risk of developing endocarditis.

Elderly patients with normal renal and liver function for a given age, as well as patients with functional disorders of the liver and kidneys, should not adjust the dose.

When treating bacterial vaginosis, the following are usually prescribed:

  • Vaginal cream: 5000 mg of cream (1 full applicator, approximately 100 mg of clindamycin) should be inserted into the vagina. The procedure is best done before bedtime. Course duration – 3 or 7 days;
  • Vaginal suppositories: 1 suppository is administered intravaginally 1 time per day, preferably before bedtime. Course duration – 3 days.

Dalacin in the form of a gel is used externally in the treatment of acne vulgaris. The drug is applied in a thin layer to the affected area (dry, clean skin). Frequency of application – 2 times a day. The recommended course duration is 6-8 weeks. According to indications, Dalacin can be used for 6 months. If after using the gel for several months the effectiveness of treatment decreases, you need to take a break for 4 weeks.

How to take Dalatsin suppositories

The initial dose is 1 suppository per day. It is administered before going to bed. The course of treatment is 3 days. For ease of use, the package contains an applicator. The candle must be freed from the foil and placed inside the device with the flat end. The patient should bend his knees while lying on his back or side.

The device with the candle is inserted deep into the vagina and pressed on the end of the applicator.

After the procedure, the applicator is removed and washed under running water and soap. You can skip the applicator and insert the candle yourself using your middle finger.

Side effects

When using Dalacin orally, the following side effects may develop:

  • Digestive system: nausea, abdominal pain, esophagitis, diarrhea and vomiting, pseudomembranous colitis, esophageal ulcer, jaundice, dysbiosis, liver dysfunction, including hyperbilirubinemia;
  • Hematopoiesis (hematopoietic system): cases of transient eosinophilia and leukopenia (neutropenia), as well as cases of thrombocytopenia and agranulocytosis, have been reported, but the connection between taking Dalatsin and these phenomena has not been established;
  • Allergic reactions: itching, urticaria, maculopapular rash; most often - a generalized morbilliform rash of moderate to mild severity; rarely - vesiculobullous and exfoliative dermatitis, toxic epidermal necrolysis, erythema multiforme, anaphylactoid reactions;
  • Other: vaginitis.

Also during therapy, the following side effects may occur: development of superinfection, disruption of neuromuscular transmission.

When treating bacterial vaginosis with Dalacin in the form of vaginal cream and suppositories, disorders of certain body systems may develop:

  • Reproductive system: vulvovaginitis, vaginal candidiasis, uterine bleeding, irritation of the mucous membrane of the vagina and vulva, vaginal infections, trichomonas vaginitis, vaginal pain, abnormal labor, menstrual irregularities, vaginal discharge, endometriosis;
  • Urinary system: urinary tract infections, dysuria, proteinuria;
  • Digestive system: taste disturbance, flatulence, nausea, diarrhea, vomiting, dyspepsia, constipation, bad breath, gastrointestinal disorders;

  • Digestive system: colitis, abdominal pain, pseudomembranous colitis, dyspeptic disorders (nausea, diarrhea, vomiting);
  • Organ of vision: burning sensation in the eyes;
  • Skin: most often – dry skin; increased production of sebaceous glands, skin irritation, urticaria, contact dermatitis;

Many women had to deal with Dalatsin vaginal cream and suppositories when treating vaginitis. Reviews about Dalatsin candles are mostly positive. If diarrhea develops after using Dalatsin orally or intravaginally, the possibility of developing pseudomembranous colitis should be considered. The disease may also appear 2-3 weeks after cessation of therapy. Its main symptoms include: diarrhea, leukocytosis, fever, abdominal pain (sometimes accompanied by mucus and blood in the stool). If necessary, diagnostic and therapeutic measures should be carried out. When using the drug externally, cases of colitis are rare, however, caution must be exercised and, in the event of prolonged or severe diarrhea, the use of the gel should be discontinued.

For cervical infections caused by Chlamydia trachomatis, monotherapy with Dalatsin in capsule form is effective for complete cure.

When taking Dalatsin orally, excessive growth of microorganisms insensitive to the drug, especially yeast-like fungi, is possible. In the event of superinfection, appropriate measures must be taken depending on the clinical situation.

Clindamycin should not be prescribed for the treatment of meningitis.

When prescribing Dalatsin in high doses, it is necessary to monitor the concentration of clindamycin in plasma. With long-term therapy, kidney and liver function tests should be performed regularly.

Before prescribing Dalatsin for the treatment of vulvovaginitis, the following possible causative agents should be excluded: Chlamydia trachomatis, Trichomonas vaginalis, Candida albicans, Neisseria gonorrhoeae and the Herpes simplex virus.

It must be taken into account that intravaginal use of the drug can cause increased growth of microorganisms that are insensitive to its action, especially yeast-like fungi.

When using the drug intravaginally, sexual contact should be avoided, as well as the use of other types of products administered intravaginally (for example, syringes, tampons).

The use of Dalacin in the form of cream or suppositories during menstruation is not recommended. In this case, the start of therapy should be postponed until the end of menstruation.

Dalatsin contains components that affect the strength of rubber or latex products (contraceptive vaginal diaphragms, condoms). The use of such products during therapy and within 3 days after using the drug is not recommended.

Avoid contact of the gel for external use with the mucous membranes of the mouth and eyes. After applying the drug, you must wash your hands thoroughly. If accidental contact occurs with sensitive surfaces (skin abrasions, eyes, mucous membranes), rinse them thoroughly with cool water.

Drug interactions

Clindamycin should not be taken orally at the same time as erythromycin and chloramphenicol.

There is cross-resistance of microorganisms to lincomycin and clindamycin.

Clindamycin should be used with caution in patients receiving other peripherally acting muscle relaxants.

With the simultaneous use of Dalacin orally with opioid (narcotic) analgesics that have a central inhibitory effect on respiratory function, more pronounced respiratory depression may develop, up to apnea.

The risk of developing pseudomembranous colitis increases with simultaneous use of Dalacin with antidiarrheal drugs that reduce gastrointestinal motility.

The spectrum of antimicrobial action of clindamycin is expanded by aminoglycosides.

The simultaneous use of vaginal cream and suppositories with other drugs for intravaginal administration is not recommended.

Overdose

There are no cases of overdose when taking recommended doses orally. When anaphylactic reactions occur, glucocorticosteroids and antihistamines . There is no antidote. Hemodialysis is ineffective.

There are also no reports of overdose of intravaginal cream and suppositories. Systemic reactions may occur: diarrhea , including hemorrhagic , pseudomembranous colitis . Symptomatic therapy is carried out.
