Properties of the drug

What properties does the drug “Relaxon” have? Reviews from experts report that this drug can have sedative, moderate anticonvulsant hypnotic, muscle relaxant and tranquilizing effects. All of them are associated with the action of receptors of the macromolecular GABA complex.

After taking the tablet, the inhibitory effect of GABA is enhanced, and interneuronal transmission in the central nervous system is suppressed.

The sleeping pill "Relaxon" noticeably shortens the time it takes to fall asleep, reduces the number of awakenings at night and improves the quality of sleep. This drug also increases the duration of sleep and practically does not cause such post-somnia effects as drowsiness and weakness in the morning.

There is no addiction to the action of the drug. Therefore, it can be taken intermittently for up to 17 weeks.

General description of the drug

Lack of adequate sleep causes a breakdown of the nervous system, suppresses the emotional state, and leads to a loss of vitality. This pathological condition in some cases requires drug treatment. Relaxon will help get rid of insomnia. This hypnotic belongs to the group of cyclopyrrolones and differs in structure from barbiturates and benzodiazepines. Unlike the latter, the drug in question is not addictive.

pharmachologic effect

Zopiclone is a hypnotic drug from the group of cyclopyrrolones, which are structurally different from benzodiazepines and barbiturates. It has the following pharmacological properties: hypnotic, sedative, tranquilizing, anticonvulsant and muscle relaxant.

These effects are associated with its action on CNS receptors related to the macromolecular GABA complex, modulating the opening of channels for chloride ions, resulting in an increase in the inhibitory effect of GABA and inhibition of interneuron transmission in various parts of the CNS.

Zopiclone has the ability to shorten the time before falling asleep and reduce the frequency of night awakenings, increase sleep duration and improve the quality of sleep and awakening. It practically does not cause post-somnia disorders: there are no feelings of weakness and drowsiness the next morning.

There is no addiction to the hypnotic effect of the drug during a long period of treatment, up to 17 weeks.

Pharmacokinetics

Absorption

Zopiclone is rapidly absorbed. Cmax in blood plasma is reached within 1.5-2 hours and is approximately 30 and 60 ng/ml after oral administration of 3.75 mg and 7.5 mg, respectively. Absorption of the drug does not depend on gender or food intake.

The binding to plasma proteins is approximately 45%.

Metabolism

After repeated prescriptions, accumulation of zopiclone and its metabolites does not occur. Interindividual differences are insignificant.

The main metabolites are the N-oxide derivative and the N-desmethyl metabolite. T1/2 is approximately 4.5 and 7.4 hours, respectively.

Separate groups of patients

In elderly patients, despite a slight decrease in metabolism in the liver and an extension of T1/2 to approximately 7 hours, no accumulation of the drug in plasma was detected even with repeated administration.

In patients with renal failure, accumulation of zopiclone or its metabolites was not detected even after long-term use.

In patients with liver cirrhosis, the clearance of zopiclone is reduced by approximately 40% in accordance with a decrease in the demethylation process.

Indications

- treatment of transient, situational and chronic insomnia in adults of various etiologies (including difficulties falling asleep, night and early awakenings).

Contraindications

- hypersensitivity to zopiclone or other components of the drug;

- myasthenia gravis;

- severe respiratory failure;

- severe liver failure;

— “night” apnea syndrome;

- pregnancy and breastfeeding;

- age up to 18 years.

Dosage

Treatment should be as short as possible and not exceed four weeks. Extension of treatment time beyond the maximum permissible is carried out after re-evaluation of the patient’s condition. The drug is taken before bedtime.

Duration of treatment.

Transient insomnia: from 2 to 5 days.

Situational insomnia: 2 to 3 weeks.

Chronic insomnia: the duration of course treatment is determined after consultation with a specialist.

Recommended dose for adults

– 7.5 mg (1 tablet). The maximum dose is 15 mg.

Although in cases of renal failure

accumulation of zopiclone and its metabolites was not detected; in such patients it is recommended to begin treatment with a dose of 3.75 mg.

The most common side effect observed with zopiclone is a bitter taste in the mouth, but may also include:

From the side of the central nervous system

: dizziness, headache, residual drowsiness after waking up;

From the digestive system:

digestive disorders: dyspepsia, nausea, dry mouth.

Allergic

skin reactions such as itching and rashes; angioedema and anaphylactic reactions are extremely rare.

Mental and paradoxical reactions:

rarely - nightmares, irritability, confusion, hallucinations, aggressiveness, confusion, depressed mood, poor coordination of movements, depression, inappropriate behavior with the possible development of amnesia.

After stopping treatment, in rare cases, withdrawal syndrome and rebound insomnia may occur.

Isolated cases of slight increases in serum transaminases and/or alkaline phosphatase were noted.

Overdose

Overdose usually manifests itself in the form of symptoms of varying degrees of central nervous system depression from drowsiness to coma, depending on the amount of the drug taken.

First aid consists of gastric lavage and taking activated charcoal. If necessary, symptomatic and supportive therapy in a hospital setting is recommended. Particular attention should be paid to respiratory and cardiovascular functions.

Hemodialysis is of little importance due to the large volume of distribution of zopiclone. Flumazenil can be used as an antidote.

Drug interactions

Concomitant use with alcohol is not recommended, as the sedative effect of zopiclone may increase.

An increased depressant effect on the central nervous system may occur in cases of simultaneous administration with neuroleptics, hypnotics, tranquilizers, sedatives, antidepressants, narcotic analgesics, antiepileptic drugs, anesthetics, antihistamines with a sedative effect, as well as erythromycin. Taking the drug reduces the concentration of trimipramine in plasma.

special instructions

In patients with nocturnal asthma attacks, in combination with methylxanthine drugs (theophylline), zopiclone reduces asthma attacks in the early morning hours, reduces their intensity and duration. When prescribing zopiclone, it must be remembered that, although the risk is minimal, the development of addiction to the drug and its abuse cannot be absolutely ruled out. The risk of dependence or abuse occurs when:

- violation of the dose and duration of treatment;

— alcohol and/or drug abuse;

- combined use with alcohol or other psychotropic drugs.

Rebound insomnia and withdrawal syndrome

The risk of such events after abrupt discontinuation of zopiclone cannot be excluded, especially after long-term treatment. Therefore, it is recommended to gradually reduce the dosage and notify the patient about this.

Amnesia

Anterograde amnesia may occur, especially when sleep is interrupted or after a significant period of time between taking the drug and going to bed. To reduce the risk of anterograde amnesia, you must:

- take the tablet immediately before bedtime;

- Ensure sleep duration is at least 6 hours.

Depression

The drug is not indicated for the treatment of depression, and may even mask its symptoms.

Use in children

Impact on the ability to drive and manage transport

Due to its pharmacological properties, zopiclone may have an adverse effect on the ability to drive and operate vehicles, as well as to engage in other potentially hazardous activities that require increased attention and speed of psychomotor reactions, therefore, during the treatment period you must be extremely careful or refrain from activities of this kind.

Pregnancy and lactation

It is not recommended to use the drug during pregnancy and for nursing mothers.

Use in childhood

Contraindication: age under 18 years.

For liver dysfunction

Contraindication: severe liver failure.
