Diclofenac has a pronounced anti-inflammatory and analgesic effect, and moderately reduces elevated body temperature.
Paracetamol has an analgesic effect, anti-inflammatory and antipyretic effects are weakly expressed. The principle of action of the drug is based on inhibition of prostaglandin .
When taken orally, Dolaren begins to act within an hour. Eating slows down the rate of absorption of the active components of the drug. Metabolites are excreted by the renal and hepatic systems.

The drug is effective in the treatment of dysmenorrhea , postoperative pain, bursitis , tendinitis , musculoskeletal disorders in the acute period, febrile conditions, and soft tissue diseases.
Directions for use and doses
The dosage is determined for each patient individually, taking into account the patient’s age, the nature and course of the disease, tolerability and the therapeutic effect of the drug.
For sporadic pain (headache, toothache), Dolaren is used from 1 to 3 tablets per day.
For algodismenorrhea, the initial dosage is 1-3 tablets (up to 4) per day. The duration of use of the drug is 5-7 days and depends on the dynamics of symptoms.
For moderately severe pain or long-term therapy, as a rule, 2 tablets of the drug per day are sufficient - one in the morning and one in the evening. The maximum dose of Dolaren in such cases should not exceed 3 tablets per day.
The drug is administered orally, without chewing, during or after meals (to achieve a quick effect, 30 minutes before meals), with a small amount of water.
Side effects
Gastrointestinal tract: dyspeptic disorders, anorexia , nausea, epigastric pain, stool disorders, flatulence . Long-term use of the drug leads to ulcerative lesions of the gastrointestinal tract, including bleeding.
Central nervous system: depression, drowsiness, headaches.
Urinary system: nephrotic syndrome, hematuria, acute renal failure, proteinuria .

Interaction
Dolaren reduces the effectiveness of furosemide , antihypertensive drugs.
When taken simultaneously, it increases the level of digoxin in the blood.
Potassium-sparing diuretics increase the risk of hyperkalemia .
Corticosteroid and anti-inflammatory drugs increase the risk of gastrointestinal bleeding.
It is not recommended to combine Dolaren with cyclosporine and methotrexate .
Dolaren Tablets
Name: Dolaren Tablets (Dolaren) Pharmacological action: Dolaren tablets contain 2 substances from the NSAID group - paracetamol and diclofenac sodium. Both agents inhibit the activity of cyclooxygenase, which promotes the formation of cyclic endoperoxides from arachidonic acid. Cyclic endoperoxides serve as the starting material in the formation of prostaglandins, which are involved in the development of inflammation and its clinical manifestations - pain, swelling, fever. Diclofenac suppresses the formation of prostaglandins by inhibiting the activity of both types of cyclooxygenase. Thus, it has a pronounced analgesic and anti-inflammatory effect. The antipyretic effect is moderate. The central mechanism of the analgesic effect of diclofenac is associated with inhibition of the formation of prostaglandins in the nervous system.
Paracetamol has a predominantly central effect, inhibiting the formation of prostaglandins in the nervous system. It is a strong analgesic and antipyretic. The anti-inflammatory effect is weakly expressed. Diclofenac is well absorbed from the digestive tract. The rate of absorption slows down slightly when taking food at the same time. Able to penetrate into synovial fluid. Metabolism occurs primarily in the liver. Some metabolites are pharmacologically active. Excretion of metabolites is carried out mainly by the kidneys, partially with bile. About 1% of diclofenac is excreted unchanged by the kidneys. Absorption of paracetamol in the digestive tract occurs quickly, mainly in the small intestine. Metabolism of paracetamol occurs in the liver. Metabolic products are excreted by the kidneys. Unchanged, the kidneys excrete 5% of the drug.
Indications for use: Dolaren tablets are recommended for the symptomatic treatment of colds, headaches, neuralgia, toothache, lumbago, osteoarthritis, sciatica, rheumatoid arthritis, radiculitis, febrile conditions, bursitis, tendinitis, myositis, dysmenorrhea, postoperative pain.

Interaction with other drugs: Dolaren reduces the clinical effects of furosemide and antihypertensive drugs, potentiates the effect of potassium-sparing diuretics. Increases plasma digoxin levels. Concomitant administration of corticosteroids increases the risk of developing gastrointestinal ulcers. Increases the toxic effects of methotrexate. Strengthens the nephrotoxic effect of cyclosporine. Potentiation of the action of anticoagulants is possible.
Overdose: Manifestations – renal failure, arterial hypotension, convulsions, dyspeptic symptoms, respiratory disorders. Treatment is symptomatic.
