Indications

Lazolvan is a mucolytic that activates the movement of cilia, reduces the viscosity of bronchial secretions and facilitates its removal from the body. While taking the drug, according to the manufacturer, the biosynthesis of surfactant increases and its breakdown is inhibited, as a result of which pathogenic bacteria do not enter the epithelial cells. The mucolytic suppresses the inflammatory process, has an antioxidant effect, and relieves cough.

If a child has chronic obstructive pulmonary disease (COPD), then long-term use of an expectorant (for at least 2 months) reduces the number of exacerbations and reduces their duration.

It is thanks to this description from the manufacturer that Lazolvan can be prescribed to children for the following diseases:

  • acute and chronic inflammation of the bronchi;
  • HOLB;
  • pneumonia;
  • bronchial asthma, in which the child produces sputum that is difficult to separate;
  • bronchiectasis (expansion and deformation of the bronchi, which disrupts their function, in addition, a purulent-inflammatory process begins in the bronchial tree).

The effect of the drug Lazolvan has not been proven!

At the KIHE-2015 conference, the President of the Association of International Pharmaceutical Manufacturers in the Republic of Kazakhstan, Vyacheslav Lokshin, presented a list of drugs popular in Kazakhstan with unproven effectiveness. Lazolvan made it into the TOP 10 drugs without evidence base.

Multiple medical studies have found that ambroxol increases the secretory function of the respiratory tract. The substance accelerates the production of surfactant and activates ciliary activity.

These effects cause increased flow and movement of mucus through the respiratory tract. Thus, the transport of sputum is improved and the cough is significantly relieved.

It was noted that with long-term treatment with Lazolvan (2 or more months) in patients with COPD, the number of exacerbations decreased, and the duration of the acute period and the duration of antibiotic treatment were shortened.

Instructions for use

The syrup should be taken orally regardless of food. The dosage of Lazolvan for a child is selected individually depending on age.

Scheme for using syrup at a dosage of 15 mg

The child should be given syrup in a dosage of 15 mg according to the following schemes:

During therapy, parents need to consider the following:

  1. If a child drinks a mucolytic for 4-5 days and there is no positive dynamics, then you should consult a doctor.
  2. It is not recommended to take the medication in dosages higher than therapeutic ones, as this will cause an overdose of Lazolvan. In case of mucolytic poisoning, the victim may experience nausea, diarrhea, vomiting, and abdominal pain. The antidote for overdose with Lazolvan is unknown, therefore symptomatic treatment is recommended. If no more than 2 hours have passed since taking the drug, then it is recommended to induce vomiting in the child and rinse the stomach.
  3. The mucolytic improves the penetration of antibiotics into the bronchial secretions. These are antimicrobial agents such as amoxicillin, erythromycin, cefuroxime.
  4. Antitussives should not be taken during treatment with an expectorant. They suppress the cough reflex, as a result, sputum stagnates in the bronchi, and it is a good environment for the proliferation of pathogenic microflora, which can cause a deterioration in the patient’s well-being.
  5. To ensure that the syrup does not lose its therapeutic effect, it must be stored at temperatures up to 25 degrees for 36 months from the date of production.
  6. Taking Lazolvan can cause Lyell's and Stevens-Johnson syndrome. The first signs of these pathological conditions may be high fever, body aches, cough, and sore throat. If these unpleasant symptoms appear, you should immediately consult a doctor. If lesions of the skin and mucous membranes appear while taking the syrup, you should stop giving Lazolvan to the child and show him to a doctor.

Pharmacokinetics

All forms of the immediate-release drug are characterized by almost complete, rapid absorption with a direct dependence on the therapeutic dose in the therapeutic concentration range.

When taken internally, the maximum content of ambroxol in the blood plasma is observed after 1.5-2 hours. When the therapeutic concentration is reached, binding to plasma proteins reaches a maximum of 90%. Transport of ambroxol from the blood to tissues when taken internally occurs quickly. The highest concentrations of ambroxol are observed in the lung tissue.

About 30% of the administered dose of the drug is subject to the effect of primary transport by the liver. The predominant isoform involved in the conversion of ambroxol to dibromanthranilic acid is CYP3A4. The remainder of ambroxol is metabolized in the liver, mainly by glucuronidation and partial breakdown to the metabolite dibromoantranilic acid and other inactive substances.

T 1/2 is 10 hours. Total clearance varies within the range of 660 ml/min, with renal clearance accounting for about 83% of the total clearance.

No clinically significant effect of age and gender characteristics on the pharmacokinetics of ambroxol was found, therefore, when choosing a dosage, these signs are not taken into account.

What is better for children: Ascoril or Lazolvan?

Ascoril is a combination medication that is available in syrup and tablets. It has the same indications for use as the drug containing ambroxol, but it has more contraindications. Ascoril is prohibited in case of allergies to its active and auxiliary components, glaucoma, hypertension, hyperthyroidism, arrhythmia, heart defects, myocarditis, impaired liver and kidney function, gastric bleeding, peptic ulcer of the digestive tract, diabetes mellitus, which cannot be compensated with the help of medications. It cannot be taken in tablet form for children under 6 years of age.

Ascoril more often provokes unwanted reactions, but this does not mean that the combined medication is worse; it may be more suitable for a particular patient.

The drug can be taken immediately after a meal or before eating it, which is considered an advantage over other drugs. The presence of sorbitol in the syrup can lead to a slight laxative effect, which is normal if diarrhea does not continue throughout the course of treatment.

The prescription of syrup and dosage is made only by a doctor, taking into account the characteristics of the child’s body. As well as the possible risk of complications during the treatment period. If within 7-10 days after the start of the course there is no improvement in the condition or a worsening of the disease is observed, you should abandon the syrup and visit a doctor. Perhaps the product is not suitable for a particular child.

Children's ACC or Lazolvan syrup?

ACC is available for sale in effervescent tablets, injection solution, syrup, and granules. The injections have no age restrictions, but an expectorant can be taken orally only from 2 years of age. ACC contains acetylcysteine ​​as its main component, which has a wider range of applications compared to ambroxol. It is effective both for pathologies of the bronchopulmonary system and for otitis media and inflammation of the paranasal sinuses.

But despite the fact that ACC has more indications for use, this does not mean that it is preferable, since it also has limitations and can provoke negative reactions. That is why only a doctor should decide which mucolytic is best.

Composition, release form

Expectorant drops look like a clear, colorless solution. They have no smell or taste. The mucolytic effect is provided by the main component – ​​ambroxol hydrochloride. 1 ml of solution contains 7.5 mg of active substance. Additionally included:

  • citric acid monohydrate;
  • sodium chloride;
  • purified water;
  • benzalkonium chloride;
  • sodium hydrogen phosphate dihydrate.

The medication is poured into darkened glass bottles equipped with a stopper and dropper to make it convenient to measure the solution. The volume of one bottle is 100 ml. The cardboard package contains 1 bottle, instructions for use, and measuring cup. The drug has several release forms with the same component. Lazolvan Rino with another active ingredient has appeared in pharmacies; it is used for a runny nose and the treatment of inflammatory processes in the sinuses.

What should I give my child Gedelix or Lazolvan?

Gedelix is ​​a herbal medicine that contains ivy as its main component. The product is available in drops and syrup. The latter is allowed from the first days of life in case of pathological processes in the respiratory tract, when thick, difficult-to-clean sputum is formed in them. Gedelix is ​​usually well tolerated, but sometimes the mucolytic can provoke an allergic reaction, nausea, vomiting, loose stools, and gastralgia. The herbal remedy is prohibited in case of hypersensitivity to its components, citrullinemia. It should not be taken in drops for children under 2 years of age, asthmatics, or patients in whom there is a high risk of laryngospasm.

Gedelix and Lazolvan are not completely interchangeable agents and therefore only a doctor should select a mucolytic.

Contraindications

  • Pregnancy (1st trimester);
  • Lactation;
  • Children under 6 years of age (syrup 30 mg/5 ml and lozenges) and up to 18 years of age (tablets);
  • Fructose intolerance;
  • Individual hypersensitivity to ambroxol and other substances of the drug;
  • Lactose intolerance, lactase enzyme deficiency, glucose-galactose malabsorption (for l/f tablets).

For pregnant women in the 2nd and 3rd trimester, people with liver or kidney failure, the drug is prescribed with caution.

Ambroxol has been proven to penetrate the placental barrier. At the same time, no negative (direct or indirect) effect on pregnancy and intrauterine development of the fetus has been established. However, the drug is prescribed with caution and if the benefit to the woman outweighs the potential risks regarding the effect on the fetus. Ambroxol is also excreted in breast milk, so it is not recommended to use the drug during the lactation period.

Evgeniy Olegovich does not recommend giving mucolytics without a doctor’s prescription. Only a doctor should prescribe it in combination with other drugs. According to a well-known pediatrician, the effectiveness of expectorants compared to drinking plenty of fluids has not been proven. Therefore, he advises giving more water to the child if he has a wet cough.

Despite the fact that the mucolytic is available without a prescription, self-medication is prohibited; only the doctor should decide how to give Lazolvan syrup to the child.
