Mercozolil is a drug produced in the form of tablets belonging to the group of thyroid hormone antagonists. The antithyroid properties of the drug are used in modern medicine to treat diseases associated with increased activity of the thyroid gland. The drug is produced in the form of tablets for oral use, which ensures ease of use.

Pharmacodynamics and pharmacokinetics

The active substance is thiamazole . The mechanism of action of the main component is aimed at blocking peroxidase , an enzyme that takes an active part in the process of iodination of thyronine in the tissues of the thyroid gland with the formation of monoiodothyronine, diiodothyronine, and subsequently triiodothyronine and tetraiodothyronine.

The drug interferes with the process of synthesis of hormones in the tissues of the thyroid gland, reduces the internal production of the hormone T4, accelerates the process of removing iodides from the tissues of the thyroid gland, slows down the basal metabolic rate, enhances the reciprocal production and synthesis of the hormone TSH by the pituitary gland, which leads to hyperplasia of the thyroid gland.

In many ways, the rate of development of the therapeutic effect depends on the initial level of hormones T3 and T4. On average, a decrease in hormone levels is recorded a week after the start of treatment with Mercazolil at a dose of 30 mg; or after a month when taking 40 mg.

After stopping treatment, hormone levels may return to previous levels within 5 days.

Pharmacological group

Mercazolil inhibits the production of hormones produced by the thyroid gland, namely triiodothyronine and thyroxine. This effect of the drug is associated with the suppression of the activity of peroxidase, that is, the enzyme that ensures the saturation of triiodothyronine with iodine. As a result, against the background of the above process, the metabolic rate in the human body decreases and the loss of iodine by the thyroid gland is activated. Against this background, the secretion of thyroid-stimulating hormone from the pituitary gland increases, which leads to an increase in the volume of the thyroid gland.

The drug is well absorbed from the gastrointestinal tract and accumulates in the tissues of the thyroid gland. The effect of the drug lasts for 24 hours. After two days, the drug is completely excreted in the urine. It is worth noting that the active component of the drug can be found in breast milk. The use of the drug by women during breastfeeding is prohibited. After oral administration, it is quickly and completely absorbed from the gastrointestinal tract. The half-life is 6 hours. The product is biotransformed in the body. The component is eliminated from the body in the form of metabolites in a volume of 70% within 2 days.

Contraindications

Mercazolil is contraindicated during breastfeeding, with granulocytopenia, with intolerance to the active component thiamazole, with severe leukopenia.

When pregnant, with severe liver pathology, with a very large goiter with concomitant narrowing of the trachea (only in the preoperative period for a short course), the drug is prescribed with caution.

Take orally after meals.

Thyrotoxicosis of mild to moderate severity: 3-4 times a day, 5 mg. Upon achieving remission (after about 1-1.5 months), the daily amount of the drug is reduced by 5-10 mg every 5-10 days, until the minimum dosage is gradually reached (once every 3 days, or 1 time per day, 5 mg), which are capable of providing a lasting therapeutic effect.

Severe thyrotoxicosis: 3-4 times a day, 40-60 mg per day, after stabilization of the condition, the dose is reduced to 5-10 mg per day. The course of therapy is designed for 1-1.5 years. The highest dosages: daily – 60 mg, single – 10 mg.

Release form and composition

Mercazolil is produced in the form of tablets: flat-cylindrical, from white with a yellowish or creamy tint to white, round with a chamfer (in plastic containers or polymer jars of 50 or 100 pcs., 1 container or jar in a cardboard box; in blister packs of 10 or 20 pcs., 5 or 10 packages (10 tablets each) or 2 packages (20 tablets each) in a cardboard box).

  • Active substance: thiamazole – 5 mg;
  • Auxiliary components: calcium stearate monohydrate – 1 mg; talc – 2 mg; potato starch – 98 mg; lactose monohydrate – 94 mg.

Overdose
