The use of Tiloron-SZ as part of complex therapy in patients with mild forms of COVID-19

Among the antiviral drugs registered in the Russian Federation, nobazite is classified as an interferon inducer.

This is N-methyl-4-benzylcarbamidopyridinium iodide (enisamium iodide) with excipients:

  • lactose monohydrate (milk sugar) - 33.4/66.8 mg;
  • MCC (vivapur 101) - 10/20 mg; sucrose (refined sugar) - 9/18 mg;
  • povidone (low molecular weight medical PVP or plasdon K17) - 3.6/7.2 mg;
  • copovidone (plasdon S630) - 1.4/2.8 mg;
  • crospovidone (polyplasdone XL10) - 3.8/7.6 mg; talc - 1.9/3.8 mg;
  • calcium stearate - 1.9/3.8 mg.

The drug nobasit is available in the form of film-coated tablets or capsules. Such forms make it possible to preserve the active substance from destruction in the stomach and achieve its absorption in the intestines.

Nobasit tablets are round, biconvex, yellow in color. Dosages of 125 mg or 250 mg are available in packs of 20 or 10 pieces. The capsules are orange in color, hard in a gelatin shell, and contain yellow or greenish-tinged powder.

Release form and composition

The dosage form of Tiloram is film-coated tablets: round, biconvex, two layers are visible at the break - a yellow to orange film shell and an orange core, in which inclusions of orange and white are allowed (in blister packs of 6, 10, 12, 18, 20, 24 and 30 pcs., in a cardboard pack of 1, 2, 3, 4, 5 or 10 packages; in polymer cans or polyethylene terephthalate cans of 6, 10, 12, 18, 20, 24, 30, 50 and 100 pcs., 1 can in a cardboard pack).

Composition of one tablet:

  • active ingredient: tilorone dihydrochloride – 125 mg;
  • auxiliary components: microcrystalline cellulose, magnesium stearate, povidone K17, croscarmellose sodium, lactose monohydrate (milk sugar), water;

Analogs are cheaper than Tiloron

Since Tiloron (as an independent drug) is not currently available in pharmacies, you, in any case, will have to consider its analogues and choose those that are cheaper. This active ingredient is contained in the following preparations:

  • Amiksin
  • Lavomax
  • Tiloram
  • Actaviron

But, in reality, the pharmacy will only offer you a choice of Lavomax and Amiksin. The first drug costs, on average, 100 rubles less.

There are other interferon inducers. For example, Kagocel, which is similar in action, but not in composition. It costs approximately two and a half times less. But you should definitely consult your doctor about the possibility of such a replacement for Tiloron.

Basic principles of treatment for COVID-19

The recommendations of the Ministry of Health regarding the treatment of COVID-19 were drawn up based on a study of clinical experience in the treatment of SARS during previous epidemics. Basic principles of treatment of coronavirus infection:

  • taking antimalarial drugs;
  • antiviral therapy;
  • use of immunomodulators.

Effective antimalarial drugs include Mefloquine, Hydroxychloroquine, and Chloroquine. To date, the principle of the effect of such medications on viral pathogens has not been fully studied. It is assumed that they prevent the virus from entering the body's cell.

Antimalarial drugs reduce the symptoms of pneumonia, but have cardiotoxic effects.

HIV protease inhibitors are most often prescribed as antiviral therapy for coronavirus. In most cases, drugs based on the drug combination of ritonavir and lopinavir are used.

As part of immunomodulatory therapy, pharmacological agents based on interferon beta-1b are prescribed. This substance effectively activates the cells of the human body to fight coronavirus infection.

Use of the drug as part of complex treatment

We also recommend reading: Kagocel for the prevention of coronavirus

Based on global clinical experience, doctors include Tiloram in complex therapy in the fight against coronavirus or to prevent its complications. But research is still ongoing. When prescribing the drug to patients with mild to moderate severity, it is possible to increase the number of leukocytes, platelets, lymphocytes in the blood, but not beyond the reference limits and a significant decrease in the level of lactate dehydrogenase by the end of the second week of treatment.

Therefore, therapy that includes Tiloram is effective and can significantly alleviate the patient’s condition and speed up the recovery process. But there is no data on the use of the drug alone in the fight against coronavirus. Therefore, its use in monotherapy is not recommended.

The medication is fully compatible with other antiviral agents, antibiotics and antihistamines. No clinically significant interactions with drugs aimed at suppressing bacterial diseases or incompatibility with alcohol have been identified.

Official information about coronavirus from the Ministry of Health of the Russian Federation

WHO recommendations for the treatment of coronavirus

Currently, recommendations from the Ministry of Health have been prepared on drugs for coronavirus - both its treatment and prevention, as well as a protocol for conducting diagnostic measures.

Patients with a confirmed diagnosis of COVID-19 are recommended to be prescribed medications that contain the following active substances:

  • ribavirin;
  • recombinant interferon beta-1b;
  • ritonavir and lopinavir.

Such pharmacological agents are included in the classic treatment regimen for complicated influenza and pneumonia. As a result of clinical studies conducted by a number of epidemiologists in different countries of the world, the drugs confirmed their effectiveness in the treatment of coronavirus infection.

Medicines, as recommended by WHO and the Ministry of Health, are prescribed only if a positive test for COVID-19 is detected.

At the same time, doctors emphasize that there is no need to take these medications on your own. Each of them has a wide list of contraindications and side effects, so taking them on their own can only worsen the course of the disease.

Compatibility with other drugs

Tiloron is recommended by specialists for use as part of complex therapy. It can be combined with antibacterial compounds, as well as other symptomatic medications, often used to alleviate the condition of patients with viral and bacterial infections.

There are analogues of this drug on the pharmaceutical market:

  • Amiksin;
  • Aktavin;
  • Lavomax;
  • Tiloram;
  • Tilorone dihydrochloride.

Despite the fact that Tiloron has high antiviral activity, using it alone to prevent the development of viral infections is at least ineffective

To prevent infection, it is very important to adhere to reasonable preventive measures, avoid crowded places, and also observe personal hygiene rules

When choosing which antiviral drugs are best to buy to prevent coronavirus, you should definitely consult with a therapist, since each medication has its own indications, contraindications for use and side effects.

Application of Arbidol

Arbidol is an antiviral drug based on the active substance umifenovir. The medicine is far from new; it was used to treat various viral infections back in Soviet times. Prescribed for the treatment of rhinoviruses and coronaviruses. The drug prevents the connection of the membranes of the causative agent of the infectious disease with the cell membrane, increases the level of lymphocytes in the bloodstream.

In addition, Arbidol tablets help strengthen and increase the body's resistance to dangerous viral pathogens and pathogenic microorganisms.

Arbidol is included in the list of the best antiviral drugs presented in the temporary recommendations of the Russian Ministry of Health regarding the prevention and treatment of coronavirus infection.

Arbidol has a minimum number of contraindications for use:

  • children under 3 years of age;
  • allergic reaction to umifenovir or auxiliary components in the composition;
  • individual intolerance to the drug.

Arbidol helps reduce respiratory manifestations of COVID-19, speed up recovery and prevent the development of viral pneumonia and other dangerous complications.

Treatment with Ribavirin

Ribavirin is the best antiviral agent for the treatment of coronavirus infection, according to the results of many clinical studies. This is a broad-spectrum antiviral drug, produced in various pharmacological forms - capsules for oral administration, solution for intravenous administration, and liquid for inhalation.

The principle of action of Ribavirin is its rapid penetration into the cell membranes of the body. In case of viral infection, the active substance ribavirin is instantly transformed into metabolites and inhibits pathogenic activity.

Ribavirin has a wide range of contraindications for use, which include:

  • heart failure;
  • renal or hepatic dysfunction;
  • autoimmune diseases;
  • severe forms of depression with suicidal tendencies;
  • thyroid diseases;
  • anemia;
  • cirrhosis of the liver;
  • childhood;
  • During pregnancy and breastfeeding.

Ribavirin can only be used for people undergoing treatment in a hospital with the mandatory presence of an intensive care unit. In some cases, it is possible for adults to use Ribavirin not in a hospital, but under the constant supervision of a medical professional with experience in resuscitation procedures.

Use of Remantadine

Remantadine is on the list of the most popular drugs for the treatment of coronavirus infection. The drug has a restorative, antiviral, immunomodulatory and antitoxic effect.

This pharmaceutical is used for both therapeutic and prophylactic purposes.

Remantadine can also be taken after infection - it slows down the reproduction of the viral pathogen after it penetrates the cell membranes.

The drug also stimulates the production of lymphocytes, inhibits the combination of viral genes with cellular cytoplasms, preventing their further reproduction and spread.

Main contraindications to the use of Remantadine:

  • all trimesters of pregnancy;
  • children under 7 years of age;
  • the presence of thyrotoxicity;
  • renal and liver failure.

Remantadine can cause a number of side effects - bloating and stomach pain, nausea, dry mouth, allergic skin rashes. In some cases, taking the drug may be accompanied by attacks of headaches, sleep disturbances, irritability, anxiety, increased excitability and psycho-emotional instability.

Paracetamol according to WHO recommendations

Paracetamol is on the list of medications officially recommended by the World Health Organization for the treatment of coronavirus infection.

This pharmacological agent does not directly affect the causative agent of the disease, but effectively fights inflammatory processes and the first respiratory symptoms.

In many cases, Paracetamol is prescribed as an alternative to Ibuprofen, which often causes negative side effects.

It was prescribed to patients during a pandemic in China, as a result of which it was found that such patients recovered approximately 3 times faster.

Paracetamol has several contraindications for use:

  • presence of hyperbilirubinemia;
  • renal or liver failure, occurring in severe form;
  • individual intolerance to paracetamol;
  • lecopenia;
  • diseases of the hematopoietic system;
  • chronic alcoholism;
  • anemia.

Paracetamol is one of a small number of pharmacological drugs approved for use during pregnancy and breastfeeding. In very rare cases, the medication can cause allergic skin rashes, stool upset, nausea, bloating and other dyspeptic disorders.

Use of Remdesivir

Remdesivir is a new pharmacological drug from America with a broad spectrum of antiviral action, belonging to the category of nucleoside analogues. The US Department of Health and the World Health Organization classify Remdesivir as one of the most effective and promising drugs in the treatment and prevention of coronavirus infection.

Remdesivir successfully passed a test trial in China, where it was prescribed to patients with Covid-19. It was found that recovery in such patients occurred 3 times faster.

To date, Remdesivir continues to undergo clinical trials. According to WHO experts, this particular drug will become the most effective in treating not only coronavirus infection, but also Marburg fever and Ebola fever.

One of the main features of Remdesivir is that it has shown high effectiveness in the fight against resistant and mutating pathogens of coronavirus infection.

Operating principle

The main ingredient in the medicine is a synthetic immunostimulating substance. It helps to quickly increase the body's defenses and prevents frequent colds and viral diseases against the background of weakened immunity. The drug has antiviral properties, therefore it quickly suppresses the activity of pathogens of pathologies of this origin.

The principle of action of the drug is based on the ability to stimulate the formation of the most important cells responsible for the patient’s immunity. In a laboratory study, experts note a significant increase in the number of lymphocytes and some other cells. As a result, the body's ability to resist increases.

Patients also note an improvement in overall well-being when using the medicine. If it is prescribed after the first signs of the disease appear, the recovery period is shortened.

The medicine also has other properties:

  • The product is effective against many pathogens. It helps improve the condition of influenza, acute respiratory viral infections, and herpetic lesions of the skin. There is a positive effect of the drug on the body of patients diagnosed with hepatitis of viral origin.
  • The medicine also helps to reduce the manifestations of the inflammatory process. As a result, the risk of developing complications due to the progression of inflammation and the addition of a bacterial infection is significantly reduced.
  • There is an improvement in the functioning of internal organs against the backdrop of an increase in the body's defenses. Patients with liver pathologies experience significant relief and prevent the development of complications in advanced disease.

The effect of using the drug lasts for a long period, which provides immunity, even after stopping therapy. The main substance of the composition prevents the proliferation of viruses, which contributes to a rapid decrease in their number in the body and gradual death.

Disruption of metabolic processes in viruses also leads to their death, reduces the risk of spreading to large areas, and penetration into healthy tissues. The use of the drug helps prevent the progression of pathologies of viral origin.

Tiloram (indications for the use of the drug require preliminary examination) is well absorbed in the digestive tract of patients. The dissolution of the film shell occurs in the small intestine, after which the main ingredient of the composition enters the patient’s blood, spreads throughout the body and has a therapeutic effect.

The drug begins to act gradually, while the ingredient stimulates the production of interferon in the intestines, after which activity is noted in the liver and blood. Despite the gradual increase in the number of immune cells in different organs and tissues, the medicine is considered effective, helping to quickly improve the condition or strengthen the immune system when it is weakened.

The bioavailability of the drug when taken orally is approximately 60%, but binding to plasma proteins occurs by 80%, which ensures high efficiency and speed of action.

The medication is processed in the patient's liver, but does not break down into metabolites. After processing, the components are excreted unchanged through the intestines. No more than 10% of the total amount of components is evacuated by the kidneys.

If there are disorders of the digestive tract, the process of absorption of ingredients may slow down, which will somewhat slow down the onset of the therapeutic effect. The period of elimination of the components of the composition in such disorders also slows down somewhat.

Useful tips

Tip #1

It is not recommended to prescribe medications on your own, even antivirals that have been tested by friends or have good reviews on the Internet. This can lead to the development of negative consequences.

Tip #2

In addition to taking medications, people with coronavirus need to reconsider their diet and add more vegetables and fruits to their diet. This will increase the supply of useful minerals and vitamins, and strengthen the body to cope with the infectious agent.

Tip #3

People with weakened immune systems who are at risk need to be more attentive to the state of the body. When the first signs of coronavirus appear, you should call a doctor, take PCR swabs and follow all the doctor’s recommendations.

Precautionary measures

The fungicide is harmless to the environment and vegetation, but poses a threat to humans if precautions are not followed:

  1. It is mandatory to have safety glasses, a respirator, gloves, and thick clothing with long sleeves.
  2. Avoid contact of the drug with the eyes, skin and mucous membranes.
  3. Avoiding the consumption of food and water during the process.
  4. After spraying, provide the opportunity to quickly change clothes, wash your hands and face with soap.

Close the opened package of granules hermetically and store in a place inaccessible to children and pets.
