Alflutop - instructions, composition of the drug, indications and contraindications, side effects

Alflutop is a drug for parenteral administration, which is used in orthopedics, rheumatology, and traumatology. The drug is intended for the treatment of degenerative-dystrophic and inflammatory pathologies of the joints and spine. Alflutop is used for the treatment of osteoarthrosis and osteochondrosis of various localizations, psoriatic, gouty, rheumatoid arthritis. The main pharmacological property of the injection solution is the restoration of cartilage tissue that has undergone destruction. When used in a course, its anti-inflammatory, anti-edematous, analgesic effect is manifested.

Osteoarthritis.

Alflutop, a medicine for joints, contains a complex of ingredients of natural origin. The high concentration of biologically active substances determines its clinical effectiveness and a wide range of contraindications. Therapy for joint diseases is carried out after examining the patient, establishing the degree of degenerative changes and the stage of the pathology.

Compound

The composition of the drug as an active substance includes a bioactive concentrate, which is obtained by extraction followed by delipidation and deproteinization from small sea fish (Black Sea whiting, sprat, Black Sea belly, Black Sea anchovy).
It contains: dermatan sulfate, mucopolysaccharides, creatane sulfate, hyaluronic acid , chondroitin sulfate, amino acids, polypeptides, Mg, Cu, Fe, Zn, K and Na ions.

Water and phenol .

Alflutop or Chondroguard - which is more effective?

Chondroguard contains chondroitin sulfate, it is a generic version of Alflutop, the drug is produced in Russia, so its cost is slightly lower. The analogue does not contain fish extract, which eliminates the possibility of developing allergies in patients with seafood intolerance.

Disadvantages of Chondrogard - it acts slower than the original drug, a full course requires at least 25 injections, and has a greater number of contraindications.

Pharmacodynamics and pharmacokinetics

Reduces the activity of hyaluronidase, an enzyme that plays a major role in the stabilization of hyaluronic acid and type II collagen and is involved in the destruction of the intercellular matrix.

Reduces capillary permeability, inhibits the destruction of the macromolecular structure of the main substance of connective tissue, stimulates restoration processes in loose fibrous connective tissue and cartilage tissue of joints.

The proteoglycans contained in the extract of marine fish cause a trophic effect and increase the hydrophilicity of cartilage (and, consequently, its elastic properties), as well as the homogeneity of bone tissue and the height of the cartilage.

Thanks to these properties of Alflutop the patient:

  • the intensity of pain decreases (movement, at rest, when climbing stairs);
  • local swelling decreases;
  • the severity of contracture decreases.

The use of Alflutop makes it possible to facilitate motor activity and increase the range of motion of the affected joint. Also, in patients receiving this medicine, the content of serum glycoproteins seromucoids and globulin, fibrinogen, the number of leukocytes and C-reactive protein in the synovial fluid decreases.

Upon completion of a six-month course of treatment, the level of hyaluronic acid in the synovial fluid doubles. The anti-inflammatory effect is observed after a week of treatment.

Indications for use: what are Alflutop injections for?

Injections of the drug Alflutop are prescribed for degenerative rheumatic diseases of the joints (indications for use: osteochondrosis , arthrosis of small joints , gonarthrosis , coxarthrosis ), ossification disorders (chondral and endochondral), traumatic dystoses , periodontopathy (the drug is used as an addition to the main therapy).

Analogs

Instead of Alflutop injections, the following drugs can be used:

  1. Ambene Bio is a domestic medicine that is a complete analogue of Alflutop. It is produced in injections, which can only be prescribed to adult patients, except pregnant women and breastfeeding women.
  2. Chondrolone is a substitute for the drug Alflutop in the therapeutic group. It is produced in capsules and lyophilisate, from which a solution for parenteral administration is made. The medication is used for osteoarthritis in adult patients, except pregnant women and breastfeeding women.
  3. Artra belongs to the Alflutop substitutes in the pharmacological group. The medication is available in tablets that can be used in patients over 15 years of age. It is not advisable for pregnant and lactating patients to take Artra.
  4. Tazan is a domestic drug that is used for osteoarthritis. It is produced in tablets approved for use in patients over 15 years of age, with the exception of pregnant women and breastfeeding women.

Instructions for use of Alflutop (Method and dosage)

According to the instructions for use, Alflutop injections are intended for intramuscular, paravertebral, periarticular, intra-articular administration, as well as for injection into pain points (for fibromyalgia ).

For polyosteoarthrosis and osteochondrosis, the course of treatment is designed for 20 days. The contents of the ampoule (1 ml) are injected deep into the muscle once a day.

If the pathological process has spread to large joints, injections should be made into each affected joint. Single dose 1-2 ml. The course of treatment includes 5-6 intra-articular injections, which are performed once every 3-4 days.

After the last dose is injected into the joint, Alflutop continues to be administered intramuscularly for another 20 days. After six months, the course should be repeated.

For sequestered spinal hernia, Alflutop is prescribed as an adjunct to the main treatment: the use of chondroprotectors for this pathology helps prevent further degenerative changes in the discs .

Drug therapy for hernia sequestration is carried out using:

  • NSAIDs;
  • diuretics (these drugs are prescribed in a short course to eliminate excess fluid and reduce pressure on the spinal column);
  • analgesics (in some cases it is advisable to do a novocaine blockade);
  • B vitamins;
  • muscle relaxants;
  • drugs that improve blood supply and nutrition of tissues near the source of inflammation;
  • anticonvulsants.

Currently, Aflutop is often prescribed for the treatment of hernias complicated by sequestration of the nucleus pulposus . The effect of its use is noticeable within a week after the start of the course. The natural substances contained in the solution activate the synthesis of cartilage tissue and have a positive effect on bone uniformity.

As a result, in a patient with a sequestered hernia , the intensity of pain decreases, the source of inflammation is stopped and the mobility of the vertebrae increases.

If after 2-3 weeks of treatment there is no positive dynamics, a decision is made on the need for a planned operation.

How to properly inject the drug into a joint?

After puncturing the joint capsule, you should make sure that the syringe needle is positioned correctly. To do this, an attempt must be made to aspirate the synovial fluid.

However, in most cases this is quite problematic, since the fluid filling the joint cavity, due to its high viscosity, can block the needle tip with dense particles.

In such a situation, the correct placement of the needle in the joint cavity can be judged only by the relative painlessness and ease of administration of the drug.

If difficulties arise with aspiration of fluid, there is no need to rotate the syringe needle, as this may injure the joint capsule, intra-articular ligaments, cartilage or synovium. hemarthrosis may develop .

In cases where the evacuation of synovial fluid does not cause difficulties, it should be removed (the maximum possible amount). This reduces the pressure in the joint cavity, reduces pain (including those caused by the introduction of an additional volume of injection solution) and makes the injection easier.

In addition, the synovial fluid removes the aggressive substances it contains (in particular, proteases), increases the concentration of the active substance of the drug in the joint and slows down its leakage through the lymphatic tract.

The medicine should be administered slowly, without using excessive force.

As long as the tip of the needle does not touch any dense tissue inside the joint, the injection is usually painless or painless. Too much resistance during insertion and/or intense pain may indicate that the needle has not entered the joint cavity. In this case, it should be pulled slightly towards you or, conversely, pushed a little deeper.

Other forms of release

You can find Alflutop ointment on sale. The manufacturer does not produce the drug in tablets.

Tablets are considered less effective when compared with an injection solution.

This is due to the fact that when taken orally, the active substances of the drug cannot enter the pathological focus unchanged and in the concentration necessary to achieve a therapeutic effect, since they undergo biotransformation already when passing through the digestive canal.

As for the ointment, it is a means of external therapy and is applied topically. Apply it to the site of pain projection 2 or 3 times a day. The course of treatment is up to three months.

Side effects and contraindications

Alflutop has virtually no contraindications - the medicine is not prescribed to patients under 14 years of age, with individual intolerance to the drug's ingredients, or allergies to fish.

Possible adverse reactions:

  • heat, burning at the injection site;
  • skin rashes, itching;
  • increased temperature in the injection area;
  • muscle pain;
  • temporary myalgia;
  • dermatitis.

Heat

Alflutop may cause fever

But negative reactions rarely occur; the medicine is well tolerated by patients of different age groups, provided that the dosages indicated in the annotation are followed.

Alflutop is not prescribed during pregnancy and lactation, since there is no reliable data on the effect of the drug on the fetus and child.

Alflutop or Chondrolon - which is better?

Chondrolone is a drug based on chondroitin sulfate (A and C) sodium salts, which are obtained from cattle cartilage. The drug is used to treat degenerative diseases of the spine and joints ( intervertebral osteochondrosis , primary arthrosis , etc.).

If we compare it with Alflutop, then these are completely different drugs in terms of the nature of their effect. Therefore, Chondrolone is not considered as a substitute for Alflutop.

According to the existing classification, Chondrolone belongs to the group of 2nd generation chondroprotectors Some scientists are inclined to believe that their effectiveness is higher than that of 1st generation drugs (which include Alflutop). If the effect of using the latter, in their opinion, is comparable to the effect of placebo, then newer drugs already provide clinically proven results.

Which is better - Dona or Alflutop?

The medicine (and its analogues) is a corrector of the metabolism of cartilage and bone tissue. Unlike Alflutop and Chondrolone, Dona uses glucosamine as an active substance.

Like chondroitin sulfate, glucosamine, having affinity for cartilage and a complex mechanism of action, inhibits the destruction of cartilage tissue and stimulates its synthesis.

Both of these substances are natural components of cartilage. The first is an integral part of the proteoglycan chondroitin sulfate molecule and is a high molecular weight polysaccharide from the proteoglycan group. The second is necessary for the synthesis of proteoglycans.

Both glucosamine and chondroitin (in the form of sulfate) have their own analgesic and anti-inflammatory effects, which are realized not by suppressing Pg synthesis, but by inhibiting the activity of lysosomal enzymes and superoxide radicals.

Both of these substances have a positive effect on cartilage due to their ability to suppress catabolic and stimulate anabolic processes in it. However, it is extremely difficult to definitively assess the degree of their influence on the course of osteoarthritis .

The downside of Don's drug is that it is available in tablet form. That is, doubts arise that its active substance reaches the lesion in the required concentration. It is difficult to judge the effectiveness of Alflutop due to the fact that the composition of the marine fish extract it contains is unknown in quantitative terms.

Which is better - Alflutop or Chondroguard?

Chondroguard is a generic version of Chondrolone . The active substance of this drug is sodium chondroitin sulfate. It is quite difficult to compare it with Alflutop, since it is impossible to say exactly what the concentration of chondroitin .

Description of the drug

Alflutop is not able to quickly relieve inflammation and swelling, or relieve a person from acute or dull joint pain. The drug has a different task - the regeneration of cartilage tissue. Pain and inflammation are successfully treated with NSAIDs, analgesics, and glucocorticosteroids. But only chondroprotectors, including Alflutop, affect the pathogenesis of the disease, accelerating metabolism in hyaline cartilage.

Alflutop and its analogues.

Coxarthrosis, osteochondrosis, gonarthrosis, spondyloarthrosis cannot yet be cured completely. The conservative methods used are aimed at reducing the severity of symptoms and preventing the spread of destructive changes. There is a constant search for drugs, the use of which would prevent a person from losing their ability to work. One of them was Alflutop. It has relatively recently appeared on the pharmaceutical market, but its therapeutic effectiveness has been confirmed by numerous clinical trials.

Alflutop belongs to biotechnological medicines. Its composition is represented by a sterile extract obtained from the processing of marine organisms. The injection solution contains a large amount of substances beneficial for joints:

  • amino acids;
  • peptides;
  • carbohydrates;
  • trace elements: sodium, potassium, calcium, magnesium, iron, copper, zinc.

The results of the experimental data revealed an interesting feature of the pharmacological action of the drug. After its administration, the biosynthesis of hyaluronic acid increases and hyaluronidase (the enzyme responsible for its destruction) is inhibited. Unlike other drugs, during the use of Alflutop these processes occur simultaneously. Proteoglycans and collagen are the main components of cartilage tissue. And hyaluronic acid serves as a kind of axis for the aggregation of complex proteins. Its high concentration stimulates the biosynthesis of proteoglycans and inhibits metalloproteinases that destroy cartilage. The more hyaluronic acid in the synovial fluid, the better its shock-absorbing properties. When the bone structures of the joint come into contact, there is no dangerous friction that causes wear and tear of the hyaline cartilage.

Clinical studies made it possible to establish another therapeutic effect of Alflutop. When the concentration of hyaluronic acid in the joint increases, the likelihood of developing secondary synovitis decreases. The chondroprotector has large molecules on which the mediators of pain, inflammation, fever - prostaglandins, leukotrienes, cytokines - are absorbed.

Alcohol compatibility

The effect of the drug is designed for damaged cartilage and bone tissue. The injection is given directly into the affected joint. Penetrating into the affected tissue, the active substance inhibits the destruction of the joint and stimulates the formation of healthy cartilage tissue.

For articular cartilage to function normally, it must absorb fluid into the joint cavity and then release it under load. The process of destruction of cartilage tissue is associated with a lack of water. As a result, cartilage loses its ability to recover.

One of the properties of alcohol is its ability to remove water from the body, which can negate the effectiveness of the drug.

However, the manufacturer’s instructions do not contain a categorical ban on drinking alcohol during treatment with Alflutop.

Experts recommend remembering that alcoholic drinks can enhance the effect of the drug, thereby provoking an overdose, and can, in combination with the drug, cause swelling of the respiratory tract.

How does the injection work?

Alflutop injection allows you to reduce the intensity of the inflammatory process in cartilage tissue. It causes an analgesic effect, improves cellular metabolic processes, and activates blood circulation.

The doctor kneads the joint

The drug starts tissue regeneration processes, normalizes their biological properties, and facilitates patient mobility.

It empowers them to act with more confidence.

The medicine is administered into the hip and knee joints and other large joints. The injection begins to have a therapeutic effect after a short period of time.

As soon as the solution begins to dissolve in the tissues, the patient experiences relief. Such efficiency becomes especially important in the treatment of arthrosis of the knee joint or hip arthritis. With these pathologies, the patient often loses mobility.

The injection is painful, but well tolerated even with repeated injections.

Reviews about Alflutop

Reviews from doctors about Alflutop, as well as reviews from patients on forums, are often contradictory. Some people question the effectiveness of the drug, while others, on the contrary, consider it a very effective remedy.

As disadvantages, patients mainly note the high cost of the medicinal mixture and sometimes the results from its use are not immediately noticeable.

Feedback from experts allows us to draw the following conclusion: the effectiveness of the drug directly depends on how timely treatment was started, and what medications were prescribed to the patient in addition to Alflutop.

To date, it has been proven that over a period of 3 years, patients receiving joint injections did not experience radiographic progression of osteoarthritis , and the achieved effect was maintained for several months.

Other studies conducted in a group of almost 4 thousand patients with osteoarthritis of the hip and knee joints showed that the use of chondroitin did not achieve a clinically significant effect. In patients who took chondroitin, compared with patients who received placebo, there was no reduction in joint pain, nor a slowdown in the narrowing of the joint space.

However, since many people are convinced that chondroprotectors help them, and also because there are no adverse events associated with their use, doctors do not see any harm in continuing treatment as long as it seems beneficial to the patient.

Since chondroprotectors inhibit the destruction of cartilage rather than restore it, it is advisable to prescribe these drugs not during an exacerbation of the disease, but for preventive purposes (for example, to prevent the development of degenerative changes that occur with a herniated disc ).

The Sport-Wiki (Wikipedia of scientific bodybuilding) indicates positive use in athletes.
