Pharmacodynamics and pharmacokinetics

Pharmacodynamics

An anticoagulant drug that has antithrombotic, fibrinolytic and angioprotective effects. Sulodexide is an extract of the mucous membrane of the small intestine of animals, which contains a heparin-like fraction and dermatan sulfate. Suppresses blood coagulation factors Xa and Pa, reduces the concentration of fibrinogen in the blood. Increases the concentration of plasminogen. The angioprotective effect is associated with restoration of the integrity of endothelial cells.

The drug, by reducing the concentration of triglycerides and reducing blood viscosity, normalizes rheological properties. In large doses, an anticoagulant effect occurs if the drug is administered intravenously.

Pharmacokinetics

When administered intravenously, it is quickly distributed throughout tissues and organs. About 90% of the active substance is absorbed in the endothelium. Cmax when administered intravenously is achieved within 5-15 minutes. The difference from heparins is that the active substance is not desulfated, so its antithrombotic activity is not reduced and is not eliminated from the body so quickly. It is metabolized in the liver and excreted by the kidneys—about 50% within 24 hours.

Angioflux: capsules and solution

In this medical article you can familiarize yourself with the drug Angioflux. The instructions for use will explain in what cases you can take 600 LE injections or 250 LE tablets, what the medicine helps with, what are the indications for use, contraindications and side effects. The annotation presents the forms of release of the drug and its composition.

In the article, doctors and consumers can only leave real reviews about Angioflux, from which you can find out whether the medicine helped in the treatment of thrombosis in adults and children, for which it is still prescribed. The instructions list analogues of Angioflux, prices of the drug in pharmacies, as well as its use during pregnancy.

The instructions for use classify the medicine Angioflux as a direct-acting anticoagulant.

Release form and composition

Angioflux is produced in the following dosage forms:

  • capsules: soft gelatin, oval, brick-red; contents – a suspension of white or white-gray color, pink or pinkish-cream shade is possible (10 or 25 pieces in contour strip packs, 5 or 2 packs in a cardboard pack, respectively);
  • solution for intravenous and intramuscular administration: transparent, from light yellow to yellow (2 ml in dark glass ampoules, 5 ampoules in a strip pack, 2 packs in a cardboard pack).

Active substance – sulodexide:

  • 1 capsule – 250 lipoprotein lipase units (LE);
  • 1 ampoule with solution – 600 LE.

pharmachologic effect

The anticoagulant drug Angioflux has angioprotective, fibrinolytic and antithrombotic effects. The mechanism of action of the drug is associated with the interaction of both fractions, the antithrombotic effect is due to the suppression of blood coagulation factor IIa, activated by Xa, increased production of prostacyclin, and a decrease in the content of fibrinogen in the blood.

Profibrinolytic properties are caused by an increase in the content of tissue plasminogen activator in the vessels and a decrease in the concentration of tissue plasminogen activator inhibitor.

Angioprotective properties are provided by restoring the functional and structural integrity of the vascular endothelium, as well as restoring the density of the negative charge of the pores of the vascular membrane. Under the influence of the drug, the rheological properties of plasma are normalized by reducing the content of triglycerides, as the lipolytic enzyme is stimulated.

The effectiveness of treatment for diabetic nephropathy is determined by its ability to reduce the thickness of the basement membrane and the production of extracellular matrix by reducing the proliferation of mesangium cells.

In large doses, when administered intravenously, an anticoagulant effect may occur.

Angioflux injections

The instructions for use indicate that Angioflux solution is used IV (bolus or drip) or IM, 2 ml (1 ampoule) per day.

For intravenous drip administration, the drug is pre-diluted in 150-200 ml of 0.9% sodium chloride solution.

It is recommended to begin treatment with parenteral administration of the drug for 15-20 days, after which they switch to taking capsules for 30-40 days.

Pills

In the form of capsules, Angioflux should be taken orally in between meals - 1 piece. 2 times a day for 30-40 days.

The full course of treatment is recommended to be carried out 2 times a year.

The dose of the drug and duration of treatment can be adjusted by the doctor depending on the results of the clinical diagnostic examination of the patient.

Side effects

  • nausea, vomiting;
  • hematoma at the injection site;
  • pain, burning;
  • stomach pain;
  • skin rash.

Children, pregnancy and breastfeeding

In the first trimester, medication should not be prescribed. In the second and third trimesters, the drug can be prescribed only as a last resort, if the benefit to the expectant mother outweighs the potential risk to the development of the fetus.

Reviews about Angioflux

Most often there are reviews and discussions of taking this drug during pregnancy. One of the physiological features of the female body during this period is an increase in blood clotting. High levels of estrogen will provoke thrombosis, so constant monitoring of d-dimer levels is necessary as a marker of fibrin formation. In case of positive tests for fibrin formation markers, antithrombotic therapy is immediately prescribed.

They especially monitor the state of hemostasis and pay attention to this indicator after IVF, since d-dimer after embryo transfer can increase sharply and affect implantation and pregnancy. According to patient reviews, it is mandatory to study the state of hemostasis already at the IVF planning stage, and if this indicator is higher than the norm for planning, then Angioflux was prescribed to everyone.

  • “... The doctor prescribed me Angioflux tablets for a month in preparation for IVF.”
  • “... I take two capsules a day, prescribed before IVF.”
  • “... I had a successful protocol on Angioflux, I took it before and after IVF.”
  • “... I think the drug is effective. I saw it in the protocol after taking d-dimer.”
  • “... They discovered the Leiden mutation, and the hematologist immediately prescribed Angioflux, this in addition to heparins!”
  • “... I have the Leiden mutation - hence all the problems with conception, miscarriages and failure to carry a pregnancy to term. This medicine was prescribed.”
  • “... I drank throughout my pregnancy, at first I was prescribed 2 capsules a day, and when heparins were added, 1 capsule. There were no side effects."
  • “... Before the protocol, I drank it for a month, in the protocol they gave injections until the 25th week.”
  • “...I have a blood clot after childbirth. I’ve been injecting Fraxiparin for two months, now I’m taking Angioflux.”

Medicinal properties

Angioflux is an anticoagulant and has fibrinolytic, anticoagulant and antithrombotic pharmacological properties. Sulodexide is a concentrated extract of the small intestinal mucosa of cattle.

The substance contains heparin-like elements and dermatan in sulfate form. Ultimately, after use, suppression of coagulation factors occurs, and fibrinogen content decreases, in parallel, plasminogen concentrations increase.

The drug is also capable of restoring endothelial cell structures.

Angioflux lowers levels of bad cholesterol in the body, improves the rheological properties of blood, and in large dosages acts as an anticoagulant. A more pronounced effect on the body occurs with intravenous injections.

After parenteral administration, the substance quickly disperses throughout the body, and 90% of the administered dosage enters the endothelium. The peak concentration of the substance in the blood is observed after 10 minutes.

The drug is completely eliminated from the body, without prior metabolization in the liver, through the kidneys in two days.

Directions for use and doses

The average cost of the medicine is 1,423 rubles per package.

The duration of therapy and dosage are selected individually for the patient. You can administer the medicine intramuscularly or intravenously at 600 units, up to two weeks inclusive.

Next, capsules are used after a two-week period of use, 250 units 2 times a day, for a month and a half.

Side effects and overdose

These include:

  • Painful sensations in the stomach
  • Pain and burning sensation
  • Skin rashes
  • Vomiting and nausea
  • Bruises and hemorrhages at injection sites.

Typically, an overdose of antiflux manifests itself in the form of severe bleeding. If this happens, you just need to stop the drug.

Reviews

There are not very many reviews about the drug Angioflux on the Internet; there is practically no information on the forums. In existing reports, people write about the effectiveness of the drug for pain in the legs, burning in the feet - pathologies that often accompany diabetes.

Patients note improved blood circulation and a positive effect on damaged nerves. The effect is observed within a week after the start of treatment, the condition of the venous system begins to noticeably improve.

Most patients indicated the absence of any side effects after the injections; there were practically no marks from the injections or they quickly disappeared. Against the general background, there is also an improvement in health; patients report that their legs get less tired, they can move more without additional effort, and there is no pain.

At the same time, people warn that during therapy they should not overload themselves.

Patients who completed the course of treatment emphasized that they were able to achieve the desired effect (reducing blood clotting, improving the condition of the venous system), they did not need to take other medications, with the exception of general strengthening ones.

Patients with increased blood clotting speak with caution about angioflux; in any case, the therapy process is under the supervision of a doctor.

Most sick patients were required to undergo a coagulogram to detect blood clotting; a conclusion about the effectiveness of therapy had to be made based on the data obtained.

After treatment, 85% of people did not need additional therapy over the next 6 months, 98% of patients had no problems with the cardiovascular system, pain in the legs disappeared, blood pressure normalized, and overall positive dynamics were noted.

Doctors say that angioflex is a necessary medication for diabetes; it allows you to get rid of such negative consequences of the disease as heart rhythm disturbances, attention and memory disorders. Experts write that self-medication is strictly contraindicated; therapy requires doctor’s supervision.

Conclusion

  1. Angioflux is prescribed for vein pathologies associated with increased blood clotting; the action of the active components is aimed at restoring arteries, blood vessels, and cell membranes.
  2. The drug is available in capsules and solution, the cost is quite high, there are very few analogues.
  3. The medicine is not recommended for use in the first trimester of pregnancy and has a limited range of contraindications and side effects.
  4. Angioflex practically does not enter into drug interactions; patients with open wounds and patients who have undergone surgery should be careful.
