"Bicillin-5": instructions for use, how to dilute, analogues and reviews

Recommendations for admission

In accordance with the instructions for use of Bicillin-5 (and other types of the drug), intramuscular injections should be made in the upper part of the gluteal muscle, and the syringe should be inserted deeply. The dosage for a long course is determined only by a doctor, however, there are general recommendations for the treatment of infectious pathologies, as well as their prevention. For patients of different age groups, the approximate dosage may be as follows:

  • Bicillin-1. Treatment of infections and prevention requires 1 injection per week (300,000 units for prevention and 600,000 units for treatment of the disease) or 2 injections per month, 1,200,000 units each. For rheumatism, the single dosage can be doubled (up to 2,400,000 units). The duration of the course depends on the pathology and its severity. The dosage for syphilis is 2,400,000 units with intervals between injections of 8 days.
  • Bicillin-3. A single dose is 300,000 units; if necessary, 2 injections are given in different buttocks. The interval between injections is 4 days; with a single dose of 600,000 units, the interval between procedures should be 6 days. Treatment for syphilis should be a total of 7 injections with a concentration of 1,800,000 units per injection, with the first injection being 300,000 units, and all subsequent injections with a full dose. For the recurrent form of the disease, the total number of injections is 14.
  • Bicillin-5. For adult patients - 1,500,000 units injected once a month, for children under 8 years old - 600,000 units once every 3 weeks, and for adolescents over 8 years old, a single dose is 1,200,000 units at monthly intervals.

Features of use in different population groups

Children, pregnant women and elderly people have their own individual physiological characteristics. Therefore, the pharmacokinetics, the direct effect of the drug, may be perverted.

Pregnancy and lactation period

Penicillins are not included in the list of teratogenic factors. But their use is undesirable. Therefore, there are strict guidelines during pregnancy. It is prescribed only when the benefit to the mother far outweighs the risk to the fetus. Dosages are selected individually by the attending physician, taking into account the characteristics.

The lactation period is a contraindication, since a small concentration passes into breast milk. This may cause an allergic reaction or the development of resistance to bacterial flora.

Children

This contingent is recommended to use only Bicillin 5. The difference in dosage depends on age.

For children from 3 to 8 years old, it is recommended to administer 600,000 IU once every 3 weeks. After reaching 8 years of age, the dose is doubled. It is recommended to administer 1.2 million IU once every 4 weeks. It is allowed to increase the dose to 1.5 million.

Aged people

People over 60 years of age are recommended to reduce the adult dose by 2 times. Since at this age the half-life slows down, due to which pharmacodynamics are enhanced.

Pharmacological properties

The main active ingredient "Bicillin 5" has an antibacterial effect against:

  • meningococcus;
  • streptococci;
  • listeria;
  • spirochetes;
  • enterococcus;
  • staphylococcus;
  • clostridia.

The antibiotic does not exhibit antibacterial activity and is not used to eliminate diseases caused by:

  • simple microorganisms;
  • fungi;
  • viruses;
  • rickettsia.

The mechanism of action of the drug is based on blocking a specific strain involved in the formation of the main active element of cell walls. As a result, their strength is impaired, and the pathogenic bacteria themselves are eliminated.

True, this ingredient also has some disadvantages:

  • instability to penicillins;
  • insignificant absorption in the digestive system;
  • lack of a positive reaction to gram-negative sources of pathologies.

Bicillin 5 injections are a combination of several poorly soluble salts. Due to prolonged resorption, a depot is formed at the injection site, which helps reduce the number of antibiotic administration procedures and maintain the required level of the drug in the blood.

"Augmentin"

A complex drug from the group of broad-spectrum antibacterial drugs, the structure of which includes the following active ingredients - amoxicillin and clavulanic acid.

The main microelement belongs to the rapid-response antibiotics of semi-synthetic origin, which has high therapeutic activity against most strains.

Tablets are considered a long-term drug, which fundamentally distinguishes them from other amoxicillin-based medications. With this, the drug can be used to eliminate the source of pneumonia that is resistant to penicillins.

After the medicine penetrates inside, the active substances of Augmentin quickly dissolve and are absorbed in the stomach and intestines. The drug exhibits its maximum pharmacological effect when the patient takes the drug before eating. The main microelement of the drug is evenly distributed throughout all body systems - the chest and abdominal cavities, and enters bile, sputum, and purulent discharge. The cost of the drug varies from 120 to 380 rubles.

Pharmacological properties

Pharmacodynamics

Bicillin-5 is a bactericidal antibiotic containing two long-acting benzylpenicillin salts. Prevents the synthesis of the cell wall of a pathogenic microbe. Shows activity against the following microorganisms:

  • gram-negative: Neisseria meningitidis, Neisseria gonorrhoeae, Treponema spp.;
  • gram-positive: Actinomyces israelii, Streptococcus spp. (including Streptococcus pneumoniae), Staphylococcus spp. (non-penicillinase-forming), Clostridium spp., Corynebacterium diphtheriae, Bacillus anthracis, anaerobic spore-forming rods.

Strains of Staphylococcus spp. that produce penicillinase are resistant to the drug.

Pharmacokinetics

Bicillin-5 has a prolonged effect; high concentrations of the drug in the blood remain for 28 days.

Pharmacokinetic parameters of the active components of Bicillin-5:

  • benzathine benzylpenicillin: after intramuscular administration, a very slow hydrolysis of the substance is observed, accompanied by the release of benzylpenicillin. The maximum concentration of the drug (Cmax) in the blood serum is observed 12–24 hours after injection. The long half-life (T1/2) allows for a stable and prolonged concentration of the drug in the blood: on the 14th day after intramuscular administration at a dose of 2.4 million IU, the concentration of the drug in the serum is 0.12 mcg/ml; on the 21st day after administration at a dose of 1.2 million IU - 0.06 mcg/ml (1 IU corresponds to 0.6 mcg). Benzathine benzylpenicillin is characterized by very weak penetration into tissues and complete penetration into liquids. Binding to blood proteins is 40–60%. The substance passes through the placental barrier in small quantities and is also excreted in breast milk. It undergoes biotransformation to a small extent and is excreted mainly by the kidneys unchanged. Less than 33% of the administered dose is eliminated within 8 days;
  • benzylpenicillin: After intramuscular administration, it is possible to reach Cmax of the drug in the blood plasma after 20–30 minutes, T1/2 can be 0.5–1 hour, in case of renal failure – up to 4–10 hours or more. Binds to plasma proteins by 60%. Benzylpenicillin is able to penetrate biological fluids, tissues and organs, with the exception of cerebrospinal fluid, prostate tissue and eyes. During the inflammatory process in the meningeal membranes, it passes through the blood-brain barrier (BBB). Penetrates the placenta and is excreted in breast milk. Excreted unchanged by the kidneys.

Mechanism of action and elimination time

Pharmacodynamics are the same and do not depend on the generation used. The drug has a bactericidal effect on many strains due to beta-lactams. These particles are inhibitors of peptidoglycan synthesis. Beta-lactams and the complex proteins mentioned have a similar “key” structure to the receptor. In case of exposure to the antibiotic Bicillin, beta-lactams are integrated into the synthesized genetic chain, blocking further production.

Peptidoglycan is a complex protein that is part of the protective shell of many bacteria. Without this protein, the cell cannot function normally and protect itself. Therefore, it is quickly found by macrophages of the human body, and the pathogenic agent is amenable to lysis (cleavage with the help of enzymes).

It has a great effect on gram-positive flora (Staphylococci, streptococci, diphtheria bacilli, anthrax pathogens), inhibits the growth of anaerobic microorganisms. And in some cases it is sensitive to gram-negative representatives (meningococci, Treponema pallidum, gonorrhea pathogen).

Pharmacokinetics is also an important link. Since it is the slow elimination of the drug from the body that makes it indispensable in the prevention of streptococcal complications.

Therapeutic concentration in the blood is achieved within the first hours after injection (2-6 hours). Maximum numbers can be seen after 12 hours. It is important that Bicillin 3 is in a therapeutic dose in the blood for at least 6-7 days after administration, and Bicillin 5 persists for 28 days. This is due to the slow biotransformation of the drug and its gradual release. The antibiotic is excreted primarily by the kidneys in the form of inactivated forms.

Bicillin instructions for use

Depending on the composition and concentration of active substances, bicillin is presented in the following options:

  1. Bicillin 1: instructions for use of this drug describe it as a non-synesynthetic penicillin antibiotic, which is similar in its antimicrobial action to benzylpenicillin. It is low toxic and does not have a cumulative effect. It has a long half-life, which ensures a high concentration of the active substance of the drug for a long period.
  2. Bicillin 3: the instructions for use of this drug are not fundamentally different from those for other drugs in this group. It contains benzathine benzylpenicillin, benzylpenicillin and benzylpenicillin procaine.
  3. Bicillin 5, consisting of benzathine benzylpenicillin and benzylpenicillin procaine.

These bicillin analogs have a similar effect, but they still differ from each other, and this should be taken into account when using them. The drug is administered only intramuscularly.

The main problem is that bicillin gets into the blood vessel. If this happens, undesirable consequences occur: Wanier syndrome, manifested in deterioration of vision, mental disorders (feelings of anxiety and depression). To avoid accidental release of the drug into the bloodstream, it is necessary to aspirate after inserting the needle. If blood is found in the syringe, the injection should be performed elsewhere.

Antibiotics, especially penicillin, can contribute to the development of fungal infections. Since low concentrations and a short course of treatment can lead to the emergence of strains of pathogens resistant to this drug, it is necessary to undergo a full course of treatment in doses determined by the doctor.

When using antibiotics, it should be remembered that the interaction of drugs of the bicillin group with other drugs is based on the principles of synergism and antagonism. In one case, the therapeutic effect is enhanced, therefore, the dose must be reduced. In another case, the activity of antibiotics is suppressed, in which case the doses need to be increased.

All bactericidal drugs are synergistic, while bacteriostatic drugs are antagonists. There is a rule: the stronger the drug, the more side effects it has. For bicillins they manifest themselves as follows:

  1. The circulatory and cardiovascular system reacts with anemia, thrombocytopenia, leukopenia, and hypocoagulation.
  2. Allergic reactions, especially characteristic of antibiotics of the penicillin group, manifest themselves in the form of skin rash, rashes on mucous membranes, urticaria, eosinophilia, joint pain, erythema multiforme, exfoliative dermatitis, Quincke's edema, anaphylactic shock.
  3. Fever and headache.
  4. Glossitis, stomatitis, arthralgia.
  5. The emergence of diseases associated with the activation of pathogenic fungi due to the elimination of their competitors - bacteria.
  6. The appearance of pain and induration at the site of constant injection of the drug.

Dilution of the drug and its intramuscular administration

It is necessary to wash your hands, wipe the ampoule with solvent with an alcohol wipe, open the ampoule, and take 5 ml of solvent. Wipe the stopper of the bottle at the injection site, carefully insert the needle and slowly, pressing the syringe plunger, inject the liquid. Without pulling out the syringe, quickly shake the components, after mixing completely, turn the bottle upside down, pull the syringe barrel so that only the end of the needle touches the liquid, and draw the required amount of antibiotic. The dose is calculated in medical units and may vary depending on the type of disease and the age of the patient.

After the medicinal substance is drawn into the syringe, it is necessary to replace the needle with a new sterile one and immediately inject the drug into the gluteal muscle, into its upper outer quadrant. The needle is inserted as deep as possible, leaving about 0.5-1 cm to the cannula; Bicillin is injected slowly, without jerking. The last 0.1-0.2 ml is not injected to prevent accidental entry of air into the muscle. If the drug crystallized during administration, it is necessary to remove the needle, dilute a new dose of the drug and inject the drug again.

Before administering Bicillin, the skin on the buttock is wiped with an alcohol wipe, which can be purchased at a pharmacy. It is recommended to wear sterile gloves during the injection to prevent the risk of infection.

Injections are given alternately in each buttock to reduce the likelihood of painful lumps. After the injection, used syringes and ampoules are disposed of; it is better to place the disassembled syringe and needle in a disinfectant solution for one hour, for example in liquid chlorine bleach. It is recommended to break or bend the needle so that children who accidentally find the syringe cannot harm themselves with it.

How to dilute bicillin

Compound

The drug Bicillin-1 contains the active substance: benzathine benzylpenicillin.

The composition of Bicillin-3, in addition to benzathine benzylpenicillin , also includes benzylpenicillin novocaine salt and benzylpenicillin sodium.

Bicillin-5 contains benzathine benzylpenicillium and benzylpenicillin novocaine salt.

Release form

Bicillin is available in powder form for the preparation of an injection solution, which is intended for intramuscular administration. The drug is packaged in glass bottles of 10 ml.

pharmachologic effect

Bicillin has bactericidal and antibacterial effects.

Pharmacodynamics and pharmacokinetics

The activity of this drug is aimed at various gram-positive microorganisms, anaerobic spore-forming bacilli, some gram-negative microorganisms, and so on. Bicillin is not effective against viruses that cause polio, smallpox , influenza , fungi, most gram-negative microorganisms, as well as strains that produce penicillinase.

With intramuscular administration of Bicillin-1, its slow absorption was noted. The drug is then hydrolyzed, releasing benzylpenicillin , which will continue to enter the bloodstream. The maximum concentration is reached after 12-24 hours, and the therapeutic effect appears after 3-6 hours.

The drug Bicillin-3 is also characterized by hydrolysis with slow release of benzylpenicillin . A single administration allows maintaining a therapeutic concentration in the blood for 6-7 days, the maximum concentration is achieved 12-24 hours after administration. At the same time, high penetration of the substance into physiological fluids and low penetration into tissues was established. The drug also passes through the placental barrier and enters breast milk. The drug is biotransformed to a small extent; excretion occurs primarily through the kidneys.

As a result of intramuscular administration of Bicillin-5, its increased concentration in the blood is detected already in the first hours. In this case, the therapeutic effect lasts for 28 days.

Indications for use of Bicillin

The main indications for the use of Bicillin are the treatment of various infectious diseases caused by microorganisms sensitive to the drug.

Therefore, the drug is prescribed for:

  • acute tonsillitis;
  • scarlet fever;
  • wound infections;
  • erysipelas;
  • rheumatism;
  • syphilis, yaws, leishmaniasis and so on.

Contraindications for use

The drug is not prescribed for hypersensitivity to its components and other penicillins.

Care must be taken when treating patients with bronchial asthma, hay fever, urticaria and other allergic diseases , intolerance to various medications.

Side effects

When treated with Bicillin, various side effects may develop that affect the activity of the cardiovascular system and hematopoiesis, manifesting itself in the form of anemia, thrombocytopenia, leukopenia and hypocoagulation.

Allergic reactions are also possible , for example: irritation and rash on the mucous membranes and skin, urticaria , joint pain, exfoliative dermatitis, swelling , and so on.

General undesirable manifestations cannot be excluded: headache, high temperature, arthralgia, glossitis, stomatitis , pain at the injection site. Long-term therapy can cause superinfections, the causative agents of which are microorganisms and fungi resistant to the drug.

Bicillin - instructions for use (Method and dosage)

According to the instructions for use of Bicillin-1, this drug is used as a preventive and therapeutic agent for infectious diseases. In this case, the peculiarities of the injection are that the injections are made deep, in the upper part of the gluteal muscle. The dosage and treatment regimen for various conditions is determined by the attending physician.

Bicillin-3 instructions for use recommend administering in a dosage of 300,000 units, if necessary, into different buttocks. The injections need to be repeated after 3 days, ultimately up to 600,000 units can be administered 6 days before. Therapy of primary or secondary syphilis involves the administration of a single dosage of 1.8 million units. A total of 7 injections are given according to a specific scheme: the first and second, 300,000 units each, with an interval of 24 hours, then 2 times a week. Other diseases according to indications also require the use of certain dosages, regimens and duration of treatment.

Bicillin-5, instructions for use recommend administering a dose of 1.5 million units once a month for adult patients, 600 thousand units once every 3 weeks for children under 8 years of age, 1.2–1.5 million units 1 once a month – for children over 8 years old.

Preparation of the suspension is allowed strictly before use, subject to aseptic conditions. The annotation for each type of drug describes exactly what to dilute and how to dilute Bicillin-5, and so on. Usually the contents of the bottle are dissolved in sterile distilled water, 0.9% sodium chloride solution or 0.25-0.5% novocaine solution. Then the resulting mixture is shaken well and administered immediately.

This drug is also actively used in veterinary medicine. It is suitable for cats and dogs, and is also used to treat fish and small mammals.

Overdose

Cases of overdose have not been reported in clinical practice.

Interaction

The simultaneous use of this drug with bactericidal antibiotics, for example: cephalosporins , cycloserine, vancomycin, rifampicin, aminoglycosides can cause a synergistic effect . Combination with bacteriostatic agents: macrolides, chloramphenicol, lincosamides and tetracyclines causes an antagonistic effect .

In addition, a decrease in the effectiveness of oral contraceptives or ethinyl estradiol . Diuretics, phenylbutazone, allopurinol can reduce tubular secretion or increase drug concentrations . Combination with allopurinol often causes allergic reactions , mainly in the form of skin manifestations.

Special conditions

Before starting treatment, you need to clarify how to dilute Bicillin-3 or other types of the drug. Usually a prescription is written in Latin, so it is best to immediately find out from the doctor how to inject the medicine, in what dosage and other features of therapy.

If blood appears in the syringe during an injection, it means that the needle has entered a blood vessel. In such cases, you need to remove the syringe and inject in another place. After this, the injection site is pressed with a cotton swab, but not rubbed.

Termination of therapy requires the development of allergic reactions , regardless of the form of manifestation.

During the treatment of sexually transmitted diseases, especially syphilis , microscopic and serological studies should be carried out before starting therapy and in the next 4 months.

To prevent the development of fungal infections, it is recommended to take vitamins B and C , as well as nystatin and levorin . When the drug is used in insufficient dosages or therapy is stopped prematurely, this can cause the development of resistant strains of pathogens.

When using Bicillin-3 in veterinary medicine, you also need to take into account all the prescriptions of specialists for animals, which have many features.

How to properly administer bicillin 3

asterisk1

. Please tell me what the secret is. I just can’t seem to get a normal, full injection of the drug Bicillin 3. Perhaps there are some nuances?

zubari

.
Method of administration and dosage: Injected deep IM into the upper outer quadrant of the gluteal muscle (IV administration is prohibited) at a dose of 300 thousand units.
If 2 injections are necessary, they are given in different buttocks. Repeated injections are made 4 days after the previous injection. A dose of 600 thousand units is administered once every 6 days. The main thing here is not to get into the vessel.

. Why doesn’t it work?

Mora

. asterisk1, what are you breeding with? Sometimes a poorly dissolved drug clogs the needle. Or is the dog twitching and you cannot inject the contents of the syringe?

asterisk1

. I dilute with novocaine 05%. 4 ml. I dissolve it very carefully. but it is possible to enter only half, because... the solution #34;thickens#34; and the needle gets clogged. I took another needle from 10 cc. syringe, it has a larger hole, and I read somewhere else that the needle should be absolutely dry, but the result is the same. Maybe it takes a certain amount of time to dissolve or something else?

Yenish

. asterisk1. take a needle from a 20 cc syringe. Even better is a veterinary needle and glass syringe. Why do you make bicillin with today’s wide selection of antibiotics? Half of the strains microflora have long been resistant to it.

asterisk1

. I see. Thank you very much. We will try.

vasyl

. bicillin3. 5. dissolve with distilled water. but it's very painful. That's why in the treatment In institutions it is diluted with a 0.25% novocaine solution. There is no such novocaine on sale. but you can dilute the dist. water 0.5% novocaine. The diameter of the needle does not matter. since in a concentration greater than 0.25% it still coagulates.

asterisk1

. I am already convinced that the diameter of the needle does not play a big role. Maybe you really need less concentration, otherwise it coagulates instantly. But I can’t give the injection quickly, I’m afraid it will get into the bone.

What place are you going to?
If it’s where it’s usually given to dogs intramuscularly, then you certainly won’t hit the bone. asterisk1 writes: I have already become convinced that the diameter of the needle does not play a big role.
It has. At one time they used bicillin a lot - the cow’s needle never got clogged

Marisha

. When you dilute the drug Bicilin 5, do you need to shake the bottle? And then how to fill it up, it foams and you can’t see anything. whether air got into the syringe or not

Yenish

. You should not shake it vertically, but roll it in your palms. then it won't foam. it crystallizes even more strongly in the needle than bicillin-3, so choose a needle with a large diameter.

Bilkis

. Personally, I administer this drug with a veterinary syringe with a thick needle. It is drawn normally, the needle does not get clogged. Such syringes and needles are reusable, you have to boil them.

Marta Kemerovof

. And the reason to put it? What kind of pain do you have? Enish has already written to you that there are already modern drugs. Write and the veterinarians will answer you.

Indications and contraindications

The use of Bicillin 5 is most often associated with the development of infectious and inflammatory processes. For example, an antibiotic is prescribed to people suffering from rheumatism. They give injections throughout the year to prevent relapses. Syphilis is also treated with Bicillin 5. It may be prescribed against a tropical skin infection called yaws or against leishmaniasis.

Bicillin is prescribed for the treatment of rheumatism

Bicillin can be injected to treat streptococcal infections if they are caused by pathogens sensitive to the drug. Scarlet fever and erysipelas are usually eliminated by injections. Among the more common pathologies, indications for use may include acute tonsillitis and wound infections. However, the antibiotic is given not only to people, but also to animals. The medicine is indicated for them for:

  • anthrax,
  • Emkare,
  • Pneumonia,
  • Wound infections,
  • Phlegmon,
  • Actinomycosis,
  • Horn of pigs,
  • Otitis
  • Mastitis,
  • Endometritis,
  • Urinary tract infections,
  • Septicemia,
  • Necrobacteriosis,
  • Pasteurellose.

For animals, injections are prohibited only in cases of possible allergies. There are more contraindications for people. The absolute ones include the period of breastfeeding and individual sensitivity to the composition. Relative contraindications that require additional consultation with a doctor include bronchial asthma, pseudomembranous colitis and renal failure. People prone to various allergies and pregnant women should also talk to a specialist.

Indications for use

Over the long existence of antibiotics, bacterial microflora has managed to mutate and develop resistance to their action. In our case, bacteria have learned to produce beta-lactamases capable of breaking down beta-lactams. To protect against inactivation of action, clavulanic acid and sulbactam with tazobactam are added to penicillins.

Since this product does not have protection, it is more often used to combat the consequences of the activity of group A beta-hemolytic streptococcus, gonorrhea and treponema pallidum.

Indications for use are:

  • syphilis;
  • gonorrhea;
  • erysipelas;
  • severe scarlet fever;
  • prevention of rheumatism after a complex sore throat;
  • tonsillitis;
  • leishmaniasis;
  • treatment of streptococcal carditis and glomerulonephritis.

An advantage in the treatment of the above diseases is given to Bicillin due to its long-term cumulative ability.

Instructions for use Bicillin 5

Before giving an injection at home, you need to read the instructions (dosage, duration of treatment for various diseases, special instructions, possible negative effects, injection technique and algorithm for diluting the powder).

For adults, the dosage of Bicillin is 1,200,000 units + 300,000 units (total 1,500,000 units). The medicine is administered once every 4 weeks. For preschoolers, the injection solution is administered once every 21 days at a dosage of 480,000 units + 120,000 units (in total 600,000 units). For schoolchildren, the dosage is 960,000 + 240,000 units (once every 28 days).

How to dilute and inject Bicillin 5

The algorithm for preparing the suspension is as follows:

  1. Open the bottle and prepare the solvent.
  2. 5-6 ml of liquid is injected into the antibiotic bottle. This can be a solution of sodium chloride, sterile distilled water or Novocaine. The solvent is introduced slowly (5 ml over 20-25 seconds).
  3. Shake the bottle thoroughly until a homogeneous solution is obtained (normally, small bubbles may remain on the walls).

Incorrect dilution of Bicillin is dangerous. The resulting suspension is used immediately. It cannot be stored for a long time. This is the main feature of the introduction of Bicillin. The main stages of this manipulation are:

  1. Preparation of materials. For intramuscular injection, you will need a freshly prepared suspension, a syringe with a needle of the proper section and length, alcohol for cleaning hands, gloves and skin, a tray, sterile cotton balls and gloves.
  2. Hand washing and disinfection. First they are washed with soap and then wiped with alcohol.
  3. Putting on gloves. They are also treated with an antiseptic.
  4. Syringe assembly.
  5. Giving the patient the necessary position (lying on his stomach or side) and creating a trusting relationship with him.
  6. Suspension kit.
  7. Treating the injection site with cotton wool. The syringe needle is inserted perpendicular to the skin deep into the muscle. The buttock is visually divided into 4 parts, and an injection is made in the outer upper quadrant.

Mechanism of action and elimination time

An antibiotic does not kill bacteria, but makes it difficult for them to multiply. This is ensured due to the constantly high concentration of the drug in the tissues and its circulation in the blood. 14 days after injection of 2,400,000 units of antibiotic, its concentration in the blood does not exceed 0.12 μg/ml. When 1,200,000 units of the drug are administered after 3 weeks, the concentration reaches 0.06 mcg/ml. In 8 days, more than 30% of benzathine benzylpenicillin is eliminated. The half-life of benzylpenicillin novocaine salt is 30-60 minutes. With kidney dysfunction, it increases to 4-10 hours or more.

Side effects and overdose

There are no data on antibiotic overdose. When using Bicillin-5, the following undesirable effects are possible:

  1. Damage to mucous membranes. Glossitis and stomatitis often develop. Glossitis is manifested by swelling of the tongue, its redness, impaired taste, pain and burning. With stomatitis, pain, ulceration and hyperemia of the mucous membrane are noted.
  2. Increased body temperature.
  3. Joint pain.
  4. Exudative erythema.
  5. Hives (skin rashes with blisters and itching).
  6. Exanthema.
  7. Exfoliative dermatitis.
  8. Anaphylactoid reactions.
  9. Anaphylactic shock. It has a protracted course. It is characterized by rashes (urticaria), depression and loss of consciousness, itching, arterial hypotension, noisy breathing, rapid pulse, sweating, cyanosis of the skin, nausea, dizziness and anxiety.
  10. Angioedema angioedema. It causes swelling of the tissues of the upper body. Asphyxia is possible as a result of narrowing of the airways.
  11. Laboratory abnormalities (decrease in red blood cells and hemoglobin, decrease in platelets and leukocytes, slow blood clotting).

If severe side effects develop, the antibiotic must be replaced. Analogues of this medication are Benzylpenicillin, Bicillin-1, Bicillin-3 and protected penicillins Augmentin, Amoxiclav and Flemoxin Solutab. Some antibiotics of other groups are available in ampoules and are not diluted. Bicillin-5 analogues may be cheaper and more effective.

How to dilute bicillin

Preparing for the injection

The drug is diluted immediately before use; storing the diluted antibiotic is not permissible. For dilution, use sterile water for injection, physiological sodium chloride solution in ampoules or 0.25-0.5% novocaine solution. In the latter case, pain due to the anesthetic effect of novocaine will be less pronounced. Do not use a cold solution for dilution. It is better to warm the ampoule in your palms for 5-8 minutes.

To administer Bicillin, you must use syringes with long needles. You will also need sterile needles in individual packaging, since the drug is diluted using one needle and injected with another. This is due to the fact that the drug quickly crystallizes and can clog the needle.

The antibacterial drug Bicillin belongs to the group of natural penicillins. It has a bactericidal effect and has a wide spectrum of effects. Gram-positive and gram-negative bacteria, some protozoa, are susceptible to harmful effects. The product is available only in bottles with white powder for dilution. Method of administration: intramuscular administration only.

special instructions

The medicine for children and adults is used with caution in cases of asthma, renal failure and pseudomembranous colitis. The antibiotic must not be administered by other means (into the spinal canal, subcutaneously or intravenously)

If it is administered intravenously, Wannier syndrome may develop. It is manifested by deterioration of vision, anxiety and a depressed state of a person. Due to the high risk of developing fungal diseases, it is recommended to use vitamins C and group B in parallel with the administration of Bicillin-5.

Pregnancy and lactation

To exclude a negative effect on the fetus, Bicillin-5 injections can be given to a woman during pregnancy only if the benefit outweighs the potential harm. This is due to the ability of the drug to penetrate the placenta and disrupt the development of the fetus. The use of antibiotics in the 1st trimester is especially dangerous, since during this period vital organs and systems are formed. When administering an antibiotic during lactation, you should temporarily stop feeding your baby breast milk.

Use in childhood and old age

For children, the medicine is prescribed at a lower dosage according to indications. For older people, the dosage regimen is standard.

For impaired renal function

Due to the fact that the antibiotic is excreted predominantly in the urine unchanged, the drug is used with caution in renal failure, when glomerular filtration and creatinine clearance are reduced

Drug interactions

There is poor compatibility of Bicillin-5 with the following medications:

  1. Bacteriostatic antibiotics of other groups (lincosamides, tetracyclines, macrolides, Chloramphenicol). In this case, the drug does not help much.
  2. Oral contraceptives (their effectiveness decreases).
  3. Allopurinol (severe allergic reactions are possible).

Phenylbutazone, diuretics, tubular secretion blockers and NSAIDs contribute to an increase in the concentration of benzylpenicillin in plasma

Reviews from doctors

Since this drug has been on the pharmaceutical market for a long time, there are many reviews about its effectiveness. Every doctor has tried it in action.

Based on expert reviews, it is effective in the treatment of chronic diseases caused by flora sensitive to beta-lactams, as they require long-term therapy. If Bicillin 1 had more pronounced side effects, then the new generation treats diseases with rare side effects.

The antibiotic received positive reviews in the prevention of rheumatic fever in children and adults. After a course of Bicillin 5, the incidence of sore throat decreased by 82%, which indicates its high effectiveness.

Dosages and regimen of use

The antibiotic can be diluted with novocaine, water for injection or saline.
Antibiotic Bicillin 5 is used exclusively intramuscularly. For this purpose, a special suspension is prepared from the powder:

  • To prepare a suspension, you can use a solution of novocaine or procaine (0.25-0.5%), sterile water for injection, isotonic sodium chloride solution.
  • A solvent in a volume of 5-6 ml is slowly introduced into the bottle with the powder under pressure.
  • The contents of the bottle are mixed by shaking until a suspension is formed.

The suspension is made immediately before administration and under aseptic conditions. The resulting solution is injected deep into the upper quadrant of the buttock intramuscularly. However, after administering the injection, you should not rub the gluteal muscle.

Cost and analogues

Retarpen is an analogue of Bicillin 5, has the same composition and mechanism of action.
The price of Bicillin 5 per bottle is very low, even in Moscow. 10 ml of medicine can be bought for about 20-30 rubles. The cost increases if you need to purchase a large amount of the drug, for example, for a hospital hospital. In this case, you can buy Bicillin 5 for about 700-800 rubles. The package will contain fifty bottles of powder.

Analogues of Bicillin 5 are represented by Extensillin, Retarpen, Moldamine and other drugs. They are made on the basis of the same active component, and therefore their instructions for use are almost identical. For example, Retarpen is also a powder for preparing a suspension, which is made from benzathine benzylpenicillin. It is indicated against rheumatism, scarlet fever, syphilis, erysipelas and acute tonsillitis.

The medicine can lead to headaches, fever, anemia, stomatitis, exfoliative dermatitis and other adverse reactions similar to Bicillin 5. In case of overdose, patients experience confusion, neuromuscular irritability, encephalopathy and seizures. Retarpen is available with a prescription. The cost reaches 2000 rubles.

Possible analogues

As a replacement for the drug, medications based on benzylpenicillin salts are most often recommended. This also includes semi-synthetic drugs based on penicillin, which have a wide spectrum of antibacterial action and are resistant to penicillinases. Some of these medications worth noting are:

  • Smallpox. An antibiotic based on phenoxymethylpenicillin, available in tablet form and also in the form of syrup. Indications include infections of the respiratory tract, ENT organs, skin, pneumococcal and streptococcal pathologies, as well as bacterial endocarditis.
  • Retarpen, Extensillin. The drugs are almost direct analogues of Bicillin (with the exception of dosages). Retarpen bottles with powder for suspension are specially divided by age category (for adults and children).
  • Amoxicillin. Analogous to the penicillin group. The drugs are among the most common antibiotics; Their advantages include a wide spectrum of action and a relatively rapid onset of therapeutic effect. However, the list of side effects of the drugs is also quite long, so it is necessary to carefully study the instructions for use for possible contraindications.
  • Ampicillin. Despite the common list of indications with Bicillin, ampicillin has a significant drawback - it is destroyed by penicillinases, which makes it less effective in some cases. Its use may be justified in the presence of intolerance to benzylpenicillin, as well as in cases of serious infectious diseases of the respiratory tract.
  • Cloxacillin. Medicines based on cloxacillin are prescribed in the presence of benzylpenicillin-resistant strains of microorganisms. The drug can also be used to treat osteomyelitis and some infectious diseases of the musculoskeletal system.

To the list of analogues it is worth adding drugs based on puricillin, picillin, standardcillin, and oxacillin. The trade name of these medications is Ranklav, Rapiclav, Oxamp, Ospamox, Libaccil, Medoclav, Prostaflin, Sultasin, Tazocin, Timentin and others.

Generics

"Bitsillin-5" has analogues in the form of various salts of benzylpenicillin produced in Russia. Other drugs based on penicillins are considered semi-synthetic substitute drugs that are resistant to penicillinase and have a wide range of antibacterial effects.

These are medications based on ampicillin and amoxicillin, available in the form of tablets and capsules. Analogues in composition:

  1. "Moldamin."
  2. "Retarpen".
  3. "Extencillin."
  4. "Benzicillin-1".

Substitute drugs according to the spectrum of action:

  1. "Amoxiclav".
  2. "Augmentin".
  3. "Ampioks".

Contraindications to the use of the drug

The main contraindication to the use of this medicine is the patient's individual intolerance to the components

Attention should be paid to the presence of an allergy to cephalosporins and sulfonamides in the anamnesis. They have a similar structure and principle of action

This medicine should not be used by sick people with bronchial asthma or those with hay fever.

Particular attention should be paid to patients who suffer from other allergic pathologies (hay fever, food and contact allergies)

This medicine is prescribed with caution to pregnant women, and also to sick people with chronic kidney failure and pseudomembranous colitis.

Drinking alcohol is also prohibited. If acetaldehyde enters the body, the effect of the drug is usually minimized or may be completely distorted. This will certainly slow down the achievement of the desired results and increase the risks of developing resistance.

Now we will find out what side effects are possible when using this remedy.

special instructions

Bicillin 5 should not be used if there is hypersensitivity to the components of the drug and if there is an individual intolerance to elements such as procaine, penicillin, and similar beta-latam antibiotics.

Bicillin 5 is used with caution and under strict medical supervision for:

  • Impaired kidney function;
  • Pseudomembranous form of colitis;
  • Allergic pathologies (hay fever, bronchial asthma).

Treatment with the drug during pregnancy and lactation is possible, if permitted by a doctor. But during the period of bearing a child, it is strictly forbidden to use Bicillin 5 in the first trimester, and during lactation, while using the medicine, breastfeeding should be stopped for a while.

Bicillin 5 can cause serious negative effects, which most often occur when the course of treatment and dosage of the drug is violated.

After the first injection you may develop:

Allergic reactions: exfoliative dermatitis, itching, fever, swelling, urticaria, chills, anaphylactic shock, arthralgia, Quincke's edema, exudative erythema multiforme.

Fever may occur after antibiotic administration

  • Laboratory abnormalities: hypocoagulation, thrombocytopenia, leukopenia, anemia.
  • Other reactions: stomatitis, glossitis.

In case of an overdose, negative reactions from the central nervous system may appear: pain in the back of the head, reflex excitation, convulsions, vomiting, arthralgia, myalgia.

In case of such manifestations, it is recommended to stop treatment with the drug and carry out symptomatic therapy.

Drug interactions:

  • With bactericidal antibiotics (rifampicin, vancomycin, aminoglycosides, cephalosporins) - enhance the effect;
  • With bacteriostatic antibiotics (tetracyclines, chloramphenicol, lincosamides, macrolides) - weaken the effect;
  • With indirect-acting anticoagulants, their effectiveness increases;
  • With ethinyl estradiol and oral contraceptives, their therapeutic effect is reduced.
  • With allopurinol, the risk of allergic reactions increases.

Treatment of syphilis with Bicillin 5 is carried out with microscopic and serological studies before starting therapy and in the next 4 months. It is recommended to simultaneously take B vitamins, vitamin C and systemic antifungal agents.

Precautionary measures

  • If blood forms in the syringe during an injection, this indicates that the needle has entered a blood vessel. Actions: you can take out the needle and inject in another place. After administration of the drug, the affected area is treated with a sterile treated cotton swab. You should not rub your buttock after the injection is given.
  • If an allergic reaction is observed during the injection on the skin - itching, redness and other manifestations, then treatment is stopped. If certain signs of anaphylactic shock occur, urgent measures are taken.
  • If Bicillin-5 is used against sexually transmitted diseases, then before starting treatment it is important to undergo a series of studies.
  • There is a risk of developing a fungus during the injection, so vitamins (group B, C) are sometimes prescribed along with the drug. Early termination of the treatment process and use of the drug in small doses leads to the formation of pathogen strains.
  • If streptococcus is being treated, it is necessary to consult a specialist before use, because it may have a negative effect.

Bicillin 5 instructions for use

This drug belongs to the group of combined bactericidal antibiotics. It consists of two benzylpenicillin salts. Its characteristic feature is the duration of action.

Bicillin 5 inhibits the synthesis of cell walls of a number of microorganisms. This group includes gram-positive microorganisms: Staphylococcus spp. Streptococcus spp. Streptococcus pneumoniae, Corynebacterium diphtheriae. Bicillin 5 is also active against anaerobic spore-forming bacilli, as well as gram-negative microorganisms such as Neisseria gonorrhoeae, Neisseria meningitidis, Treponema spp.

The drug is inactive against strains of Staphylococcus spp. producing penicillinase.

Bicillin 5 is used to treat the following diseases:

  • any infectious-inflammatory diseases in the acute period;
  • periodic courses to prevent relapses of rheumatism;
  • syphilis;
  • yaws;
  • all cases of streptococcal infections.

The drug is a highly effective means of fighting infections. This high efficiency is due to its low biotransformation in the body, as well as the duration of maintaining constantly high concentrations in the body. The elimination of bicillin is slow: the weekly amount of the administered drug is removed by only 33%.

Contraindications for bicillin 5 and its analogues include:

  • allergies (since this is a drug of the penicillin group, an alarming factor is an allergy to fungi, especially mold);
  • breastfeeding period;
  • bronchial asthma (due to a high incidence of attacks due to an allergic reaction);
  • pregnancy (except in cases where it is necessary to treat both mother and child);
  • renal failure;
  • pseudomembranous colitis.

In general, the drug has very few contraindications. Most often it cannot be used due to an allergic reaction and kidney failure. In the latter case, the drug is not discontinued, but its dose is reduced - from 75 to 25% of the recommended daily dose.

Bicillin injections (3 and 5): what does the drug help with?

Experts recommend antibacterial medication for pharmacotherapy in people affected by one or another infectious process provoked by pathogens susceptible to active substances.

Main indications:

  • diseases of the upper respiratory structures - tracheitis, pharyngitis, tonsillitis;
  • infectious diseases of ENT organs;
  • scarlet fever;
  • focal pneumonia;
  • erysipelas;
  • syphilis;
  • gonorrhea;
  • yaws.

For prophylactic purposes, the drug is used for the chronic course of rheumatoid diseases.

Important! Only a specialist should decide on a course of administration of a medication with pronounced antimicrobial activity. Self-medication is absolutely unacceptable, since the risk of severe complications is high.

Peculiarities

The drug has a number of special instructions for use:

  1. It is prohibited to administer the medication intravenously.
  2. After the procedure, the place where the drug was injected should not be rubbed; it is necessary to lightly press down and hold for a while.
  3. If signs of allergy occur, the drug should be discontinued.
  4. If anaphylaxis occurs, emergency measures must be taken: use Norepinephrine and administer glucocorticosteroid medications.

In addition, microbiological tests must be carried out before starting therapy and for four months after it if syphilis is suspected.

To prevent the development of fungal diseases, it is important to take ascorbic acid and B vitamins. Using small dosages or completely canceling treatment before the prescribed course can lead to the emergence of resistant viruses

When using Bicillin-5 together with antibacterial agents that have a bactericidal effect, antibiotics increase the effect of the first drug, and those with a bacteriostatic effect reduce it. When using the medication, the effectiveness of tablet contraceptives decreases. Diuretic drugs, for example, allopurinol, phenylbutazone, reduce the excretion of ornidazole, resulting in an increase in its plasma content.

"Amoxiclav"

The drug belongs to the antibiotics of a wide spectrum of influence. "Amoxiclav" is active against a huge number of bacteria, which are the sources of various infectious diseases. The medication is considered a representative of the therapeutic group of a combination of semisynthetic antibacterial agents of the penicillin series.

"Amoxiclav" is available in the form of tablets for oral administration, fourteen pieces per package. The main active microelements of the drug are amoxicillin.

The main components of the drug are absorbed from the intestines. Their plasma levels reach pharmacological levels within thirty minutes after taking the capsule; the maximum concentration is reached after approximately 120 minutes.

The cost of the drug is 100-700 rubles.

Side effects of Bicillin

As a result of taking Bicillin, the following adverse reactions may develop:

  • bronchospasm;
  • interstitial nephritis;
  • tinnitus, headache and dizziness;
  • stomatitis, nausea, diarrhea, inflammation of the tongue, impaired liver function;
  • disturbances in blood pressure, disruption of myocardial function;
  • thrombocytopenia, eosinophilia, leukopenia, hemolytic anemia, agranulocytosis;
  • dermatitis, angioedema, urticaria, chills, anaphylactic shock, skin rashes, fever, swelling;
  • superinfection caused by drug-resistant microorganisms (especially in weakened patients).

Interaction with other drugs

It is not recommended to combine administration of the drug with non-steroidal anti-inflammatory drugs, ethinyl estradiol, or oral contraceptives. The drug increases the effectiveness of indirect anticoagulants. In combination with aminoglycosides, macrolides, and sulfonamides, the drug exhibits a synergistic effect. The effect of the drug is reduced by bacteriostatic drugs (chloramphenicol), and its activity is increased by beta-lactamase inhibitors. Diuretics, allopurinol, tubular secretion blockers, non-steroidal anti-inflammatory drugs block tubular secretion, resulting in an increase in the concentration of the drug in the blood plasma. When used together with allopurinol, the risk of developing a skin rash increases. Benzylpenicillin reduces the clearance and increases the toxicity of methotrexate.

Release form and composition

Bicillin 5 is supplied to the pharmacy in the form of a powder, which is subsequently diluted to obtain a suspension for intramuscular administration. Initially, the substance is white or yellowish in color and is prone to clumping. The active component of the drug is benzathine benzylpienicillin. Based on it, several versions of drug bottles are manufactured, which differ mainly in dosage:

  • Bicillin 1. Contains either 600,000 units of the active component, or 1,200,000 units.
  • Bicillin 3. In addition to benzathine benzylpenicillin, this powder contains benzylpenicillin sodium and benzylpenicillin novocaine salt. They are contained in quantities of 200,000 units or 40,000 units.
  • Bicillin 5. Consists of 1,200,000 units of benzathine benzylpenicillin and 300,000 units of benzylpenicillin novocaine salt.

Thus, Bicillins 3 and 5 contain not only the main active component, but also novocaine salt. These options are supplied to pharmacies in packages that hold different numbers of vials. You can buy a pack containing one, five, ten or fifty bottles. When purchasing large volumes of the drug, it is necessary to strictly observe their storage conditions. Bicillin 5 can be kept at a temperature of 15 to 25 degrees. This should be a dry place, protected from ultraviolet rays.

Characteristic

Any type of medicine is available in identical 10 ml bottles. There are 50 bottles in a package. Inside there is a homogeneous white powder. Bicillin 3 has its own characteristics. In this generation of the drug, the color may be white or light yellow.

The powder is practically odorless. The taste is bitter. After contact with water, it takes on a homogeneous structure, forming a stable suspension. If the resulting solution is not administered within 1 hour, the properties change. The solution becomes loose with many small lumps, which makes further administration impossible.

The price of the antibiotic Bicillin is very low. Now Bicillin 1 has almost gone out of circulation, since it was replaced by a similar Bicillin 5. But if you can find it, then the average price for 1 bottle of such salt will be 10 rubles. Bicillin 3 and 5 do not differ much in the price category, the range ranges from 30-50 rubles per bottle.

Shelf life must be indicated on each individual bottle. The duration of possible use is 3 years. It is advisable to store at a temperature of 8-15°C in a dark place protected from children. If the conditions are violated, the active substance may lose its properties.
