Clopixol is an antipsychotic drug used to treat schizophrenia, bipolar disorder, and cognitive decline. The drug first appeared on the market back in 1964 and currently continues to be actively prescribed by psychiatrists. It has three trade names on the market - Clopixol, Clopixol Depot and Clopixol-Acufaz.

Pharmacological properties

"Clopixol" is an antipsychotic that has an exceptional inhibitory effect. Depending on the dosage, it can be used as a sedative, indispensable in the treatment of acute manifestations of psychosis. Using its inhibitory effect on the patient, experts prescribe it for all types of anxiety, hostile states, aggression or excessive agitation.

There are different types of the drug that have different degrees of impact on the patient’s psyche, but contain the same active substance. Their purpose depends on the clinical picture of the disease, the patient’s medical history and the stage of treatment of the disease. There are three forms of the drug:

  • "Clopixol";
  • "Clopixol-acufase";
  • "Clopixol depot".

A single dose of Clopixol-Acufaz has a quick and effective effect on the patient’s psyche and can reduce psychotic symptoms for a period of up to 2-3 days. As a rule, 1-2 injections are enough to achieve the required therapeutic effect.

Due to the pronounced addictive effect of zuclopenthixol, these injections are prescribed in the first stage of treatment until a calm state is achieved. After this, you can reorient the patient to take the tablet form of the medication.

The nonspecific sedative effect of Clopixol tablets occurs 2 hours after ingestion, the maximum level of effect is reached after 7-8 hours, then the reaction slows down, subsides and becomes less pronounced with a second dose.

The therapeutic effect of Clopixol Depot in comparison with Clopixol tablets is more prolonged, which helps to achieve positive dynamics when treating patients who do not always follow the doctor’s instructions. This type of drug prevents frequent relapses if the patient interrupts therapy.

Indications for use

The dosage form "Clopixol" according to the instructions is used for any psychotic manifestations accompanied by hallucinations, paranoia, delusions, and disruptions in mental activity. Similar symptoms are characteristic of acute and chronic schizophrenia, manic-depressive psychotic state, mental retardation accompanied by psychomotor agitation, senile dementia with signs of spatial orientation failure and confused consciousness.

In the initial period of therapeutic measures to relieve acute manifestations of psychosis (including cases of mania) and the chronic type in the acute stage, “Clopixol-acufaz” is usually prescribed.

The scope of use of "Clopixol Depot" is limited to schizophrenia in any form of its manifestation, accompanied by hallucinations, delusions, and mental disorder. The drug of this form helps to relieve the state of agitation, hostility and aggressiveness.

Analogues of the drug

In case of severe individual intolerance to the drug, you can use one of the analogues of Clopixol. Among the most popular are Clozapine, Ketilept, Quetiapine and Haloperidol. Let's look at them in more detail:

  1. Clozapine
    Clozapine is an atypical antipsychotic. Its undoubted advantage is that it does not cause extrapyramidal disorders or drug-induced parkinsonism. Even among atypical antipsychotics, it is the safest. It is easy to find on the market under the name Azaleptin.

    Chemically, Clozapine has a similar structure to both tranquilizers and antidepressants. Unfortunately, it has a rather dangerous side effect - it reduces the number of granulocytes up to agranulocytosis. Therefore, it is not used as a first-line remedy, but only in case of ineffectiveness of other antipsychotics.

  2. Ketilept is an atypical antipsychotic that acts as an antagonist of neurotransmitter receptors in the brain. Used in the treatment of schizophrenia and bipolar disorders. Not used in children under 18 years of age due to lack of clinical data.

    The increased risk of suicide must be taken into account. An influx of suicidal thoughts in a patient may go undiagnosed for a long time. Ketilept dosage regimens depend on the individual sensitivity of the patient.

  3. Quetiapine
    Quetiapine is an atypical antipsychotic prescribed to patients with acute and chronic schizophrenia, major depressive disorder and other mental disorders. It is prescribed orally 2 times a day. May cause drowsiness, dyspepsia, sedation, decreased blood pressure.

    Based on the results of modern experimental studies, one can judge an increased risk of malignant neoplasms in mice administered Quetiapine. It is similar in effectiveness and frequency of side effects to Haloperidol.

  4. Haloperidol is a synthetic antipsychotic for parenteral and oral use. Its main feature is the possibility of use not only in adults, but also in children. Effectively relieves mania, paranoid psychoses, hyperactivity, aggression, and tendency to self-harm in mentally retarded people. Contraindicated in patients with heart failure, parkinsonism, ventricular arrhythmia.

Dosage forms and composition. Storage rules

"Clopixol" is sold in biconvex film-coated tablets. In accordance with the concentration of the main component, their colors differ:

  • pale pink (dosage 2 mg);
  • pink-brown (dosage 10 mg);
  • red-brown (dosage 25 mg).

In cross section, all tablets are white. They are packed in plastic bubbles of 50-100 pieces, the outer packaging is a cardboard box.

Pills

The active ingredient of the drug is zuclopenthixol dihydrochloride. Among the excipients, it is possible to use castor oil, potato starch, lactose monohydrate, glycerol, microcrystalline cellulose, copovidone, talc and magnesium stearate. The outer capsule may contain hypromellose, macrogol, iron oxide and titanium dioxide.

The active ingredient of “Clopixol Depot” is zuclopenthixol decanoate, and “Clopixol-acufaz” is zuclopenthixol acetate. These dosage forms are available in glass ampoules filled with liquid for intramuscular use. "Clopixol Depot" is a transparent, oily liquid substance of a yellowish tint, bottled in transparent 1 ml ampoules of colorless material, packaged in cardboard boxes.

Preparations containing zuclopenthixol are stored at a temperature of no more than + 25 degrees. The expiration date is written on each medicinal unit. Packages with “Clopixol depot” and “Clopixol-acufaz” must be protected from the penetration of light rays and placed out of the reach of children and pets.

Instructions for use of "Clopixol" and its dosage forms

The dosage and regimen of taking the drug is determined by a specialist. He analyzes the clinical picture of the course of the disease, calculates the required amount of active substance required by the patient per dose.

Clopixol tablets are prescribed orally, dividing the daily dose into several parts. In the first phase of treatment, it is recommended to take the medicine in small portions, observing how the patient’s body reacts. Gradually the amount of medication is increased until a positive result occurs.

In case of acute manifestations of schizophrenia, mania and severe agitation, the patient is prescribed 10-50 mg of the drug per day. The initial dose can be divided into 10-20 mg with a cycle of 2-3 days and increased to 75 mg. The maximum you can take per day is up to 150 mg.

Instructions for use of "Clopixol" contain detailed information about the dosage of the drug for a particular disease, but self-administration of the drug is unacceptable. The sedative effect of Clopixol can cause addiction to the main active ingredient.

Clopixol-acufaz ampoules are prescribed as intramuscular injections into the gluteal region. Injections cannot be used for a long time. Most often, a dose of 1-3 ml is recommended. Increasing the portion of the active substance or reducing the interval between administration of the drug is allowed only on the recommendation of the attending physician.

During treatment, it is possible to switch from taking one form of the drug "Clopixol" to another. Intramuscular injections can be replaced by taking tablets orally and vice versa. Zuclopentaxol decanoate has a dosage-dependent sedative effect. The instructions for use of Clopixol Depot ampoules contain information about a possible transition to this drug from Clopixol-Acufaz tablets or injections. In this case, the sedative effect may be reduced. The active component of Clopixol Depot is broken down when it enters the body. Its maximum concentration in blood serum is recorded after 7 days of administration. Half-life occurs after 3 weeks. After a second course, this period increases to 3 months.

When switching to the use of tablets or depot form, you should adhere to the following scheme:

  1. When switching to tablet form - 40 mg 2-3 days after the last injection. It is recommended to divide the dose into several doses. If necessary, the dosage can be increased by 10-20 mg over 2-3 days.
  2. When switching to the depot form - 1-2 ml after administration of zuclopenthixol acetate. The medicine is administered intramuscularly every 2-4 weeks. If necessary, the dose can be increased or the intervals between injections shortened.

The instructions for using Clopixol for injections indicate the possibility of mixing injectable forms with zuclopenthixol in one syringe. After such a procedure, Clopixol Depot is prescribed individually. A local reaction to injections may occur, so it is recommended to administer no more than 2 ml of solution at a time. Large doses can be divided into two doses.

Contraindications and effects on other medications

The instructions for Clopixol contain a list of contraindications for use. It is not recommended to use the drug in cases of acute forms of alcohol and barbiturate intoxication, exposure to opioid analgesics, or coma.

Contraindications

This drug is contraindicated in acute alcohol intoxication , as well as acute intoxication with opioid analgesics and barbiturates , coma , hypersensitivity to the components of the drug.

Overdose

Taking into account some reviews, the instructions for use of “Clopixol” indicate cases of overdose. This mainly applies to oral administration. An overdose of an injectable drug is unlikely. Changes in ECG parameters and arrhythmias may occur.

  • increased heart rate, tachycardia;

  • anuria, oliguria, polyuria;
  • drowsiness, hyperkinesia, hypokinesia, akathisia;


    Akathisia

  • dry mouth, hypersecretion of saliva;
  • constipation, diarrhea, dyspepsia, nausea;
  • nervousness, anxiety, sleep disturbances, decreased libido, insomnia, depression;
  • myalgia;
  • hyperhidrosis, scabies;
  • increased appetite, weight gain;
  • fatigue, constant feeling of tiredness and depression.

Rarely occurring side effects include the following:

  • increased blood pressure, unhealthy blush on the face;
  • erectile disfunction;
  • apathy, increased libido, nightmares;

  • decrease in the number of leukocytes, granulocytes, neutrophils, platelets in the blood;

  • migraine.

If the patient experiences hyperthermia or hypothermia, drowsiness, extrapyramidal disorders, convulsions, and even impaired consciousness (even coma), an overdose of Clopixol should be suspected.

There is no etiological treatment for overdose, so the set of emergency measures is standard: it is necessary to perform gastric lavage as quickly as possible and administer intravenous drips of norepinephrine to avoid shock. Seizures are treated with benzodiazepines, and extrapyramidal disorders are treated with antiparkinsonian drugs (Soleron).

Special instructions when using Clopixol

It is not advisable to take the drug during pregnancy and breastfeeding. There is evidence that when the mother was treated with antipsychotic drugs, breastfed children showed signs of intoxication (overexcitability, tremor, lethargy).
