Composition and release forms of the drug

Lokoid is a drug belonging to the pharmacological group of topical glucocorticosteroid drugs.

Depending on the percentage of water and active ingredients, the product is available in several dosage forms:

  1. Emulsion (Locoid Crelo) consists of 85% water, so it is used for delicate skin. Lokoid emulsion is used for seborrhea to soften the scalp.
  2. Cream has a water to lipid ratio of 70/30. The light texture of the cream allows you to use it at any time of the day or night. It is quickly absorbed and does not leave greasy marks on the skin and clothes. It is used if itching and irritation are not accompanied by cracks and flaking (excessive dryness).

  3. Lipocrem It contains 30% water, and the rest is lipids and nutrients. The drug softens the crust, eliminates flaking, and effectively fights eczema and dermatitis.

  4. Ointment It has a greasy consistency and is used if itching and inflammation are accompanied by dryness and flaking.

Regardless of the form of release of the drug, the main active ingredient is hydrocortisone - a hormonal substance that relieves itching, redness and irritation of the skin.

Ointment

In addition to the active substance hydrocortisone, the ointment uses a vaseline base, which also has a number of useful properties.

Firstly, Vaseline is oily and serves as a conductor, passing molecules of the active substance through the pores of the skin.

Secondly, it has a protective effect, enveloping the epidermis and creating a breathable film from negative external factors. Quickly has an analgesic effect and relieves itching.

Cream

Softens and nourishes the skin.

In addition to hydrocortisone, the composition contains cytostearyl alcohol, which has a disinfecting effect.

Paraffin is used to protect against moisture, microbes, and ultraviolet radiation.

Emulsion

The composition contains borage oil, which has a quick healing effect.

Thanks to its delicate texture and minimal amount of active ingredients, it can be used on the delicate skin of a child from 6 months.

It is produced in a convenient bottle, which allows you to apply the drug pointwise, for example, when the scalp is affected.

special instructions

Locoid Crelo emulsion should not be applied to the periorbital area, as this increases the risk of developing glaucoma. Systematic exposure of the conjunctiva to hydrocortisone butyrate can lead to increased intraocular pressure.

For patients with severe lichenification, dryness, and hyperkeratosis, it is preferable to use Lokoid in the form of Lipokrem or ointment.

It is not recommended to use the suspension for atrophic changes in the skin.

Factors that increase the risk of local/systemic side effects: application to large surface areas, prolonged use, application of occlusive dressings, childhood.

Lack of effect within 2 weeks of continuous therapy requires clarification of the diagnosis.

As with the use of other corticosteroids, Lokoid Crelo should be used for the shortest course at the minimum effective course dose sufficient to stop the pathological process.

Indications for use

Lokoid is a synthetic drug for external use.

Not all patients understand what Lokoid helps with and what is the mechanism of action of this medicine on the body.

It quickly relieves puffiness, thanks to the acceleration of tissue regeneration at the cellular level. The drug also accelerates blood circulation, relieves inflammation and itching. Absorbed in the granular layer of the epidermis when used externally.

To notice the result, you need to apply the cream or ointment systematically as prescribed by your doctor 1-3 times a day.

The course of treatment is determined individually, depending on the type of disease and characteristics of the body.

Prescribes the drug for the following diseases and symptoms:

  1. Psoriasis is a non-infectious disease that manifests itself as excessive drying of the skin.
  2. Eczema is an inflammation of the skin, manifested by dryness, the appearance of scales, pustules and pimples.
  3. Atopic dermatitis is a hereditary skin disease caused by allergies (it is often called diathesis in infants).
  4. Contact dermatitis is an acute form of allergic inflammation and skin rash.
  5. Seborrheic dermatitis is an inflammation of certain areas that are prone to oily skin (occurs due to disruption of the sebaceous glands or lack of hygiene).

A small amount of the active ingredient can penetrate into the blood, so before using any dosage form of Lokoid, you should consult your doctor and carefully read the instructions.

The drug has some contraindications:

  • hypersensitivity to the substances included in the composition;
  • the presence of wounds, infectious lesions, trophic ulcers;
  • cannot be used in the presence of mycoses, candidiasis and other diseases caused by fungal influence;
  • the presence of malignant and benign neoplasms on the skin.

During pregnancy and lactation , Lokoid preparations can be used if the potential benefit outweighs the possible risk. The doctor individually determines the dosage for each patient.

In order not to harm your health, before using the drug you need to be examined by a doctor and consult about the possibility (or impossibility) of its use.

Contraindications

  • bacterial skin infections (staphyloderma, streptoderma, gram-negative folliculitis, etc.);

  • Before application, the affected area should be washed with soap and water or wiped with an antiseptic drug, for example, Miramistin or Chlorhexidine.

  • Lokoid is applied 1-2 times a day with massaging movements to accelerate penetration into the deep layers of the epidermis.

  • The course of treatment is determined individually for each patient, but should not exceed 4 weeks for adults and 10 days for children from 5 years, and 7 days for children from 6 months to 5 years.

If after 1 week of daily use of the drug there are no changes for the better, you need to contact your doctor so that he can re-examine and prescribe another drug or supplement the course of treatment.

Reviews

Maria Ivanova, 49 years old, Moscow:

For 4 years now, my hands have been cracking to the point of bloody wounds. Doctors cannot accurately diagnose it; it is not eczema or psoriasis, perhaps an allergy. It is difficult to treat without an accurate diagnosis. The skin dries out greatly, especially after contact with water. I tried many zinc-based ointments and creams, but nothing helped. The dermatologist recommended Lokoid ointment; it is very oily and effectively softens the skin.

Yulia Ivashchenko, 29 years old, Minsk:

I used Lokoid emulsion to treat the child. Initially, I didn’t consult with doctors and started using it against redness from diapers, then against seborrhea. When the child’s scalp was too flaky, they turned to the pediatrician, but he said that there was no need to stop using Locoid.

The first results from using this product were visible after 4 days. After 1.5 weeks, all symptoms and peeling disappeared.

Use in childhood

Locoid Crelo is approved for use in children from 6 months of age. If it is necessary to apply the emulsion to the face or under occlusive dressings (diapers, nappies), it is necessary to reduce the duration of the course of therapy. The application surface area should not exceed 20% of the child’s total body area.

In children, the development of suppression of adrenal cortex function may occur faster than in adults. Excretion of growth hormone may also be reduced. Therefore, long-term use of the drug requires monitoring of body weight, height and plasma cortisol levels.

During the observation of children who received Locoid Crelo at a weekly dose of 30–60 g for 4 weeks, compared with those who received hydrocortisone ointment 1%, dysfunction of the adrenal cortex was not detected in any child. However, in terms of this indicator, no differences between hydrocortisone 17-butyrate and conventional hydrocortisone were observed.

Analogs

There are several types of analogues. The first is direct analogues - these are drugs that contain the same active ingredient as the main drug (in this case, hydrocortisone).

The following drugs fall into this category:

  1. Buticort ointment based on hydrocortisone is a synthetic corticosteroid for topical use in dermatology. Used for dermatitis, miliaria, seborrhea, psoriasis. It is prohibited to use for mycoses and open wounds due to the risk of adverse reactions. When using, avoid contact with mucous membranes. When using for children, you should first consult with your pediatrician.

  2. Cortiderm. The ointment is used in dermatology for eczema, dermatitis and allergic rashes. It is advisable to use for cracks and severe drying of the upper layer of the epidermis. Essential oils and stearic acid are used as additional components of the composition, which have a softening and moisturizing effect.

  3. Laticort - cream and ointment - dermatological preparations based on hydrocortisone. The composition also includes citric acid and cetostearyl ester, which additionally have an anti-inflammatory effect. Used for dermatitis of non-infectious origin.

The second group of analogues are indirect . These are generics that have different compositions, but they also work and are used for the same diseases as Lokoid:

  1. Alergocort – a drug belonging to the pharmacological group of corticosteroid drugs. The main active component is triamcinolone, which has an anti-inflammatory effect.

    The drug relieves itching and redness at the site of dermatological diseases - rashes, cracks, abrasions, diaper rash. It is forbidden to take for infectious diseases - smallpox, chickenpox, scab. In individual cases, it is used in pediatrics to treat children from 1 year of age.

  2. Polcortolone is a dermatological drug based on triamcinolone. Not recommended for use by children under 6 years of age and pregnant women. Widely used in dermatology for dryness, cracks, psoriasis. It does not help with infectious diseases and fungal pathologies because it does not contain antifungal substances.
  3. Afloderm cream is also a synthetic corticosteroid drug used in dermatology. The active substance is alkomethasone dipropionate, which quickly and effectively relieves inflammation. When applied, it eliminates itching and rashes, moisturizes the skin, and promotes healing.

Let us note once again that Lokoid is a synthetic corticosteroid drug that is widely used in dermatology.

It has a number of advantages and disadvantages that you need to familiarize yourself with first so as not to harm your health.

Properties

Hydrocortisone butyrate is a synthetic corticosteroid with known therapeutic activity. The main effects of the drug will be the following:

  • Anti-inflammatory.
  • Antiallergic.
  • Decongestant.

The steroid substance affects the metabolism of arachidonic acid, inhibiting the formation of prostaglandins at the site of inflammation. Under the influence of hydrocortisone, exudation and infiltration are reduced, cell membranes are stabilized and histamine production is reduced. The latter becomes decisive in ensuring the antipruritic effect.

Thus, the drug suppresses inflammatory reactions and allergies, which is manifested in a decrease in swelling, redness and itching. The skin elements become less intense and undergo reverse development. And the unique formula of Lokoid Crelo emulsion allows it to be used for weeping processes and damage to the scalp.

The main component of the drug Lokoid Crelo is the synthetic steroid hydrocortisone, which provides therapeutic effects.

Side effects of the drug Lokoid

local reactions (skin and subcutaneous tissue):

  • skin atrophy (thinning of the epidermis, telangiectasia, purpura and striae);
  • rosacea-like and perioral dermatitis;
  • rebound effect, making corticosteroid withdrawal difficult;
  • slower wound healing;
  • effect on the eyes: increased intraocular pressure and the risk of developing cataracts (if the drug is systematically exposed to the conjunctiva);
  • depigmentation, hypertrichosis;
  • contact allergy.

Systemic effects . In adults, when GCS is used topically, they are very rare, but they can be serious. With prolonged use of the drug, suppression of the function of the adrenal cortex may occur.

Pharmacodynamics and pharmacokinetics

  • Suction. After application, the active substance accumulates in the epidermis, mainly in the granular layer; systemic absorption is negligible. A small amount of hydrocortisone 17-butyrate is absorbed into the systemic circulation unchanged.
  • Metabolism. Most hydrocortisone 17-butyrate is metabolized to hydrocortisone and other metabolites directly in the epidermis and subsequently in the liver.
  • Excretion. Metabolites and a small part of unchanged hydrocortisone 17-butyrate are excreted by the kidneys and through the intestines.
