  • During therapy or after its completion, the risk of a complication, OHSS, cannot be excluded. How long does stimulation last for IVF?

The same protocol affects women differently, so the timing always varies. Ideally, a short protocol lasts 4 weeks, and the stimulation itself lasts 10–12 days. In this case, the size of the follicles must be monitored. They should increase by 2 mm daily.

In the presence of ovarian diseases, women require preliminary hormonal treatment. The long protocol provides 30 days of preparatory period and 10 days of active stimulation.

Dosage

Orgalutran is used in combination with other drugs. To stimulate ovulation, medications containing FSH are prescribed. They begin to be used on days 2 or 3 of the cycle. Orgalutran is introduced into the treatment regimen on day 6 after the start of FSH use, which corresponds to days 8 or 9 of the cycle. It is used once a day at a dose of 250 mcg.

Sometimes there is an increased response of the ovaries to stimulation of superovulation. In this case, it is recommended to administer Orgalutran on the 5th day of stimulation with gonadotropins. This corresponds to day 7 or 8 of the menstrual cycle.

It happens that the ovaries react too weakly to stimulation, and the follicles grow more slowly than usual. In this case, it is allowed to use Orgalutran later than 6 days after the first administration of FSH. The optimal timing is determined by the doctor. At the same time, he is guided by the size of the follicles according to ultrasound data, as well as the level of hormones in the blood.

On average, Orgalutran is used for 5 days. When using the drug, the probability of an increase in LH to more than 10 IU/ml, as well as progesterone to 1 ng/ml or more, does not exceed 1.2%. Thus, the effectiveness of Orgalutran is comparable to GnRH agonists. When using them, the risk of achieving a LH peak is about 0.8%.

In women with a pronounced ovarian response to stimulation, an excessive increase in the level of luteinizing hormone is possible until the 6-7th day of the cycle, when Orgalutran is not yet introduced into the therapy regimen. Complications of stimulation

1-2 hours after a single subcutaneous administration of the drug Orgalutran® at a dose of 250 mcg, the Cmax of ganirelix in the blood plasma is about 15 ng/ml. The absolute bioavailability of the drug Orgalutran® after subcutaneous administration is 91%.

Pharmacokinetic parameters after repeated subcutaneous administration of the drug Orgalutran® (1 time/day daily) are similar to those after a single administration. After repeated administration at a dose of 250 μg/day, Css (approximately 0.6 ng/ml) was achieved within 2-3 days. Pharmacokinetic analysis demonstrates an inverse relationship between body weight and serum concentrations of Orgalutran®.

Metabolism and excretion

The main compound circulating in plasma is ganirelix. Metabolized by enzymatic hydrolysis with the formation of peptide fragments.

T1/2 is approximately 13 hours and clearance is approximately 2.4 l/h. The drug is excreted through the intestines (about 75% in the form of metabolites) and kidneys (about 22%; ganirelix is ​​the main compound found in urine).
