Antifungal drug. Clotrimazole, the active substance of Canesten spray, is an imidazole derivative and has a wide spectrum of antimycotic action. The mechanism of action of clotrimazole is associated with inhibition of ergosterol synthesis, which leads to disruption of the structure and functions of the cytoplasmic membrane of fungi. It has fungistatic or fungicidal activity depending on the concentration at the site of infection.

Canesten is active against dermatophytes, yeast-like, mold fungi, as well as pathogens of pityriasis versicolor (Pityriasis vers.) and erythrasma. Along with antimycotic, Canesten has an antimicrobial effect against Trichomonas vaginalis, gram-positive (streptococci, staphylococci) and gram-negative microorganisms (Bacteroides, Gardnerella vaginalis).

Canesten is an antifungal topical cream containing clotrimazole. The drug Canesten is characterized by a wide spectrum of action, its activity is expressed against mold and yeast fungi (including Prodoturola and Cladospporium spp., Asppergillus spp., Torulopsis and Madurella spp., Criptococcus neoformans, as well as Candida spp.), dermatophytes (including Microspporum spp., Epidermophyton floccosum, Trichophyton spp.). Dimorphic fungi (Histoplasma capsulatum, Coccidiodes immitis, Blastomyces dermatitidis) and Nokardia actinomycetes are also sensitive to the drug.

Clotrimazole, which is the active component of Canesten cream, belongs to the group of imidazole derivatives and has a fungicidal and fungistatic effect characteristic of this group. For example, clotrimazole inhibits the production of ergosterol, nucleic acids and proteins in the cells of sensitive microorganisms, which first leads to disruption of reproduction and development, and then to disruption of membrane integrity and death of fungi.

The minimum concentrations of clotrimazole that have a pronounced antifungal effect during the studies were up to 8 mg/l. Canesten, creating a level of at least 10-20 mg/l, exhibits a fungicidal effect against fungi of the genus Candida and dermatophytes.

Strains that are resistant to Canesten are rare. Operating principle

Canesten simultaneously has fungistatic and fungicidal (high concentrations) effects. In simple terms, it not only blocks the growth of fungi, but also their reproduction and causes cell death. The average course of treatment is about 4 weeks; upon completion (if clinical manifestations disappear), it is advisable to continue treatment for another 14 days.

The duration of therapy for erythrasma is 2-4 weeks, for lichen versicolor - 1-3 weeks.

Before lubricating the feet, wash them with warm water and soap and dry them thoroughly, especially between the toes. For fungal infections of the skin of the legs, after achieving a therapeutic effect, it is recommended to continue treatment for 2-3 weeks.

Vaginal cream or gel is administered in portions of 5 g deep into the vagina once a day for 3 days. For candidal vulvitis or balanitis, use 2-3 times a day for 1-2 weeks. Vaginal tablets are inserted deep into the vagina: 500 mg once, or 200 mg for 3 days, or 100 mg for 6-7 days (once a day, in the evening). The course of treatment is usually combined with treatment of the external genitalia with a cream, ointment or solution. If necessary, the course of treatment can be repeated. For sanitization of the birth canal, a single administration of one tablet is recommended. For urethritis, a 1% solution is instilled into the urethra for 6 days.

To prevent reinfection, simultaneous treatment of sexual partners is necessary. In the second and third trimesters of pregnancy, the use of a vaginal applicator is undesirable. For trichomoniasis, it is recommended to combine treatment with chemotherapy drugs (metronidazole orally).

After applying the cream, occlusive dressings should not be applied.

Contact with the mucous membrane of the eyes should be avoided.

In patients with liver failure, the functional state of the liver should be periodically monitored.

If signs of hypersensitivity or irritation appear, treatment is stopped.

If there is no effect within 4 weeks, the diagnosis should be confirmed.

Directions for use and dosage

Canesten spray should be sprayed externally and the cream should be applied, gently rubbing, onto previously cleansed and dried areas of the skin. Frequency of application – 2-3 times a day.

The duration of treatment is determined taking into account the type of pathogen and the nature of the disease. Average duration of therapy depending on the disease:

  • Candidiasis of the external genitalia – 1-2 weeks;
  • Dermatomycosis – 3-4 weeks;
  • Tinea versicolor – 1-3 weeks;
  • Erythrasma – 2-4 weeks.

Treatment of fungal diseases of the skin of the legs should be continued for another 2 weeks after the symptoms disappear.

Canesten tablets are intended for intravaginal use - they are administered in 1 piece. per day before bedtime, deep into the vagina while lying on your back. An applicator is included for ease of administration.

Duration of treatment depending on the dose of tablets:

  • 100 mg – 6 days;
  • 200 mg – 3 days;
  • 500 mg – 1 day.

If the infection affects not only the vagina, but also the external genitalia, vaginal tablets are recommended to be combined with Canesten cream. Canesten is a drug based on clotrimazole. This active substance is the first-line drug of choice for diseases caused by yeast fungi of the genus Candida.

The drug Canesten is available in three forms:

  • cream;
  • vaginal tablets;
  • spray.

The cream is intended for the treatment of skin infections. It contains clotrimazole (0.01 g of active ingredient per 1 g), alcohols, water, polysorbate. The cream is available in tubes of 20 g.

Canesten vaginal tablets and suppositories are available in two dosages - 100 and 500 mg of clotrimazole in one suppository (tablet). Auxiliary and formative components in the composition:

  • lactic acid;
  • lactose;
  • calcium lactate;
  • starch;
  • shell components.

The drug in a large dosage (500 mg) is available in packages of one vaginal tablet. The product in a lower dosage (100 mg) is available in 6 tablets in a cardboard package. Each package contains applicators for convenient administration of the medication into the vagina.

The spray contains 1 g of clotrimazole per 100 g (1% solution) and excipients. It is available in 30 ml bottles.

Instructions

Most often, the drug is prescribed for the treatment of skin mycoses in the form of a cream. Spray and vaginal tablets are used in the treatment of vaginal candidiasis and fungal infections of the groin area (in particular the mucous membrane of the external genitalia).

For adults

Canesten in any dosage form should be applied only with clean hands to an area that has been previously cleaned of contaminants.

For children

There is no experience of use in children. Therefore, self-prescription of the drug is prohibited. The decision to treat a child with Canesten cream should be made by a doctor (vaginal forms, by definition, are not suitable for young children).

During pregnancy and lactation

Canesten can be used externally during pregnancy (in the 2nd and 3rd trimester) exclusively as prescribed by an obstetrician-gynecologist.
