Pharmacological properties

Antiparkinsonian drug, penetrates the blood-brain barrier. Blocks central M-cholinergic receptors, inhibits the transmission of excitation in the autonomic nervous system. It affects the neurons of the striatum, the activity of which decreases.

Intended for oral administration. Quickly stabilizes the patient's condition, eliminates tremor of the limbs, muscle rigidity, and catalepsy. The anticholinergic effect of the peripheral plan is manifested at an insignificant level.

Take this medicine strictly as directed by your doctor. An overdose of biperiden can lead to negative consequences. At the same time, it is impossible to predict in advance which organ will be at risk.

High solubility and binding to blood plasma proteins (90–94%) ensures good penetration of the active substance into organs and tissues. The required concentration is achieved after 15–40 hours. Akinetone is released only in the form of derivative compounds: piperidine and bicycloheptane.

Eating does not affect the absorption of the active and excipients. Akineton penetrates well into all biological fluids of the body. Absorbed from the gastrointestinal tract and metabolized in the liver, excreted by the body in feces and urine within 24 hours. In people over 65 years of age, this time can increase to 38–40 hours, which is associated with reduced renal function characteristic of this age.

Release form and composition

Akineton is available in the form of flat, cylindrical, white tablets and in the form of a clear solution for injection.

Each tablet contains two milligrams of biperiden hydrochloride, the main active ingredient. The auxiliary components included in the tablets are: microcrystalline cellulose, potato and corn starch, calcium hydrogen phosphate dihydrate, lactose manohydrate, copovidone, magnesium stearate, distilled water, talc.

The tablets are packaged in blisters of ten or twenty pieces.

In the solution for injection, one milliliter contains five milligrams of biperiden lactate, the main active ingredient. And the auxiliary components in the solution include: sodium lactate and distilled water. The solution for injection is packaged in ampoules of one milliliter. One package contains five ampoules with injection solution.

Indications for use

Recommended for the correction of extrapyramidal disorders, it is effective and has a “mild” effect. Oral administration of Akineton and its analogues in tablets (Benzac, Mendylex, Biperidine) is indicated:

  • adult patients with Parkinson's disease;
  • with a complex of extrapyramidal disorders caused by taking antipsychotics (tremor, dystonia, tics, chorea, myoclonus, cardiopathy, etc.);
  • in case of intoxication with phosphorus-containing substances or nicotine.

Increased sensitivity may develop, which is typical for older people with disorders of the central nervous system and a tendency to epilepsy.

Patients with tardive dyskinesia, which develops due to systemic use of antipsychotics, are at risk. The drug enhances the manifestations of erratic movements of a choreo-like nature.

Directions for use and dosage

In case of Parkinson's disease in severe form of Akineton, it is important for an adult patient to administer Akineton slowly, using intravenous droppers, or intramuscularly in small doses - two to four milliliters, which should be divided into several injections per day.

Taking Akineton in tablets begins with small dosages, no more than one milligram twice a day. Then, depending on the therapeutic effect, the dosage can be gradually increased to two milligrams twice a day. The maximum dosage of this drug should not exceed sixteen milligrams per day, divided into four doses.

A single dose of this drug can be prescribed in case of disorders of the body’s motor system:

  • for children under one year of age, 0.2 ml intravenously;
  • from one to six years, 0.4 ml intravenously;
  • over ten years old, 0.6 ml intravenously;
  • for adult patients, 0.5-1 ml intravenously (but not more than four ml per day).

If necessary, a repeat dose of Akineton is administered no earlier than thirty minutes after the first administration of the drug. If a side effect is observed, the administration of Akineton must be stopped.

Tablets of this drug are taken during food intake or immediately after it.

The duration of treatment is determined by the doctor and directly depends on the severity of the disease.

Discontinuation of the drug should occur gradually, by reducing the dosage.

Overdose

The effect of the drug is accompanied by dilation of blood vessels, a decrease in blood pressure, increased contraction of smooth muscles, and increased secretion of the salivary and sweat glands.
In case of an overdose (daily or one-time), a toxic effect occurs. Main symptoms:

  • hyperemia;
  • cardiac dysfunction;
  • temperature fluctuations;
  • psycho-emotional disorders (hallucinations, confusion, memory loss, overexcitation).

In some cases, collapse, cardiac arrest, depression of the respiratory system, and death are possible.
Treatment
A standard dosage of Physiostigmine, a reversible acetylcholinesterase inhibitor, is required. In emergency cases, resuscitation measures may be required.
