Pharmacological characteristics

Glucocorticosteroids for external use prevent the accumulation of certain leukocytes, reduce the release of inflammatory exudate and lymphokines, and also stop the circulation of cells that can absorb some particles, crush them and make infiltration less intense.

GCS has antipruritic, anti-inflammatory, antiexudative and antiallergic properties.

Pharmacodynamics and pharmacokinetics

The drug Dermovate belongs to the group of glucocorticosteroids and is used for external (local) use. Application of ointment or cream prevents the marginal position of neutrophils in the vascular bed, reduces the production and inflammatory effusion of lymphokines , prevents the migration of macrophages granulation and infiltration processes , thereby providing anti-inflammatory, antiexudative, antiallergic and antipruritic effects of a local nature.

The average Cmax of clobetasol is observed in plasma 13 hours after the first application of 30 g of the drug in the form of an ointment to healthy skin (or after 8 hours with repeated application) and is equal to 0.63 ng/ml.

The average Cmax of clobetasol when reapplying 30 g of the drug in cream form after 10 hours is slightly higher than when applying an ointment.

After a single application of 25 g of ointment after 3 hours in patients suffering from eczema and psoriasis the average Cmax of the drug in plasma is, respectively, 4.6 ng/ml and 2.3 ng/ml.

The metabolism of the active substance of the drug Dermovate has not been fully studied.

Side effects

When using the drug Dermovate topically, hypersensitivity was very rarely observed.

Among the local manifestations of hypersensitivity, the following were noted: rash, erythema, itching, local burning sensation of the skin, urticaria and contact dermatitis of allergic etiology, which occur at the sites of application of the drug and are similar to those symptoms for the treatment of which the drug was prescribed.

If hypersensitivity symptoms occur, therapy with Dermovate should be discontinued.

On the part of the endocrine system, symptoms of hypercortisolism , which manifest themselves when using a large amount of ointment or cream, as well as when applying the drug to large areas of the skin, and are a consequence of significant systemic absorption of the drug. These manifestations are most likely in patients of childhood and infancy, as well as when using a sealed bandage after applying the drug. It is worth remembering that in infants, the use of a diaper can act as an airtight bandage.

When adult patients use doses of the drug not exceeding 50 g per week, inhibition of pituitary or adrenal function is temporary, with the return of normal parameters of these organs after discontinuation of a short course of therapy using local glucocorticoids .

Rarely, when Dermovate was prescribed, dilation of the superficial vessels of the circulatory system was noted, especially with intensive rubbing of the drug into the folds of the skin or the use of occlusive dressings.

atrophy and atrophic streaks were rarely observed in the skin and subcutaneous tissues . Very rarely, there was an exacerbation of the main symptoms of the disease, changes in pigmentation, thinning of the skin, hypertrichosis and the development of psoriasis in the pustular form . These manifestations also most often occur when the drug is rubbed into skin folds or when occlusive dressings are used.

Local hormonal therapy

Hormonal ointments contain glucocorticosteroids, which have a strong effect on the endocrine system, reducing the production of hormones. The patient feels the effect of using such products immediately after application. Due to this property, steroid drugs are prescribed at the peak of exacerbation of the disease.

Hormonal ointments should not be used for more than one week, otherwise it can cause serious disruptions in the functioning of the body:

  • development of urolithiasis;
  • the appearance of addiction to the product;
  • skin pigmentation;
  • atrophic changes in the dermis;
  • hormone imbalance.

Instructions for use (Method and dosage)

The instructions for Dermovate recommend applying a thin layer of cream or ointment to the affected areas of the skin, once or twice a day, depending on the severity of the lesion.

Dermovate cream is more suitable for pathologies with severe inflammation and weeping , while the instructions for use recommend using Dermovate ointment for diseases accompanied by dry skin.

The course of therapy with Dermovate is determined individually based on the diagnosed disease and its severity, but should not exceed 4 weeks, without further assessment of the advisability of its continuation.

In case of exacerbation of previously treated skin diseases, repeated short courses of therapy with Dermovate may be prescribed.

If necessary, if the external manifestations of the disease do not completely disappear, after completing the course of treatment with Dermovate, they switch to the use of less aggressive external agents from the group of glucocorticosteroids.

In particularly severe cases of skin lesions, especially in the presence of hyperkeratosis, the anti-inflammatory effect of Dermovate ointment and cream can be enhanced by using plastic film as a dressing at night. An occlusive dressing is applied to the affected area for a greater positive effect, if achieved, further progress in treatment can be maintained without applying an occlusive dressing.

The maximum recommended weekly dose of ointment or cream is 50 g per week.

How to use the product correctly

For psoriasis, the use of Dermovate ointment is not uncommon. The product can quickly cope with the existing problem, but do not forget about the rules of use and basic precautions. It will be very good if a person uses the specified ointment, guided by the following recommendations:

  • Apply only to clean, dry skin;
  • Wash your hands before each application of medicinal ointment;
  • when using this product and other creams, observe the time interval before applying each;
  • do not apply the product to healthy areas of the skin;
  • avoid contact of the medication with the mucous membranes of the nose and eyes;
  • do not use after the expiration date;
  • Complete the prescribed course of treatment completely, regardless of the effectiveness of the results.

The ointment should be used extremely carefully. Before purchasing a product, you should pay attention to the conditions in which it was stored and check for quality certificates. If discomfort occurs during treatment of psoriasis with this drug, a person should notify the doctor about this effect.

Dermovate should be kept away from sunlight and children. This medication must be used daily.

In cases of chronic overdose, due to prolonged use of the drug or the use of large doses, the development of hypercortisolism syndrome , which is an indication for reducing the dose of Dermovate or discontinuing therapy using local corticosteroids. Cancellation of therapy should be carried out under the constant supervision of the attending physician, due to the risk of possible development of adrenal insufficiency.

For liver dysfunction

In patients with liver dysfunction, in the case of systemic absorption of the active substance Dermovate (when applied to large areas of skin over a long period of time), its metabolism and elimination rate slow down somewhat, which can significantly increase the risk of developing systemic toxicity. Therefore, in patients of this category, the drug is used in small quantities for a limited period of time, while paying special attention to achieving the required clinical effect.

special instructions

Therapy using drugs from the group of glucocorticosteroids is not recommended for a long period of time, especially when prescribing this group of drugs for the treatment of patients of young childhood, due to the possible suppression of the function of the adrenal cortex.

The use of Dermovate drugs in childhood requires mandatory monitoring of the patient's condition by the attending physician, at least once every 7 days.

As a result of long-term treatment with glucocorticosteroids, atrophic changes in the skin may appear, most often more pronounced on the skin of the face.

If it is necessary to apply ointment or cream to the eyelids, it is necessary to protect the mucous membrane of the eyes from contact with the drug, as this can lead to the development of glaucoma.

Therapy of psoriasis with the use of Dermovate may be accompanied by relapses of the underlying disease, the development of a generalized pustular form of pathology, increased tolerance to the drug, systemic and local toxic reactions occurring due to violations of the barrier functions of the skin, and therefore, constant monitoring of the patient’s condition is necessary .

If signs of a secondary infection joining the underlying disease are detected , appropriate antibacterial treatment is prescribed .

For any symptoms indicating generalization of infection , therapy should be discontinued and adequate treatment using antibacterial drugs should be prescribed.

It should be remembered that when using an occlusive dressing, moist, warm conditions are created that contribute to the development of bacterial infection , and therefore it is necessary to thoroughly clean the affected areas of the skin before applying the next dressing.

It is not recommended to dilute the drug Dermovate in the form of a cream.
